BACKGROUND Effective preventive treatments for dental decay exist, but caries experience among preschoolers has not changed, with marked disparities in untreated decay. Despite near-universal use of SMS text messaging, there are no studies using text messages to improve the oral health of vulnerable children. OBJECTIVE This randomized controlled feasibility trial aimed to test the effects of oral health text messages (OHT) versus a control (child wellness text messages or CWT). OHT was hypothesized to outperform CWT on improving pediatric oral health behaviors and parent attitudes. METHODS Parents with a child aged &lt;7 years were recruited at urban clinics during pediatric appointments (79% [41/52] below poverty line; 66% [36/55] black) and randomized to OHT (text messages on brushing, dental visits, bottle and sippy cups, healthy eating and sugary beverages, and fluoride) or CWT (text messages on reading, safety, physical activity and development, secondhand smoke, and stress) groups. Automated text messages based on Social Cognitive Theory were sent twice each day for 8-weeks. Groups were equivalent on the basis of the number of text messages sent, personalization, interactivity, and opportunity to earn electronic badges and unlock animated characters. Assessments were conducted at baseline and 8 weeks later. Data were analyzed with linear mixed–effects models. RESULTS A total of 55 participants were randomized (28 OHT and 27 CWT). Only one participant dropped out during the text message program and 47 (24 OHT and 23 CWT) completed follow up surveys. Response rates exceeded 68.78% (1040/1512) and overall program satisfaction was high (OHT mean 6.3; CWT mean 6.2; 1-7 scale range). Of the OHT group participants, 84% (21/25) would recommend the program to others. Overall program likeability scores were high (OHT mean 5.90; CWT mean 6.0; 1-7 scale range). Participants reported high perceived impact of the OHT program on brushing their child’s teeth, motivation to address their child's oral health, and knowledge of their child's oral health needs (mean 4.7, 4.6, and 4.6, respectively; 1-5 scale range). At follow up, compared with CWT, OHT group participants were more likely to brush their children’s teeth twice per day (odds ratio [OR] 1.37, 95% CI 0.28-6.50) and demonstrated improved attitudes regarding the use of fluoride (OR 3.82, 95% CI 0.9-16.8) and toward getting regular dental checkups for their child (OR 4.68, 95% CI 0.24-91.4). There were modest, but not significant, changes in motivation (F1,53=0.60; <italic>P</italic>=.45) and self–efficacy (F1,53=0.24; <italic>P</italic>=.63) to engage in oral health behaviors, favoring OHT (<italic>d</italic>=0.28 and <italic>d</italic>=0.16 for motivation and self–efficacy, respectively). CONCLUSIONS The OHT program demonstrated feasibility was well utilized and appealing to the target population and showed promise for efficacy.

Abstract Background: Oral health is considered as a prominent factor that contribute to quality of life. Hormonal changes during pregnancy can influence oral health. Message framing can play an important role in motivating oral health behaviors. The aim of the present study was to investigate the effect of message framing on the oral health behaviors and dental plaque among pregnant women.Methods: This randomized field trial was conducted in 2017 on 108 pregnant women in Izeh county, Iran.The participants were randomly divided into three groups; gain-framed, loss-framed, and control group. The research instrument included a two parts questionnaire containing demographic information and oral health knowledge, attitude, behavioral intention, self-efficacy, practice, and dental plaque index. Gain-and loss-framed messages were sent to the intervention groups via cell phones, but the control group did not receive any messages. The participants’ dental plaque were clinically assessed. Data were analyzed using SPSS version, 23.0. P-value less than 0.05 was considered statistically significant.Results: The mean scores of knowledge, attitude, behavioral intention, and self-efficacy were significantly increased in the gain- and loss-framed intervention groups (gain knowledge (6.05±1.90; CI:5.40-6.71); gain attitude(5.00±3.61; CI:3.75-6.24); gain behavioral intention (2.91±1.59; CI:2.36-3.46); gain self-efficacy (4.42±2.69; CI:3.50-5.35); loss knowledge (5.71±1.52; CI:5.19-6.23); loss attitude (4.54±2.67; CI:3.62-5.46); loss behavioral intention (2.25±1.94; CI:1.58-2.92); loss self-efficacy (3.97±2.60; CI:3.07-4.86) (p<0.001). No significant difference was observed in the control group (knowledge (0.38±1.45; CI:0.12-0.89, p=0.135); attitude (0.88±3.63; CI:0.38-2.14, p= 0.166); behavioral intention (-0.38±1.74; CI: -0.98-0.22, p=0.2); self-efficacy (.0.50±3.23; CI: -1.62-0.62, p=0.37). The rising trend in the mean score of the behaviors in the intervention groups was significantly higher than that in the control group (p<0.001). The ratio of dental plaque on teeth surfaces in the gain- and loss-framed intervention groups also showed a significant decrease compared to control group (gain dental plaque (16.46±11.66; CI:12.46-20.47, p<0.001); loss dental plaque (15.06±8.52; CI:12.13-17.99, p<0.001). Conclusion: Text message intervention improved knowledge, attitude, behavioral intention, self-efficacy, and dental plaque among pregnant women. The results show that text messages mattered, but the framing did not"; because there was a difference between intervention and control groups but not that "message framing matters" because the particular framing of the messages had no pronounced effect.

Abstract Background : Although many researchers, in different social sciences fields, are being attracted to the new potential promotional phenomenon, the influence of peer connections, the literature in dental public health is very poor. This article investigates the role of peer influence on individuals’ oral health knowledge and behavior among adolescents. Methods: The purpose of the study was to explore the correlation of secondary school students’ oral health behaviors and knowledge within friendship groups. A valid and reliable questionnaire was developed. A total of 421 students (228 boys and 193 girls) aged between 12-13 years old participated in the study. Ordinal regression analysis was used to identify the correlation between an individual’s oral health knowledge and behaviors and his/her friends. Results: The results demonstrated a strong relationship between students’ oral health behaviors and their peer connections within their friendship groups at school. Specifically, tooth brushing frequency of a student had a strong correlation with similar behavior of his/her friends. In addition, investigation of the role of the second level connections (friends of one’s friend with no direct connection) showed a significant correlation in brushing behavior. However, the results revealed no strong correlation of oral health knowledge among friends within their social network. Conclusion: This paper highlighted the importance of the role of peer influence on oral health behavior. Understanding the relation between oral health behavior and social network would help policy makers for more cost effective oral health promotion programs among adolescents.

The aim of this pilot study was to investigate the acceptability of an oral health-related mobile application developed for young children based on the feedback given by the children and their parents. Another aim was to evaluate the self-reported change in children’s oral health behaviors during a short test period. The application—a virtual pet integrated into a child’s daily routines—aimed to promote oral hygiene and dietary behaviors in children. A total of 36 4–12-year-old voluntary children were given a mobile phone with the installed application. After the 5-week testing period, the feasibility of the application and possible changes in the children’s oral health behaviors were asked using an electronic questionnaire. Most of the children considered the application clear (n = 34), amusing (n = 31), and useful (n = 29). The children’s tooth brushing manners improved both qualitatively and quantitatively: the time used for tooth brushing increased and the children learned how to brush different tooth surfaces. Mobile applications can be fun and useful in oral health promotion; while playing, children can learn good oral health-related behaviors. Mobile applications integrate oral health promotion into children’s daily environment and routines.

Abstract Adherence to health behaviors is often poor, including adherence to at-home technology-based interventions. This study (N=120) explored adherence to a cognitive training intervention delivered via computer tablet, assessed adherence over a 4.5 month period, explored how individual difference factors shaped adherence, and tested the efficacy of message framing manipulations (positive vs. negative framing) in boosting adherence. Individual difference factors predicted adherence, including variations in self-efficacy and belief in the efficacy of cognitive training. Overall message framing had little impact. However, during the final portion of the study in which participants were asked to play as much or as little as they wanted instead of following a schedule, participants who received positively framed messages engaged with the intervention more. Implications for predicting and boosting adherence to home delivered technology-based interventions will be discussed.
