Complete crown fracture is a dental injury affecting enamel, dentin, and pulp and unfortunately crown is lost usually due to loss of tooth structure. Root-crown lengthening can be performed if an acceptable root-crown ratio is preserved for the retention of root. This paper presents a case report of a mandibular canine incisor tooth having complete crown fracture. Root-crown lengthening is performed by surgical extrusion of the root coronally, splintted using a semi-rigid splint and prognosis was successful after one year later. Treatment of complete crown fracture by surgical extrusion and splintting may be a good alternative for general practitioners since it is an easy and short time requiring technique necessiating less equipments.  How to cite this article: Yiğit Özer S, Uysal İ, Bahşi E. Aim: This case report describes the use of a surgical guide in a crown-lengthening procedure to improve the aesthetics of the anterior interdental papillae in an adult female patient. Methodology: A 40-year-old female underwent a crown-lengthening procedure with guidance provided by a surgical template to correct unsatisfactory aesthetics in the anterior gingiva and prosthesis. Results: The patient’s functional and aesthetic expectations were met successfully with interdisciplinary treatment, including surgical crown lengthening and the placement of all-ceramic restorations. Conclusions: This case showed that satisfactory anterior aesthetics can be achieved by comprehensive examination, careful planning of treatment employing a surgical guide, and prosthodontist–periodontist teamwork, even in the complete absence of interdental papillae.  How to cite this article: Kılıç D, Alkan BA, Kılıç K. AbstractTo evaluate the effect of extended adult exposure to authentic spoken English on the perceptual mastery of English /r/ and /l/, we tested 12 native speakers of English (A), 12 experienced Japanese (EJ) who had spent 12 or more years in the United States, and 12 less experienced Japanese (LJ) who had spent less than one year in the United States. The tests included the forced-choice identification of naturally produced /r/s and /1/s and the labeling of word-initial synthetic tokens that varied F2 and F3 to form an /r/-/l/-/w/ continuum. The F.Js’ mean performance in both tasks was closer to that of the As than the LJs, but nonetheless fell short. Extended exposure may improve /r/-/l/ identification accuracy; it does not ensure perfect perceptual mastery.

Post endodontic restoration in cases of grossly destructed teeth are difficult as the available tooth structure is less, margins are subgingival and of compromised prognosis because the restoration might encroach upon the periodontium affecting its status to deteriorate. Interdisciplinary approach using forced eruption or Orthodontic extrusion along with minimal surgical crown lengthening can give adequate supragingival tooth surface for proper restoration and protect its root support from getting violated. This is the case report of an unsalvageable tooth that is restored to satisfactory functional and esthetic restoration utilizing interdisciplinary approach.

Endoperiodontal lesions are characterized by the presence of periodontal and pulpal disease within the same dental element. Periodontal and pulpal tissues are related to each other by lateral and accessory canals, the apical foramen, and the dentinal tubules. In the dental literature, whether periodontal disease affects pulpal disease or vice versa remains controversial. Pulpal disease is often accepted to cause periodontal disease. The treatment of endodontic-periodontal combined lesions requires both endodontic therapy and periodontal regenerative procedures. The success rate of treatment comprising only an endodontic procedure for such lesions is low. Periodontal regenerative therapy has been used widely to regenerate periodontium lost due to periodontal disease. Crown lengthening is a surgical procedure designed to increase the extent of the supragingival tooth structure, so that the clinician can restore the tooth. Crown lengthening procedure is done to maintain normal biologic width and increase crown length for retention of prosthesis. Various techniques have been proposed to perform CLP, such as gingivectomy, undisplaced flap with or without osseous reduction, apically repositioned flap with or without bone reduction, and orthodontic forced eruption with or without fibrotomy. Selection of one of this CLP technique depends upon esthetics, clinical ratio of crown to root, structure of root, location of furcation, position of tooth and the capacity of the tooth to be restored. This case report illustrate two different methods of doing crown lengthening procedure the selection of case depends upon various soft tissue and hard tissue parameters. Owing to the various advantages, disadvantages and associated limitation with different methods, we opted for surgical CLP with scalpel method. Uneventful healing was observed in both our cases. No post-operative complications was observed in both cases. Thus to conclude success rate of CLP is high but appropriate selection of case is required. In our case report both the methods of CLP shows significant result in increasing the crown length and maintaining the biological width.

Advances in adhesive dentistry have led to increased use of indirect restorations. In some situations, indirect composite techniques are more advantageous than direct composite filling techniques, such as establishing proper occlusal and interproximal anatomy, reducing polymerization shrinkage stress, and promoting the degree of conversion. This article presents a case about restoring the lower right first molar with extensive loss of tooth structure by the composite onlay to achieve a proper anatomic form and rehabilitate chewing function. This one-year clinical case encourages clinicians to manage large decay of posterior tooth conservatively. The given functional and esthetic outcomes demonstrate the promising applicability of the indirect composite technique.

Abstract Aim The purpose of this case report is to describe the clinical procedure for fabricating fiber-reinforced composite (FRC) onlay composite resin restorations using a FRC as the substructure. Background A variety of therapeutic modalities are available to restore teeth with moderate coronal defects in the posterior region of the mouth. For patients who refuse complete crown restorations or when minimal tooth reduction is preferred, a FRC restoration can be a good alternative to conventional restorative techniques. Case Report A 42-year-old female patient presented with an endodontically treated mandibular right first molar with extensive destruction of the coronal tooth structure. To conserve the remaining tooth structure a FRC resin core substructure was fabricated and veneered with Ceramage dentin and enamel hybrid composite to create the final restoration. Summary FRC restorations using adhesive technology appears to be a promising restorative option. However, further clinical investigation will be required to provide additional information on this technique. Based on the clinical and radiographic findings in the present case, the fabrication of a conventional crown was avoided in order to conserve the remaining tooth structure. Clinical Significance The restoration of badly damaged teeth is a challenge for clinicians when cast crown restorations are not an option for the patient. At two different times, Time 1 and Time 2, 13 participants in Japan (8 Japanese and 5 Americans) were asked to spontaneously respond in English to this prompt: “Tell me about one of the most exciting or dangerous moments in your life.” The Japanese responded during their first and fourth years of college, which involved an interval of 42 months. The Americans were native speakers of English and responded earlier and later in their one year study abroad program in Japanese language and culture. Three questions addressed by this paper were the following: (a) What types of topics and narrative structures characterize these 26 stories? (b) What types of speaker-initiated repairs appear, and are the repairs the same or different at Time 1 and Time 2? (c) How are the repairs related to different listener (American, Japanese, Filipino, and Taiwanese) assessments of the intelligibility of the narratives?

ABSTRACTCoronal fractures of permanent dentition are the most frequent type of dental injury. If the original tooth fragment is retained following fracture, the natural tooth structures can be reattached using adhesive protocols. The development and use of fiber-reinforced composite root canal posts make possible of the reattachment of the crown esthetically. This case report presents a clinical technique to reattachment maxillary lateral incisor tooth after trauma using direct fiber-reinforced post systems. At the 1-year follow-up, a slightly marginal discoloration, but no recurrent decay or composite clefts were detected and the resultant appearance was acceptable to the patient. Aim: This case report demonstrates the application of piezocision in correcting persistent crowding in the lower anterior region of dental arch. Methodology: A female aged 14 years and 3 months presented to the Selcuk University Department of Orthodontics due to aesthetic complaints. Clinical and radiological examination revealed crowding in the upper and lower dental arches as well as skeletal and dental Class I malocclusion. First, bonding and arch wire were applied to the patient’s upper jaw. The patient then had check-up appointments at 4-week intervals. In the second session, bonding was applied to the lower jaw. As the leveling was completed in the upper jaw, thicker arch wires were used. However, 4 months after the bonding of the lower anterior region, leveling did not occur. Consequently, piezocision was applied to the lower anterior region. Then, two applications of 0.014'' and 0.016'' NiTi wire were carried out, respectively. After the completion of the leveling, intrusion was provided with the utility arch placed on the lower incisor brackets. Orthodontic treatment was concluded with the use of NiTi and steel angle wires.
