Abstract Excessive gingival display space and gingival hyperpigmentation are major concerns for a large number of patients visiting the dentist. Melanin hyperpigmentation usually does not present a medical problem, but patients usually complain of dark gums as unaesthetic. This problem is aggravated in patients with a “gummy smile” or excessive gingival display while smiling. Esthetic periodontal plastic surgery is especially rewarding in such individuals with compromised esthetics. A case is reported here on the cosmetic correction of “black gums” and “gummy smile.” Periodontal plastic surgery combining gingival depigmentation and esthetic crown lengthening was performed in a single appointment using scalpel surgical technique. The outline of steps involved in the surgical procedure is demonstrated and a brief review of the various gingival depigmentation techniques is depicted here. Citation Roshna T, Nandakumar K. Introduction: Today, the appreciation of aesthetic dentistry by society brought to the search for materials and techniques that improve the appearance of smile, which is essential in maintaining physical and mental health of the patient. Objective: This study aimed to report a case of gummy smile involving Periodontics and Restorative Dentistry. Case report: This case, despite other possible alternatives, was solved through periodontal surgery (clinical crown augmentation associated with osteotomy) and indirect restorations with porcelain crowns. Conclusion: In our experience this procedure is safe, predictable, with minimal risks or side effects, and it is a treatment option for these cases.

Aim: The aim of this paper is to describe a clinical case in which the patient's gingival melanin pigmentation was treated using diamond burs and scalpels.Case report: The present study describes a periodontal plastic surgery procedure to correct or improve the amount of gingival melanin pigmentation apparent on mandibular and maxillary arch of the patient.Conclusions: After the surgical procedure successfully performed, and considering the high degree of satisfaction of the patient, the author concluded that the studied technique is easy to perform and stands out because of the excellent results achieved.

Altered passive eruption/gummy smile is a common challenge in patients requiring aesthetic treatment. A specific surgical protocol was designed and tested in patients with altered passive eruption. Standardized preoperative X-rays were used to assess crown length at baseline and to place submarginal incisions. Osseous respective therapy was performed to achieve biological width. Clinical outcomes were recorded 6 months after surgery. Eleven patients with a total of 58 teeth were treated with flap surgery and osseous resective therapy at upper anterior natural teeth. At the last followup, a significant and stable improvement of crown length was obtained when compared to the baseline (P<0.0001). All patients rated as satisfactory in the final outcomes (final VAS value = 86.6). In conclusion, this study showed that periodontal plastic surgery including osseous resection leads to predictable outcomes in the treatment of altered passive eruption/gummy smile: A careful preoperative planning avoids unpleasant complications and enhances postsurgical stability of the gingival margin.

Background: A hydrocele is a fluid - filled sac surrounding a testis that resultsin the swelling of scrotum. They can develop due to inflammation or injury within the scrotum.Objective: This study aimed to evaluate the efficacy and adaptation of different surgicalprocedures in the repair of hydrocele. Study design: Comparative, Retrospective study .Placeand duration of Study: The study was conducted at Fauji Foundation Hospital Karachi , Al-TibriMedical College, Karachi and Civil Hospital Karachi from January, 2000 to December, 2013.Patients and methods: Patients with primary vaginal hydrocele registered during the periodof thirteen years in the out patient department of surgery were selected. A total of 300 patientswere assessed clinically, diagnostically and radiologically about the status of hydrocele beforethe surgical procedures and anesthetic opinion. The different surgical technique were carriedout in different group of patients asJaboulay’s technique in 70 patients, Lord’s procedure in 70patients, Aspiration and Sclerotherapy in 05 patients, Window operation technique in 05 patientsand the Hydrocelectomyby supra pubic procedure were carried out in 150 patients out of total300 registered patients. The data collected and analyzed statistically in SPSS version 19.00.Results: The different procedures have been adopted surgically in patients with primary vaginalhydrocele. The results showed that among the different operative techniques adopted, the bestprocedure regarding hydrocelectomy is the supra pubic approach line of treatment. It showedbetter result and recovery with very minimum complications and side effects as compared toother surgical procedure. Conclusions: The data thus concluded that hydrocelectomy donevia supra pubic approach in number of patients proved to be the best procedure because ofhaving very little complications.

Periodontal Plastic Surgery comprises a limited number of surgical procedures which focus on the establishment of normal morphology and architecture of periodontal tissues in order to achieve aesthetical, biological and functional outcomes. The main surgical techniques concern the correction of morphology, position and amount of gingiva and in some cases the reconstruction and augmentation of alveolar ridge. Those surgical procedures include the treatment of gingival recession, surgical crown lengthening, augmentation of the width of attached gingiva and,the vertical or horizontal augmentation of alveolar ridge. Each surgical technique is followed by determined indications, contra-indications, advantages, disadvantages and, predictability.
