We often use topical antibiotics for simple excisions or the skin after minor wounds. This is a long-standing practice to minimize scarring by preventing wound or surgical site infection to promote wound healing. Is this old practice the right way for today? Is there a scientific basis for it and can it persist? These questions are related to the increase in antibiotic-resistant bacteria in Korea. Currently, the guidelines for the use of oral antibiotics have been established and are being applied relatively well-controlled to prevent overuse and misuse, while discussions for evaluating the adequacy of the prophylactic use of topical antibiotics or establishing correct guidelines are lacking. Based on related research data, American Academy of Dermatology has been continuously campaigning against the abuse of prophylactic antibiotics by the public and doctors, following the increase in the abuse of topical antibiotics since 2013. Therefore, we discuss the necessity of using topical antibiotics for the prophylaxis based on literature evidence. Now is the time to make efforts to prevent the increase of antibiotic-resistant bacteria through the correct use and by reducing the prescription of topical antibiotics in Korea.

Summary There is growing concern about human-induced antibiotic resistance and on the occurrence of antibiotic-resistant, potentially pathogenic bacteria in the environment. The aim of this study was to investigate the incidence of resistant, clinically relevant bacteria at bathing sites. In total, 27 of 263 bathing sites authorized under the EU Bathing Water Directive (3 per Austrian state) were sampled during the summer of 2017. Samples were tested for antibiotic-resistant bacteria by enrichment in thioglycollate broth and cultivation on chromogenic media. The screening for potentially pathogenic antibiotic-resistant bacteria was negative in 23 of the 27 samples. Antibiotic-resistant bacteria were detected from 4 of the 27 bathing sites: one Pseudomonas aeruginosa and three resistant Enterobacteriaceae (piperacillin/tazobactam-resistant Enterobacter cloacae with high-level expression of AmpC beta-lactamase, carbapenem-resistant Enterobacter mori, extended-spectrum beta-lactamase-producing Escherichia coli). Despite the occurrence of resistant bacteria, we consider the public health risk at Austrian bathing sites as low.

Antimicrobial resistance is considered an important threat to global health and has recently attracted significant attention from the public. In this study, the concentrations and size distribution characteristics of culturable airborne total bacteria (TB) and four antibiotic-resistant bacteria (tetracycline-resistant bacteria (TRB), ciprofloxacin-resistant bacteria (CRB), erythromycin-resistant bacteria (ERB), and ampicillin-resistant bacteria (ARB)) were investigated for approximately one year to explore their variations under different seasons, diurnal periods, and air quality levels. The concentrations of TB and four antibiotic-resistant bacteria in winter and night were higher than during other seasons and diurnal periods. Their maximum concentrations were detected from air under moderate pollution or heavy pollution. PM2.5, PM10, SO2, and NO2 were positively related to TB and four antibiotic-resistant bacteria (p < 0.01), whereas O3 and wind speed were negatively related to them (p < 0.05). The particle size of TB and four antibiotic-resistant bacteria were mainly distributed in stage V (1.1–2.2 µm). Bacillus was the dominant genus of ARB (75.97%) and CRB (25.67%). Staphylococcus and Macrococcus were the dominant genera of TRB (46.05%) and ERB (47.67%), respectively. The opportunistic pathogens of Micrococcus, Sphingomonas, Enterococcus, Rhodococcus, and Stenotrophomonas were also identified. This study provides important references for understanding the threat of bioaerosols to human health.

In recent years, antibiotic-resistant bacteria with an impact on human health, such as extended spectrum β-lactamase (ESBL)-containing Enterobacteriaceae, methicillin-resistant Staphylococcus aureus (MRSA) and vancomycin-resistant Enterococci (VRE), have become more common in food. This is due to the use of antibiotics in animal husbandry, which leads to the promotion of antibiotic resistance and thus also makes food a source of such resistant bacteria. Most studies dealing with this issue usually focus on the animals or processed food products to examine the antibiotic resistant bacteria. This study investigated the intestine as another main habitat besides the skin for multiresistant bacteria. For this purpose, faeces samples were taken directly from the intestines of swine (n = 71) and broiler (n = 100) during the slaughter process and analysed. All samples were from animals fed in Austria and slaughtered in Austrian slaughterhouses for food production. The samples were examined for the presence of ESBL-producing Enterobacteriaceae, MRSA, MRCoNS and VRE. The resistance genes of the isolated bacteria were detected and sequenced by PCR. Phenotypic ESBL-producing Escherichia coli could be isolated in 10% of broiler casings (10 out of 100) and 43.6% of swine casings (31 out of 71). In line with previous studies, the results of this study showed that CTX-M-1 was the dominant ESBL produced by E. coli from swine (n = 25, 83.3%) and SHV-12 from broilers (n = 13, 81.3%). Overall, the frequency of positive samples with multidrug-resistant bacteria was lower than in most comparable studies focusing on meat products.
