Cancers of unknown primary (CUP) that we rarely encounter,conversely rank fourth in cancer-related deaths in males and females. In this case, we aimed to present how the primary of an inguinal mass, which could have been diagnosed as CUP in the 80’s, could be detected using advanced immunohistochemical methods.

The metastasis of extra-mammary malignancy to breast is extremely rare; literature reports the incidence between 0.4-1.3%. Primary sites include the contralateral breast, leukaemia, lymphoma, malignant melanoma, sarcoma, lung, prostate, ovary, colon and the stomach. Here we present a rare case in which lung cancer was found to metastasise to the breast. Initially the patient presented with chest symptoms and a left breast lump was detected clinically. The radiological and histological investigations confirmed the diagnosis of primary lung cancer with breast metastases. Prognosis of such cases is generally poor.

A 57-year-old man was admitted with complaints of stiffness on the right clavicle. Imaging study revealed swelling of multiple lymph nodes and pancreatic mass, but there were no such findings in the lung. Endoscopic ultrasound-guided fine needle aspiration for mediastinum lymph node and pancreatic mass revealed a poorly differentiated adenocarcinoma. He was diagnosed of pancreatic cancer with multiple lymph node metastases. Chemoradiotherapy was conducted. Six months later, he suffered from cardiac tamponade and died. The autopsy revealed a small nodule of poorly differentiated adenocarcinoma in the lung that could not be pointed out by computed tomography. Immunohistochemistry showed that both tumors of the lung and pancreas were positive for thyroid transcription factor-1 and napsin A, which are specific markers of lung cancer. Final diagnosis was primary lung cancer with metastases to the pancreas and heart. Pancreatic metastasis from occult lung cancer is rare. Lung cancer must be suspected when carcinoma of unknown primary is detected, even in the absence of the image findings of the lung. Immunostaining is useful in determining the primary site; hence, obtaining the cancer tissue is important.

AbstractSoft-tissue metastasis refers to the growth of cancer cells originating from internal cancer in the soft tissues. In most cases, soft-tissue metastasis develops after initial diagnosis of the primary internal malignancy and late in the course of the disease. In very rare cases, they may occur at the same time or before the primary cancer has been detected. In our case, the soft-tissue metastasis and the primary lung cancer were diagnosed at the same time. To the best of our knowledge soft tissue metastasis on the anterior abdominal wall of unknown primary lung cancer is an extremely rare condition and only a handful of cases have been reported till date .We report a case of unsuspected soft tissue metastasis on the anterior abdominal wall of an unknown primary lung cancer with relevant discussion on the same.

We report an extremely rare case of 79-year-old man, who was discovered with coincidental lung metastasis of prostate cancer and primary lung cancer. The patient presented with low prostatespecific antigen and two lung lesions: one in the right lower lobe, and one in the right upper lobe, 3 years after, he was treated with external beam radiotherapy for Gleason score 4+3=7, cT3aN0M0 prostate cancer. A computed tomography-guided needle biopsy of a nodule in the right lower lobe revealed suspicious metastasis of prostate cancer. Thoracoscopic excisions of both lesions were performed, and each lung lesion was diagnosed as being metastatic prostate cancer and primary lung cancer.
