Body composition assessment is an important tool in both clinical and research settings able to characterize the nutritional status of individuals in various physiologic and pathologic conditions. Health care professionals can use the information acquired by body composition analysis for the prevention and treatment of diseases, ultimately improving health status. Here we describe commonly used techniques to assess body composition in healthy individuals, including dual-energy x-ray absorptiometry, bioelectrical impedance analysis, air displacement plethysmography, and ultrasonography. Understanding the key underlying concept(s) of each assessment method, as well as its advantages and limitations, facilitates selection of the method of choice and the method of the compartment of interest. AbstractSoccer referees are a specific group of the athletes’ population whose careers peak from 30 to 45 years old. An athlete's performance is not only determined by physical training but also by a lifestyle, e.g. eating habits. The purpose of this study was to verify current eating habits and resulting body composition of a group of elite international soccer referees. At an international FIFA seminar 60 elite international soccer referees (aged 39.2 ± 4.2 years) were enrolled. A body composition assessment was performed with skinfold thickness and bio impedance analysis, while eating habits were evaluated with a multi-pass 24-hour dietary recall. The body composition showed a normal weight condition with a fat content of 11.4 ± 2.5%. Macronutrients showed a low level of carbohydrates (43.6 ± 5.4%) and a high level of fat (40.0 ± 4.5%). Micronutrients showed a low level of calcium, potassium, zinc, magnesium, iodine, vitamin B12 and vitamin B9. Even though their body composition was within the normal range, the current eating habits of elite international soccer referees did not appear to follow the nutrition guidelines. Therefore, it would be advisable to provide knowledge on nutrition for this particular category of sports subjects, an individualized nutritional plan would be advisable, in order to achieve and maintain better performance and appropriate body composition for their role.

Objective. To investigate the preoperative body composition, metabolic characteristics, and postoperative changes in patients with active acromegaly and analyze the effects of gender and age. Methods. We included 36 patients with untreated acromegaly and 37 patients with nonfunctional pituitary adenomas. Adipose tissue (AT), the visceral fat index (VFI), sclerotin, protein, skeletal muscle, total body water (TBW), intracellular water (ICW), and extracellular water (ECW) were measured using bioelectrical impedance analysis (BIA). Total energy expenditure (TEE) and basal metabolism (BM) were measured with a cardiopulmonary and metabolic analyzer (CMA). Tricep skinfold thickness (TST), bicep circumference, waistline, hipline, and calf circumference were measured with a skinfold caliper and tape. These indices were measured before surgery and 3 months and 1 year after surgery. Results. Overall, AT, VFI, and TST were lower, whereas sclerotin, protein, skeletal muscle, TBW, ICW, ECW, TEE, and BM were higher in acromegaly patients. Postoperatively, TST rose initially and then decreased, the waistline increased, and sclerotin, skeletal muscle, TEE, and BM decreased. Changes in these indices differed with gender and age in unique patterns. Conclusions. Body composition and metabolism in acromegaly patients changed after surgery, and gender and age influenced these changes.

Abstract Context: Bioelectrical impedance spectroscopy (BIS) and skinfold anthropometry (SKF) have been used to monitor body composition among patients with HIV wasting; however, validation of these techniques during recombinant human GH (rhGH) treatment has not been performed. Objective: Our objective was to evaluate the degree of agreement between criterion measurements of dual-energy x-ray absorptiometry (DXA) and those of BIS and SKF in patients with HIV wasting treated with rhGH. Design and Setting: We conducted a randomized, double-blinded, placebo-controlled, two-period crossover trial at the University of Toronto and Mount Sinai Hospital (Toronto, Canada). Patients: A referred sample of 27 community-dwelling men with HIV-associated weight loss (≥10% over preceding 12 months) despite optimal antiretroviral therapy participated in the study. Intervention: Intervention was one daily injection of rhGH (6 mg) or placebo self-administered for 3 months in a crossover fashion with a 3-month washout. Main Outcome Measures: Fat-free mass (FFM) and fat mass (FM) were measured by BIS, SKF, and DXA before and after rhGH and placebo treatment. Results: FFMBIS was not significantly different from FFMDXA after rhGH treatment (P = 0.10). Mean differences (bias ± sd) according to Bland-Altman analysis were smaller for SKF than for BIS (P < 0.05) at all time points, yet treatment-induced change in FM was better detected with BIS than with SKF. BIS estimates of FFM and FM showed better agreement with those of DXA after rhGH treatment (1.6 ± 4.6 kg and −2.1 ± 3.9 kg) compared with baseline (3.8 ± 3.5 kg and −4.1 ± 3.6 kg) and placebo (2.7 ± 4.4 kg and −3.1 ± 4.6) (P < 0.05). BIS overestimated and SKF underestimated the treatment-induced changes in FFM and FM. Conclusions: SKF was more accurate than BIS when measuring body composition in patients with HIV wasting before and after rhGH treatment; nonetheless, the accuracy of BIS increased after treatment. Change in FM because of treatment was not accurately assessed with SKF.

Abstract Current guidelines do not recommend bioelectrical impedance analysis (BIA) in patients with implanted cardiac devices. There is no data on the influence of such devices over the parameters assessed by BIA. We aimed to assess the influence of cardiac devices on the parameters assessed by BIA as well as to evaluate the likelihood of electromagnetic interference of BIA in patients with implanted cardiac devices. Sixty-two consecutive patients over 18 years of age who underwent single (PM) or multisite (CRT) pacemaker or defibrillator (ICD) implantation were included. Body composition assessment was done using a single frequency device, on both right and left sides, before and after cardiac device implantation. During BIA analysis after device implantation, we did real-time telemetry to assess electromagnetic interference. Patients were 67+14 years old and 51.6% male. PM was implanted in 52 patients (83.9%), ICD in 7 (11.3%), ICD with CRT in 2 (3.2%) and CRT in 1 (1.6%). During real-time telemetry, there was no electromagnetic interference including interruption of telemetry. Default device programming did not change after BIA assessment. After surgery, resistance and fat mass were smaller, while cellular mass, fat-free mass, metabolic rate and total body water/ body weight increased, on right and left sides measurements. We concluded that decreased resistance and related parameters after device implantation were probably influenced to a change in hydration status, regardless of the implanted device. Bioimpedance analysis is safe in patients with an implanted cardiac device.
