Excision arthroplasty is an alternative procedure for degenerative arthritis of the trapezium carpal bone that has failed non-surgical treatment. Autologous or heterologous interposition materials are used following excision of the trapezium, but may not prevent first metacarpal-scaphoid joint collapse or subsidence. AlloDerm® is a cadaver-harvested, immunologically inert dermal collagen graft. This report documents the uncomplicated use of AlloDerm® in 13 thumbs of 9 patients who had first carpometacarpal (CMC) joint excision interposition arthroplasty for degenerative arthritis. Joints functioned well after arthroplasty with AlloDerm®. These clinical results compare favorably to results in 6 Silicon implant CMC arthroplasty patients, and 3 autologous dermal graft CMC arthroplasty patients.   Future clinical research, on joint stability, the durability of this collagen graft, newer dermal collagen substrates, and prevention of metacarpal subsidence, will determine the ultimate role of AlloDerm® in interposition arthroplasty.

Study Design: Literature review. Objectives: An increasing number of obese patients requires operative care for degenerative spinal disorders. The aim of this review is to analyze the available evidence regarding the role of obesity on outcomes after spine surgery. Peri-operative complications and clinical results are evaluated for both cervical and lumbar surgery. Furthermore, the contribution of MIS techniques for lumbar surgery to play a role in reducing risks has been analyzed. Methods: Only articles published in English in the last 10 years were reviewed. Inclusion criteria of the references were based on the scope of this review, according to PRISMA guidelines. Moreover, only paper analyzing obesity-related complications in spine surgery have been selected and thoroughly reviewed. Each article was classified according to its rating of evidence using the Sacket Grading System. Results: A total number of 1636 articles were found, but only 130 of them were considered to be relevant after thorough evaluation and according to PRISMA checklist. The majority of the included papers were classified according to the Sacket Grading System as Level 2 (Retrospective Studies). Conclusion: Evidence suggest that obese patients could benefit from spine surgery and outcomes be satisfactory. A higher rate of peri-operative complications is reported among obese patients, especially in posterior approaches. The use of MIS techniques plays a key role in order to reduce surgical risks. Further studies should evaluate the role of multidisciplinary counseling between spine surgeons, nutritionists and bariatric surgeons, in order to plan proper weight loss before elective spine surgery.

Background: Frey’s syndrome is a well-known complication of parotid surgery; its prevention may be achieved by the use of an interpositional barrier between the overlying flaps and the exposed parenchymal bed of parotid gland. The aim of this study was to retrospectively evaluate clinical outcomes with and without the interpositional placement of a porcine dermal collagen graft (PDCG) for prevention of syndrome occurrence. Methods: We conducted a 20-year retrospective study including the patients who had undergone “formal” (superficial, total, or subtotal) parotidectomies for benign pathologies. The inclusion criteria also involved patients that were (i) regularly monitored about clinical symptoms related to syndrome, and (ii) examined with Minor starch-iodine test. The severity of the diagnosed syndrome was retrospectively evaluated according to the grading score system of Luna-Ortiz. To assess group differences in terms of the extent of dissection in operating sites, we estimated the tumor and histological specimen volumes using the available dimensions. Results: We included 73 patients who had undergone 76 formal parotid surgeries. The surgical sites were divided into 2 groups: (1) Group A consisted of 44 sites that were reconstructed with a SMAS flap, and (2) Group B, comprised 32 sites where a PDCG was additionally applied as an artificial preventive barrier. At a mean follow-up of 26.3 months, a significantly lower incidence of clinically diagnosed Frey’s syndrome was found after the use of dermal collagen interpositional barrier ( P = .031). Specifically, subjective symptoms were reported at an incidence of 31.8% in Group A and 6.7% in Group B. Minor’s test was positive at an incidence of 59.09% in Group A and 21.87% in Group B ( P = .004, 95% CI). Severe Frey’s syndrome was observed in 31.82% of the patients of Group A and in 3.12% of the patients of Group B ( P = .002, 95% CI). Since there were no statistical significant differences between the volumes of the removed tumors and the excised histological specimens, the extent of dissection was not proved to influence the occurrence of Frey’s syndrome in the compared groups, Conclusion: Porcine dermal collagen is a safe, practical, and useful means for parotid reconstruction, since it seems to contribute in prevention of Frey’s syndrome when increased amount of glandular tissue has to be removed. Additional randomized controlled studies with bigger samples are required to better assess the PDCG use in parotid surgery.

The incidence and prevalence of neuroendocrine tumours (NETs) are on the rise. Although NETs are a heterogeneous group of tumours, they have some similar properties, for example, that they can concentrate neuroamines and tend to have a high degree of somatostatin receptor (SSR) expression. These mechanisms can be exploited and this article discusses the important role of radionculide imaging and radionculide therapy in the management of NETs based on these mechanisms. This article reviews the current literature and discusses the role of radionuclide imaging in NETs both in terms of SSR imaging and neuroamine (metaiodobenzylguanidine [MIBG]) imaging. We discuss state-of- the-art 68Ga-radiopeptide imaging and indications for it use. We also discuss the role of 18F-FDG and other tracers in the management of NETs. The second half of the article focuses on radiotargeted treatment of NETs, discussing I-131 MIBG therapy and focussing on the emergence of peptide receptor radionuclide therapy. We discuss the clinical results, toxicities and patient selection for PRRT. The aim of our investigation was to determine the ratio of maximal torque values and the torques in the certain positions of range of movements (ROM) between hamstring (H) and quadriceps femoris (Q) muscles at medium and high velocity of movement in concentric (CC) and eccentric (ECC) action of hamstring muscles. The knee muscles of 15 amateur female short and middle distance runners were tested by the dynamometer system in the isokinetic movements with the angular velocity of 90º/s and 240º/s in CC and at the velocity of 90º/s in ECC H/ CC Q muscles contractions. The torque values produced by the muscles are detected at the different angular positions of the ROM with the step of 10º. The ratios of H/ Q muscles torques are calculated. The H/Q muscles maximal torques ratio is 0.51 ± 0.13 at the velocity of 90º/s in CC and 0.60 ± 0.09 in ECC H/ CC Q muscles contractions, and 0.59 ± 0.09 CC at the velocity of 240º/s. The H/Q maximal torques ratio and this ratio in the knee extreme extension and flexion at the ECC contraction of H is higher due to greater torques produced by the H in comparison with Q muscle. The H must be stronger to decelerate the thigh and lower leg extension in the late swing phase of running and to extend the hip in early stance phase to provide powerful sprint running and prevent the knee and H injury. The H/Q muscles torques ratio in extended knee positions are similar in medium (90º/s) and fast (240º/s) velocity of motions because CC action of H muscles cannot prevent extreme knee extension.

Abstract-: Breast cancer is the most common type of malignancy affecting women worldwide and also being the mostly diagnosed one. Mammography being the gold standard for diagnosis but there are high chances that it can give false- negative as well false-positive test results. Finding cost-effective, readily available and increased sensitivity as well as specificity for diagnosis is the need right now to decrease the mortality as well as morbidity rate. Application of biomarkers to the clinical use has paved way for a better prognosis, diagnosis, detection, screening and better clinical results. The efficacy of the treatment is enhanced. Biomarkers are known to cause advancement in breast cancer study and are expected to improve the quality of life in patients. Not only tissue biomarkers but serum and circulating biomarkers are also of significance in patients. This review highlights the particulars about the current use and application of biomarkers in effective prognosis, detection and treatment of breast cancer and also the ones which are currently being studied under trials and have the potential to be advantageous and more specific in the near future.
