Aim: This article described a physiological impression technique to design denture surfaces by the action of the surrounding musculature during functional movements based on "neutral zone concept" in patients with severely atrophied mandibular ridge. Methodology: Dual surface impression technique was employed for the fitting and polished surface using silicone impression material. Results: This technique provided a stable base with correct borders to perform all functional movements of the lips, cheeks, and tongue. Conclusion: The patient improved the ability to maintain the adaptive muscle behaviors necessary to control the denture movement in function and rest.   How to cite this article: Abdel-Khalek EA, Said MM. Endoperiodontal lesions are characterized by the presence of periodontal and pulpal disease within the same dental element. Periodontal and pulpal tissues are related to each other by lateral and accessory canals, the apical foramen, and the dentinal tubules. In the dental literature, whether periodontal disease affects pulpal disease or vice versa remains controversial. Pulpal disease is often accepted to cause periodontal disease. The treatment of endodontic-periodontal combined lesions requires both endodontic therapy and periodontal regenerative procedures. The success rate of treatment comprising only an endodontic procedure for such lesions is low. Periodontal regenerative therapy has been used widely to regenerate periodontium lost due to periodontal disease. Aim: This case report describes the use of a surgical guide in a crown-lengthening procedure to improve the aesthetics of the anterior interdental papillae in an adult female patient. Methodology: A 40-year-old female underwent a crown-lengthening procedure with guidance provided by a surgical template to correct unsatisfactory aesthetics in the anterior gingiva and prosthesis. Results: The patient’s functional and aesthetic expectations were met successfully with interdisciplinary treatment, including surgical crown lengthening and the placement of all-ceramic restorations. Conclusions: This case showed that satisfactory anterior aesthetics can be achieved by comprehensive examination, careful planning of treatment employing a surgical guide, and prosthodontist–periodontist teamwork, even in the complete absence of interdental papillae.  How to cite this article: Kılıç D, Alkan BA, Kılıç K. The hybrid impression technique consists of an initial alginate impression that provides a pre-operative cast upon which a diagnostic wax-up and a silicone index impression are made. This work is digitized; thus, the altered final digital impression is limited to absolute minimum time, effort and ensures comfort for the patient.

Complete crown fracture is a dental injury affecting enamel, dentin, and pulp and unfortunately crown is lost usually due to loss of tooth structure. Root-crown lengthening can be performed if an acceptable root-crown ratio is preserved for the retention of root. This paper presents a case report of a mandibular canine incisor tooth having complete crown fracture. Root-crown lengthening is performed by surgical extrusion of the root coronally, splintted using a semi-rigid splint and prognosis was successful after one year later. Treatment of complete crown fracture by surgical extrusion and splintting may be a good alternative for general practitioners since it is an easy and short time requiring technique necessiating less equipments.  How to cite this article: Yiğit Özer S, Uysal İ, Bahşi E. Patients with resorbed mandibular ridge poses a challenge for prosthodontic rehabilitation. The ongoing diminution of denture foundation defies the basic mechanism of denture stability and support. The technique of neutral zone to construct a denture that is shaped by muscle function and is in harmony with the surrounding oral structures can remarkably improve prognosis of such patients. The neutral zone is the area in the oral cavity where during function; the forces of the cheeks and lips pressing inward neutralize the forces of the tongue pressing outward. Thus physiologically optimal teeth arrangement and harmonious denture contours and can be achieved to maximize prosthesis stability, comfort, and function for patients. This clinical report describes rehabilitation of a patient with severely resorbed mandibular ridge using neutral zone impression technique.

AbstractOne of the most commonly faced problems among long term denture wearers is the reduction in the denture foundation. Prosthetic rehabilitation of a patient with severely resorbed ridge is the most challenging therapy a prosthodontist can undertake. In order to have a favourable prognosis for the denture therapy, impression technique selected should be based on the present state of the basal tissue support. This article presents the application of neutral zone concept being incorporated in impression making to achieve successful complete denture therapy.

Aim: This case report demonstrates the application of piezocision in correcting persistent crowding in the lower anterior region of dental arch. Methodology: A female aged 14 years and 3 months presented to the Selcuk University Department of Orthodontics due to aesthetic complaints. Clinical and radiological examination revealed crowding in the upper and lower dental arches as well as skeletal and dental Class I malocclusion. First, bonding and arch wire were applied to the patient’s upper jaw. The patient then had check-up appointments at 4-week intervals. In the second session, bonding was applied to the lower jaw. As the leveling was completed in the upper jaw, thicker arch wires were used. However, 4 months after the bonding of the lower anterior region, leveling did not occur. Consequently, piezocision was applied to the lower anterior region. Then, two applications of 0.014'' and 0.016'' NiTi wire were carried out, respectively. After the completion of the leveling, intrusion was provided with the utility arch placed on the lower incisor brackets. Orthodontic treatment was concluded with the use of NiTi and steel angle wires. In modern dentistry, fibre-reinforced fixed dental prostheses are considered a useful alternative to classical metal-ceramic restorations. This method allows a conservative approach for replacing missing teeth that overcomes some of the drawbacks of conventional prostheses. Our patient required extraction of tooth #46 because of an apical lesion of the mesial root, and underwent extraction by hemisection. Abstract Oral functions involve unique interplay of oral structures and muscles. Any interference with their movements by a denture would result in denture instability. If the denture is placed in a zone where the displacing forces of tongue, lips, cheeks and modiolus are balanced, then the denture will be retained more effectively during function. This zone is known as neutral zone. If the denture strays outside/inside the neutral zone it will be unstable during the activities such as talking, swallowing and mastication. The neutral zone technique is used to minimize the displacing forces of the surrounding structures. This is a case report in which neutral zone impression technique was used to solve the problem of denture instability.

Brief Background Fabrication of any dental prosthesis over a compromised residual alveolar ridge is really a challenge for prosthodontists. Fibrous or flabby ridge and highly resorbed residual alveolar ridge are two clinical situations that require special considerations during construction of any prosthesis. Mobile soft tissue and flattened alveolar ridge significantly compromise denture stability and support. There are different treatment modalities to manage these clinical conditions like surgical excision of soft tissue, ridge augmentation, modification in impression technique, etc. Materials and Methods This case report is about an old completely edentulous male patient who reported with a complaint of difficulty in chewing with his present denture. Discussion Prosthodontic management of a patient with compromised residual alveolar ridges needs special consideration . Selection of treatment modality depends on patients general health ,clinical condition of residual alveolar ridges , financial capacity and skill of the clinician A modification from conventional impression procedure was used to record maxillary flabby anterior ridge and neutral zone concept was applied for teeth arrangement to improve denture stability in atrophied lower alveolar ridge. Summary and Conclusions Surgical removal of soft tissue and implant supported prosthesis may not be possible in all situations. In that case a conservative approach using modified impression technique, neutral zone concept provides the basic triad of a successful denture prosthesis that is retention, stability and support.
