The management of patients with traumatic injuries to their dentition poses a serious challenge in everyday general dental practice. For the rehabilitation of the complicated subgingival crown fracture of anterior teeth, multidisciplinary approach is often indicated. A combination of endodontic, orthodontic, periodontal and prosthodontic approach may be required. Orthodontic or periodontal intervention becomes an integral part for the exposure of the sound tooth structure of fractured anterior teeth with fracture line extending subgingivally. The aim of this paper is to discuss the immediate endodontic management followed by orthodontic extrusion of traumatized upper anterior teeth with fracture at the subgingival level. In order to expose the sound tooth structure for prosthodonticintervention, orthodontic extrusion was performed after endodontic treatment. To avoid extraction of the involved teeth, the multidisciplinary approach was adopted and finally the teeth were restored prosthodontically. The final result was esthetically pleasant and periodontically sound.

Complicated crown-root fractures account for a small percentage of traumatic dental injuries seen in children; however, management of these injuries can be very challenging to clinicians. Factors such as complexity of the injury, patient’s age and dentition stage, patient’s cooperation, and parental demands may have some bearing on the type of treatment undertaken and its outcomes. In some children, these injuries may have significant impact on their quality of life. The purpose of this article is to describe two cases of complicated crown-root fracture which were successfully managed through orthodontic extrusion using a sectional fixed orthodontic technique. The basis for the treatment technique and its favourable outcomes were highlighted with its advantages and drawbacks.

This article describes the management of a case of oblique crown root fracture of a central incisor where the fractured fragment was used as a temporary replacement crown in order to maintain esthetics during the period of orthodontic extrusion of the remaining root. Placement of the final restoration after orthodontic extrusion resulted in good esthetics and periodontal health postoperatively. The merits and demerits of this treatment modality are discussed.

The aim of this paper is to present a complex rehabilitation, of fractured tooth, with implants in anterior region considering the orthodontics extrusion to clinical success. At 7 years old, the patient fractured the maxillary left central incisor and the dentist did a crown with the fragment. Twenty years later, the patient was referred to a dental clinic for orthodontic treatment, with the chief complaint related to an accentuated deep bite, and a professional started an orthodontic treatment. After sixteen months of orthodontic treatment, tooth 21 fractured. The treatment plan included an orthodontic extrusion of tooth 21 and implant placement. This case has been followed up and the clinical and radiographic examinations show excellence esthetic results and satisfaction of patient. The forced extrusion can be a viable treatment option in the management of crown root fracture of an anterior tooth to gain bone in a vertical direction. This case emphasizes that to achieve the esthetic result a multidisciplinary approach is necessary.

The present case report describes a multidisciplinary approach involving endodontic, restorative, surgical and periodontal treatment and rehabilitation of a central incisor with crown-root fracture (CRF) in an 11-year-old child. Although in the future possible sequelae and complications should be considered since it is an extensive trauma, the prognosis of the present case is favorable, with the occurrence of repair, evidenced by clinical, radiographic and tomographic success, after one year and six months of follow-up. It was concluded that the association of prolonged flexible splint, biological endodontic treatment, gingivoplasty and restorative/rehabilitative procedures seem to be an adequate and conservative management for CRF cases.

ABSTRACT Objective To review the literature searching for a consensus for the choice of orthodontic extrusion as treatment for crownroot fracture. Materials and methods An electronic search was performed in the databases PubMed, Cochrane Central Register of Controlled Trials and Scopus and a manual search of the Journal Dental Traumatology. Results Forty articles were found in PubMed and 38 in Scopus and after removal of duplicate sample 51 contained articles. Of these, 48 were excluded for not having orthodontic treatment, no follow-up or follow-up less than 6 months, or not report the presence of crown-root fracture. In manual search in Dental Traumatology 20 articles were found, but none of them met the prerequisites established. So, three articles formed the basis of the study. Conclusion The choice of how to treat orthodontic extrusion of crown-root fracture was effective and stable, without root and periodontal changes. Factors, such as root formation and presence of pulp vitality were decisive for determining the stages of treatment, however, there is no consensus based on scientific evidence about these protocols.
