The current global severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) pandemic has thrust intensive care medicine to the forefront of health care practice in Australia and New Zealand. Indeed, reports from other countries and jurisdictions convey highly confronting statistics about the scale of this public health emergency, particularly in terms of the demand on intensive care unit (ICU)services. Whether this occurs here remains to be seen, although if such a scenario does eventuate, it will represent an unprecedented challenge to our community. In parallel, these events offer the opportunity for greater coordination, improved communication, and innovation in clinical care, which are principles that in many ways define our specialty.

ABSTRACT INTRODUCTIONSweden is unique in adopting a ‘no-lockdown’ public health approach to the SARS-CoV-2 (COVID-19) outbreak. There were fears that health services would not be able to care for high numbers of COVID-19 patients. AIMTo describe and review the emergency response of a public primary and community health-care organisation in Stockholm, Sweden, to the demand for care for COVID-19 and non-COVID-19 patients during March–July 2020, and summarise preparations for the months to follow. METHODSThis was a rapid implementation action research case study, which also draws on one author’s experience as Chief Executive Officer and other members’ experience in an emergency management group. RESULTSSweden experienced similar mortality per million population to the UK, despite the different public health strategy used to address the COVID-19 outbreak. The Stockholm-integrated public primary and community health-care service, serving a population of 2.3 million, made many changes quickly. One change included coordinating non-acute private health-care services, following the local government emergency directive to do so. DISCUSSIONIt is possible that the fast and effective response by management and services in primary and community health care reduced infection and hospital demand, which contributed to a lower mortality than otherwise expected. The actions and preparations described for Stockholm’s response may provide ideas for other health-care systems. The partnership research approach between the Karolinska Medical University and the Region Stockholm health-care system used in this project shows that rapid research methods have advantages for both partners in an emergency situation.

Health development is a way of increasing awareness, willingness and ability to live healthy for everyone so that an optimal degree of public health can be achieved. The purpose of knowing how the relationship between nurse competence and community health care activities. This study employed quantitative approach and correlation analysis. The population was nurses who work at public health center in Makassar City, Indonesia, however there were 118 nurses who meet the criteria in their selection. There were relationships between attitudes, skills, and competencies with the level of implementation of community health services and that there is an interaction between competence and training. The results of the competency analysis obtained an OR value of 6.429, meaning that public health center nurses who have good competence have a chance of 6.429 times to carry out community health care activities optimally. Most dominant with the implementation of public health care is the interaction between competence and training. The competence of nurses need to be improved in order to optimize the implementation of community health services through training, coaching through assigned teams, and collaborating with peers and providing support in the form of policies for rewards and sanctions such as nurse career paths.

Innovation can come in many forms, this chapter states: changes in current processes that make them more efficient or effective; shifts toward different processes to achieve the same goal; or larger changes that may include rethinking the task itself. While innovation can be part of the normal work flow, innovation is disruptive when it changes established roles and relationships. The chapter looks at the role of innovation in aims at improving public health. Innovation is hard to define, and means different things to different people. Innovation is occurring in population health care practice. We should embrace the characteristics of innovation, the chapter concludes.
