In recent years, many researchers have devoted time to designing algorithms used to introduce external information from knowledge graphs, to solve the problems of data sparseness and the cold start, and thus improve the performance of recommendation systems. Inspired by these studies, we proposed KANR, a knowledge graph-enhanced attention aggregation network for making recommendations. This is an end-to-end deep learning model using knowledge graph embedding to enhance the attention aggregation network for making recommendations. It consists of three main parts. The first is the attention aggregation network, which collect the user’s interaction history and captures the user’s preference for each item. The second is the knowledge graph-embedded model, which aims to integrate the knowledge. The semantic information of the nodes and edges in the graph is mapped to the low-dimensional vector space. The final part is the information interaction unit, which is used for fusing the features of two vectors. Experiments showed that our model achieved a stable improvement compared to the baseline model in making recommendations for movies, books, and music.

Abstract Background The year 2020 was dramatically characterized by SARS-CoV-2 pandemic outbreak. COVID-19-related heart diseases and myocarditis have been reported. Case summary A 45-year-old healthy male was admitted to the intensive care unit of our hospital because of cardiogenic shock. A diagnosis of COVID-19 infection and myocarditis was done. We present here several peculiarities about diagnostic workup, myocardial histological findings, choice of treatment, and the patient clinical course at 3 and 8 months of follow-up. Discussion COVID-19 myocardial damage and myocarditis are mainly linked to the cytokine storm with mild myocardial inflammatory infiltrate and very unusual platelet microclots in the setting of the microvascular obstructive thrombo-inflammatory syndrome. Counteracting the inflammatory burden with an interleukine-1 inhibitor appeared safe and led to a dramatic and stable improvement of cardiac function.

Attempts to curtail substance use problems have been directed at their virtually every aspect, from interdicting illegal drugs to investments in biomedical research. The problems’ persistence, however, including but not limited to the latest “opioid epidemic,” suggests that these costly measures have not resulted in a stable improvement in the past and do not promise lasting success in the future. Translation of research results, pertaining mainly to the factors that elevate risk and lead to disorder, has not been satisfactory. A review of relevant facts in the first of two connected articles indicates the need for changes in approaches to substance use and lays foundation to methodological research design protocols described in the second paper.

Covid-19, which has severely disrupted the economic systems in all countries, has led to significant changes in the financial structures of businesses. While the atmosphere of uncertainty created by the Covid-19 affected the businesses with weak liquidity structures the most, it also affected the financial and operational processes of the businesses in different ways. This study aims to examine the financial situation in the case of a basic food production business, which is within the scope of large and medium-sized enterprises, and evaluate its financial performance by the ratio analysis method within the framework of the Covid-19 effect. For this purpose, the liquidity status, asset use efficiency, financial structure, and profitability ratios of the enterprise before and during the Covid-19 period were examined and evaluated. It is seen in the study's result that the asset use and turnover rates of the enterprise are managed effectively and there is a stable improvement in the profitability ratios. It is observed that the liquidity ratios of the enterprise tend to decrease. We can say that the fact that the business was not among the sectors whose activities were restricted during the Covid-19 period and it operated in the basic food production area, which is a basic need for humans, is the reason why it was relatively less affected by the process.

AbstractTo analyze functional and anatomical response patterns to dexamethasone (DEX) implant in diabetic macular edema (DME), to describe proportion of responders and non-responders, and to propose a new DME grading system. Retrospective, multicenter, observational cohort study. Naïve and non-naïve DME patients were treated with DEX, with visual acuity (VA) ≥ 0.2 logMAR and central subfield thickness (CST) of ≥ 300 µm. Functional and anatomical responses were graded after 2 and 4 months, and categorized as early and stable improvement, early and progressive improvement, pendular response, delayed improvement, and persistent non-response. 417 eyes were included (175 treatment naïve eyes). Compared to non-naïve eyes, naïve eyes showed a very good functional response (VA gain ≥ 10 letters) more frequently after 2 and 4 months (56% and 57% [naïve] vs. 33% and 28% [non-naïve], p < 0.001). A VA gain < 5 letters (non-response) after 2 and 4 months was seen in 18% and 16% of naïve eyes, and in 49% and 53% of non-naïve eyes (p < 0.001). A lack of anatomical response was rare in both groups, but more frequently in non-naïve eyes (12% vs. 4%, p = 0.003). Functionally and anatomically, naïve eyes showed most frequently an early and stable improvement (functionally: 77/175 44%; anatomically: 123/175 eyes, 70%). Most non-naïve eyes experienced no significant improvement functionally (97/242 eyes, 40%), despite a mostly early and stable improvement anatomical response pattern (102/242 eyes, 42%). Functional but not anatomical response patterns were influenced by baseline VA. Naïve and non-naïve eyes show different functional and anatomical response patterns to DEX implant. Functional non-responders are rare in naïve eyes, whereas anatomical non-response is unusual in both groups.

Introduction: There are no reliable prospective studies on the effectiveness of topiramate in Bulgarian adult patients with drug-resistant epilepsy. Aim: The aim of the study was to conduct an open, prospective study on various aspects of topiramate (TPM) effectiveness in Bulgarian patients with drug-resistant epilepsy.&nbsp; Patients and methods: The study included patients with epilepsy who attended the Clinic of Neurology at the University Hospital in Plovdiv, Bulgaria. Patients completed diaries for seizure frequency, seizure severity, and adverse events. There were regular documented visits at 3 or 6 months during the first year of TPM treatment and at 6 months afterwards, with a dynamic assessment of seizure fre-quency, severity, adverse events, and EEG recordings.&nbsp; Results: TPM was used as an add-on treatment in 120 patients (69 males, mean age 37 years). There was a relatively mild and stable dynamic improvement of seizure severity, a satisfactory seizure frequency reduction in 37% of participants, a stable mean seizure fre-quency reduction (47%) from month 6 to month 24 of treatment and a stable responder rate (48-51%) during the same period. New seizure types (focal with impaired awareness with/without evolution to bilateral tonic-clonic seizures) occurred in 5 patients. There were adverse events (dizziness/vertigo, irritability, speech disturbances, memory impairment, concentration problems, tremor, loss of appe-tite and weight, weakness, numbness, bradypsychia, confusion, visual hallucinations, sleepiness, insomnia, headache, itching, unstable gait, nausea, and vomiting) in 20% of patients.&nbsp; Conclusions: TPM treatment is associated with low and stable improvement of seizure severity, good and stable improvement of sei-zure frequency, possible worsening of seizure control and appearance of new seizure types, good safety and tolerability.

Contact dermatitis is a type of skin eruption. It is caused by chemical or physical substance comes in contact with skin that causes allergic reaction. Contact dermatitis can be caused by different compound in home or work.  It is chronic or continuous, non-contagious in nature. Here, a18 year’s old girl presented with a popular eruption over flexor aspect of wrist joint and over both ear lobes over last 3 to 4 years. It was a diagnosed case of contact dermatitis. She was successfully treated by individualized Homoeopathic treatment with single medicine Calcarea Carb over 4 to 5 months. She initially did not show stable improvement on homoeopathic medicine selected on the basis of symptoms. On changing the medicine considering mental symptoms and physical general, she gradually showed good improvement and reached remission by 3 months. Remission maintained without any recurrence over the next 2 years. It shows positive role of Homoeopathic treatment in contact dermatitis in a young woman.
