Objective To assess the potential relationship between benign prostate hyperplasia (BPH) and metabolic syndrome in men under 60 years old. Methods We searched the Medline, Embase, and Web of Science databases for studies of patients with metabolic syndrome and BPH using the key words ‘metabolic syndrome’, ‘benign prostatic hyperplasia’, and ‘BPH’. The odds ratios (ORs) and 95% confidence intervals (95%CIs) were extracted from the included studies and the role of metabolic syndrome in BPH and its characteristics (International Prostate Symptom Score (IPSS), total prostate volume (TPV), postvoid residual (PVR)) were evaluated by meta-analysis. Results Six comparative studies comprising 61,826 individuals were identified and included in this meta-analysis. There were significant correlations between metabolic syndrome and BPH (OR = 1.24, 95%CI = 1.19–1.29), clinical BPH (OR = 1.37, 95%CI = 1.03–1.70), and TPV (OR = 2.34, 95%CI = 1.25–3.42). However, there was no significant association between metabolic syndrome and IPSS (OR = 1.19, 95%CI = 0.35–2.04) or PVR (OR = 2.15, 95% CI = 0.95–3.34). Conclusions These results indicate that metabolic syndrome is significantly and positively correlated with the incidence of BPH in younger men aged <60 years. However, there was no significant relationship between metabolic syndrome and BPH-related symptoms.

Objective: This study was to evaluate the associations among metabolic components, prostate specific antigen (PSA), and prostate volume (PV) in benign prostate hyperplasia (BPH) patients.Material & method: During the period from January 1, 2010 to August 31, 2013, 61 mens were diagnosed with BPH were enrolled. PV by transabdominal ultrasonography of the prostate, serum PSA, metabolic syndrome (MS) related parameters were investigated. MS was defined according to the modified National Cholesterol Education Program Third Adult Treatment Panel Guidelines. We evaluated the correlation between PSA, PV and MS components using Spearman’s test and Independent t test.Results: PV were not correlated with MS criteria (p=0.591) PSA total also were not correlated with MS criteria (p=0.305). PV were not correlated with each components MS; cholesterol total (p=0.593), trigliserid (p=0.479), HDL (p=0.702), LDL (p=0.512), blood glucose (p=0.317). PSA total were not correlated with each components MS; cholesterol total (p=0.255), trigliserid (p=0.543), HDL (p=0.446), LDL (p=0.615), blood glucose (p=0.987). Conclusion: There were no associations among metabolic syndrome criteria and each components, prostate volume and PSA total.

Metabolic syndrome (MetS) is a highly prevalent complex disorder among adults worldwide which includes cardiovascular risk factors such as central obesity, insulin resistance, dyslipidemia and hypertension. MetS or its components are prevalent in patients with benign prostate hyperplasia (BPH), a common urogenital disorder of adult male with increasing age. This cross-sectional study was conducted in the Department of Biochemistry, Dhaka Medical College, Dhaka from July 2014 to June 2015 with a total of 100 subjects. Of these 50 patients of BPH attending the Department of Urology, Dhaka Medical College Hospital, Dhaka was taken as Group I and 50 age-matched apparently healthy adults was taken as Group II. According to the National Cholesterol Education Program Adult Treatment Panel (NCEP-ATP III, 2005) criteria, components of MetS as fasting plasma glucose, serum triglyceride, serum HDL-C level, blood pressure, waist circumference were assessed and compared between groups by chi-square (x2) tests and unpaired student's 't' test as appropriate. A p value <0.05 at 95% CI was considered as significant. Metabolic Syndrome was significantly more common in BPH patients than that of healthy individuals (72% VS 38%, p=0.001). Mean±SD of fasting plasma glucose and serum triglycerides were significantly higher in Group I than that of Group II (5.7±0.6 mmol/l vs 5.13±0.97 mmol/l, p=0.001; and 191.3±33.9 mg/dl vs 159.2±45.5 mg/dl, p=0.001, respectively). Percentage of subjects having high blood pressure, high serum triglyceride and low serum HDL-C were significantly high in BPH patients (Group I) than that of healthy individuals of Group II (BP: 72% VS 44%, p=0.005, TG: 90% vs 54%, p=0.001, HDL-C: 72% VS 54%,p=0.001). Metabolic syndrome or its components are more common among patients with benign prostate hyperplasia. Routine investigations to detect MetS can be done to reduce the risk of cerebrovascular diseases, cardiovascular diseases and type 2 diabetes mellitus in BPH patients.Bangladesh J Med Biochem 2015; 8(2): 42-48

BACKGROUND Studies evaluating the relationship between metabolic syndrome (MetS) and lower urinary tract symptoms (LUTS) in men with benign prostate hyperplasia (BPH) are lacking in Indonesia. This study aimed to discover the association of LUTS and MetS in men with BPH. METHODS Subjects who underwent biopsy were recruited from Cipto Mangunkusumo Hospital, Jakarta, Indonesia from January 2014 to January 2018, but only men who had biopsy-proven BPH were included. Body mass index, waist circumference, fasting blood glucose, triglyceride, high-density lipoprotein, prostate volume (PV), and international prostate symptom score (IPSS) were collected before the biopsy. MetS criteria were based on the National Cholesterol Education Program Adult Treatment Panel III. IPSS was assessed for LUTS and consisted of irritative and obstructive symptoms and quality of life (QoL). Independent t-test or Mann–Whitney test was used to analyze numerical data. RESULTS Of 227 men with biopsy-proven BPH, 87 (38.3%) were diagnosed with MetS. PV was similar in men with or without MetS (54.4 [20.3–100] versus 49.9 [19.5–100] cm3, p = 0.239). Men with MetS generally had more LUTS (15 [1–30] versus 11 [0–35], p = 0.005), more irritative symptoms (8 [0–20] versus 6 [0–20], p = 0.007), and lower QoL (4 [0–6] versus 3 [0–6], p = 0.018). CONCLUSIONS BPH patients with MetS had greater LUTS, particularly irritative symptoms and QoL score.

Pendahuluan. Benign Postate Hiperplasia (BPH) adalah penyakit yang umumnya terjadi pada pria lansia yang disebabkan oleh penuaan. Hiperplasia prostat adalah pertumbuhan jaringan nodul fibroadenomatosa pada prostat. Pembesaran prostat jinak merupakan penyakit yang tersering kedua setelah batu saluran kemih didapatkan secara klinis di Indonesia. Tujuan. Penelitian ini bertujuan untuk mengetahui hubungan antara usia, obesitas dan riwayat diabetes mellitus dengan kejadian Benign Prostate Hyperplasia (BPH) grade IV di Rumah Sakit Dr. M. Haulussy Ambon periode 2012-2014. Metode. Jenis penelitian ini adalah penelitian analitik desain Cross-Sectional, dengan menggunakan catatan medis data di ruang operasi di Rumah Sakit Dr. M. Haulussy Ambon Tahun 2012-2014 dan memperoleh jumlah sampel yang memenuhi kriteria inklusi sebanyak 239, yang diambil dengan teknik total sampling. Analisis dilakukan dengan analisis univariat dan bivariat menggunakan uji Chi Square. Hasil yang di temukan dalam penelitian ini bahwa kejadian BPH lebih besar pada mereka yang berusia> 65 tahun dan 56-65 tahun dibandingkan dengan usia 46-55 dan <46 tahun dengan hasil tes menunjukkan adanya hubungan antara usia dengan BPH dengan nilai (p= 0,000), ada hubungan antara obesitas dengan nilai BPH (p=0,019) dan riwayat diabetes mellitus setelah menggunakan uji Chi-Square, hubungan antara riwayat diabetes mellitus dengan BPH dengan nilai (p = 0,000). Kesimpulan. Ada hubungan antara umur, obesitas dan riwayat diabetes mellitus dengan kejadian BPH.
