The present paper discusses children who have been the victims of sexual abuse in their own family. It focuses on the special role of pediatricians and medical staff in identifying such children and providing them with initial assistance by reporting the situation to the authorities. The first part of the paper surveys the short- and long-term effects of childhood sexual abuse, including the physical and emotional impact of sexual exploitation and severe neglect. This section discusses the symptoms shown by abused children, and how they express and deal with their trauma. It is extremely important for pediatricians to be sensitive to the possibility of patients being abused at home, as this is an area still largely regarded as a societal taboo. Also included in this section a discussion of the effects that are manifested when the victim has grown to adulthood, such as personality disorders. The second part of the paper deals with how pediatricians must act when they encounter such a situation in which they suspect sexual abuse in the family. They should strive to identify the problem and bring it to the attention of the authorities. Discovery of the problem is the most vital part of the path to the victim's recovery. The paper also discusses the aspect of treatment, advising doctors who encounter this problem on ways of dealing with it.

That adults with mental handicap∗ are particularly vulnerable to sexual exploitation is not new, although relatively unacknowledged or investigated. Indeed successive acts under the mental health legislation have sought to protect them, particularly those with severe mental handicap. With the growing awareness and identification of sexual abuse of children, and the growing knowledge of the long-term psychological effects (Jehu, 1989), which may be even more severe for people who have disabilities (Kennedy, 1990; Sinason, 1989), it has been acknowledged that not only are the mentally handicapped particularly vulnerable to abuse (Benedict et al, 1990), but the problem may well be seriously underestimated by those working in management positions (Brown & Craft, 1989). While it is recognised that the mentally handicapped have rights and need to express their sexuality, they also have the right not to be exploited or abused. We therefore need to know more about the numbers, indicators and circumstances where this group may be sexually abused and to develop policies, systems and training to protect them (Brown & Craft, 1989). This pilot study was a step in this process.

Research findings are consistent in showing a strong, specific and coherent association between childhood sexual abuse (CSA) and long-term psychiatric problems. Occupational therapists working in mental health services must therefore have contact with survivors of abuse, yet the literature reveals a paucity of research on their role. Semi-structured interviews were conducted with nine occupational therapists to explore their knowledge and practice concerning this issue. All respondents considered that awareness of CSA was pertinent to their profession and should be taught during education and training. None of them had received teaching in this area. Most respondents thought that it was not their role to deal with CSA in depth. However, the context in which they worked provided an opportunity for initial disclosure which might not happen elsewhere and needed to be handled skilfully. Occupational therapy techniques, including creative methods, link in well with other approaches, especially counselling, in treating the long-term effects of CSA. Continuing educational requirements and the role of occupational therapy with this client group need to be addressed by the profession.

This experimental study examines the role of oral and written repetition strategies in consolidating new vocabulary in the classroom context. The participants in this study were divided into three treatment groups: oral, written and oral+written. A pretest and three delayed post-tests given at one day, one week and six weeks intervals were utilised in the study. The short and long-term retention of 12 unknown words was investigated over one semester. The results revealed that all three types of repetition strategies were effective in retaining new vocabulary in the short-term. However, in the long-term, the oral+written group achieved superior results while the oral group was the least effective. The findings on the effectiveness of employing these strategies across two levels of vocabulary knowledge (meaning recall and form recall) are discussed. These findings demonstrate the importance of repetition strategies in vocabulary learning.

Adolescents face high rates of victimization, yet little is known regarding the criminal consequences of these experiences. Using data from the National Youth Survey, this investigation compared the relative and combined effects of adolescent violent victimization perpetrated by family and nonfamily members on self-reported criminal offending from adolescence to early adulthood. The results demonstrate that both types of violence have an immediate and sustained impact on criminal involvement, although the effect is somewhat stronger for nonfamily victimization, and for both types, the relationship tends to weaken over time. In addition, those experiencing both types of victimization report a higher frequency of offending compared to those experiencing only one type. The findings indicate the need for prevention programs aimed at decreasing the prevalence of adolescent victimization, as well as intervention efforts to help victims from becoming offenders.

Changing gender roles and caretaking roles in the family have resulted in increased father involvement in the day-to-day care of their children. As a result, there is a need to better understand the nature of father–child relationships within the context of multiple circumstances. One specific example is the role of nonoffending fathers in the care of children after child sexual abuse disclosure. This article first reviews how the attachment theory has evolved to include fathers and then reviews the unique contribution of paternal attachment to the care of child victims of sexual abuse. Implications for practice include enhanced engagement and intervention strategies that incorporate fathers effectively in services, better utilizing fathers as allies in the care of their children.

Child and adolescent sexual abuse is considered to be a serious public health concern and a devastating form of violence with serious physical, mental, and behavioral consequences in the short and long term. This qualitative study aimed to identify maternal reactions to the disclosure of sexual violence and explore the comprehension regarding child and adolescent sexual abuse held by the mothers of child victims. Twelve mothers were interviewed, whose children had experienced a situation of sexual abuse and were receiving psychosocial care in a municipality in southern region of Brazil. The collected data were organized and analyzed using the principles of Grounded Theory with the Atlas.ti 7.5.7 software. From the analysis of the narratives, three main categories emerged: mothers with positive reactions, mothers with ambivalent reactions, and mothers with negative reactions. Mothers that presented positive reactions believed the report and supported their children, showing a comprehension that sexual abuse can occur in different ways and not only when there is penetration. The participants that had ambivalent reactions oscillated between maintaining or breaking off the relationship with the perpetrator of the violence, expressed initial disbelief regarding the report of the abused child, and had a comprehension that sexual abuse could be a form of affection. Mothers that had a negative reaction to the disclosure of abuse presented a contradictory narrative, minimizing the effects of the violence and placing the victims at fault regarding the abuse suffered. The importance of considering the possible relationships between the maternal comprehensions regarding child and adolescent sexual abuse and the reactions of these mothers during the professional intervention was demonstrated.

This study examines short- and long-term maladaptive outcomes in a sample of sexually abused females and a comparison group. The sample consists of intrafamilial sexual abuse victims ages 6–16 years at entry into the study and a demographically similar comparison group. The outcomes examined included measures of behavior and psychological problems such as aggressive behavior, depression, dissociation, and low self-esteem; and measures at two time points, first at entry into the study (median age 11 years) and approximately 7 years later (median age 18 years). The specific questions being addressed were (a) whether subgroups or profiles, based on the specific characteristics of the sexual abuse experienced, can be identified in this sample of abused females; and (b) whether these profile groups predict different patterns of adverse short- or long-term outcomes.
