The purpose of this chapter is to develop a methodology that will contribute in locating optimal evacuation routes in case of fire that are based on minimal carbon monoxide (CO) exposure during the evacuation procedures. The proposed methodology is tested using simulated fire scenarios from which CO concentration over time curve is extracted from all available evacuation routes and presented in a weighted form based on the accumulating effect of CO inhalation in the form of fractional effective dose (FED). The safety limits of the FED model on which the optimization process is based are determined using a model for the prediction of carboxyhemoglobin (COHb) levels in human blood. The COHb model is associated with predicted clinical symptoms that are the basis for determining the level of incapacitation at which the mineworkers are incapable of completing their evacuation. Also in the process of improving the fire risk analysis, the proposed methodology enables the development of evacuation plans that are based on the results of modeled fire scenarios combined together with the results of the anticipated hazards generated by CO inhalation. The results presented in this chapter indicate a more precise approach in the process of planning the evacuation system inside the underground mines.

One of the major challenge from unplanned growth in the cities is the fire incidents posing a serious threat to life and property. Delhi, the capital city of India, has seen unplanned growth of colonies resulting in a serious concern for the relevant agencies. This paper investigates the relation between potential causes of fire incidents during 2013-2016 in South-West Delhi Division of Delhi Fire Services as part of risk analysis using the data about fire stations & their jurisdictions, incidents of fire, water reservoirs available, landuse and population data along with the divisional & sub-divisional boundaries of South-West Delhi division under Delhi Fire Service. Statistical and Geospatial tools have been used together to perform the risk analysis. The analysis reveals that difference in actual occupancy and defined landuse as a part of unplanned growth of settlements is found to be the main reason behind the major fire incidents. The suggested mitigation measures focus on legal, policy, physical & technological aspects and highlight the need to bring the systemic changes with changing scenario of demographics and infrastructure to accommodate more aspects of ground reality.

Background: Human milk contains antibodies against Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) following Coronavirus Disease 2019 (COVID-19). These antibodies may serve as protection against COVID-19 in infants. However, the evolution of these human milk antibodies over time is unclear. Research Aim: To elucidate the evolution of immunoglobulin A (IgA) against SARS-CoV-2 in human milk after a SARS-CoV-2 infection. Methods: This longitudinal follow-up study included lactating mothers ( N = 24) who had participated in the COVID MILK study. To assess the evolution of SARS-CoV-2 antibodies, serum and human milk samples were collected 14–143 days after the onset of clinical symptoms related to COVID-19. Enzyme-Linked ImmunoSorbent Assay was used to detect antibodies against the ectodomain of the SARS-CoV-2 spike protein. Results: SARS-CoV-2 antibodies remain present up to 5 months (143 days) in human milk after onset of COVID-19 symptoms. Overall, SARS-CoV-2 IgA in human milk seems to gradually decrease over time. Conclusion: Human milk from SARS-CoV-2 convalescent lactating mothers contains specific IgA antibodies against SARS-CoV-2 spike protein up to at least 5 months post-infection. Passive viral immunity can be transferred via human milk and may serve as protection for infants against COVID-19. Pyrrhic disease is a congenital anomaly that occurs during embryogenesis due to the fixation of the splenic angle of the colon by a short and highly located left transverse-diaphragmatic ligament, creating a sharp bend and forming a "wellbore". In this case the passage of feces on a cross colon becomes difficult, there is also its sagging to a small pelvis. This pathology is characterized by paroxysmal pain (aggravated by exercise and after eating) and prolonged constipation, which progresses over time. Hilaiditis syndrome is a rather rare pathology in which there is an interposition of the hepatic angle of the colon between the liver and the diaphragm. There are permanent and intermittent localizations. The work generalizes the experience of evaluation of clinical manifestations and remote results of treatment of children with chronic colostasis caused by fixation abnormalities of the colon. 58 children were detected to have Payre’s disease, with Cyilaiditi’s syndrome – 3 children. 24 patients with Payre’s disease and 2 Cyilaiditi’s syndrome were operated on. To assess the effectiveness of surgery, children were divided into two groups: I group – comparative and II group - experienced. In I group (n=12 children) – the analysis of surgical treatment was performed traditionally. In II group (n=14 children) – the analysis of surgical treatment was conducted by means of the methods proposed. Traditional surgical treatment of Payre’s disease in children was followed by relapse of chronic constipation in 45,45%, pain in 50%, flatulence in 33,33% and failure of the ileocoecal closing apparatus in 100% of children. Unsatisfactory outcomes of surgical treatment of Cyilaiditi’s syndrome was observed in a child from the comparative group. Relapse of clinical symptoms to a lesser degree than before the surgery was found in 1 child from the experienced group. To treat Payre’s disease the following operation is proposed: intersection of the left diaphragm-colon ligament, resection of transverse colon and colofixation of the left bending of the colon. To treat Cyilaiditi’s syndrome (in case of dolichoascendocolon) the following operation is suggested: hepatopexy, resection of the right bending of the colon with ascending transversal anastomosis “end to end”, fixing of right bending of the colon. Their reasonability is being proved.

This study was designed to investigate the natural history of atlantoaxial instability in individuals with Down syndrome and to determine whether significant changes in C1-C2 relationship are taking place over time. Although more than 400 patients with Down syndrome who are presently followed at the Child Development Center had cervical spine radiographic examinations in the past, only 141 patients who had serial radiological examinations and whose radiographs were available for reevaluation participated in this study. The results of our investigations revealed that there were only minor changes (1 to 1.5 mm) of atlanto-dens interval measurements over time in 130 (92%) patients with Down syndrome. Eleven patients (8%) had changes of atlanto-dens interval measurements between 2 and 4 mm over time; however, none of these patients had any clinical symptoms. The analyses of data obtained from several subgroups (males and females, various age groups, and patients with and without atlantoaxial instability) did not show any significant changes of atlanto-dens interval measurements of successively obtained radiographs. Our recommendations for and rationale of routine screening for atlantoaxial instability and follow-up examinations are discussed in detail.

During a fire in enclosed spaces, having structures with a good level of resistance is very important. The post flashover fire time interval, during which rescue squads operate, is important to verify if the structure can still resist fire for an acceptable time interval. This can be determined through the REI value. Hence, the way the fire develops must be examined together with the trend of the temperature that might guarantee that the structure will resist the heat flux released during the combustion. This article examines and compares, through a case study, the most important methods for analysis of the fire risk: the prescriptive approach and the simplified performance-based approach. The performance-based method (more suitable for the actual planning demand with respect to the more cautious prescriptive approach) is affected by the parameters influencing its development. The goal of this article is to provide a graph (based on parameters like the type of building, opening factor “O” and fire load “q”) that might be used by designers and architects to carry out the planning phase and adopt fire prevention systems before dealing with the assessments required by the engineering field for the fire risk analysis.

Purpose The purpose of this study is to develop optimal evacuation plan to provide valuable theoretical and practical insight in the fire evacuation work of similar structures, by proposing a systematic simulation-based guided-evacuation agent-based model (GAM) and a three-stage mathematical evacuation model to investigate how to simulate, assess and improve the performance efficiency of the evacuation plan. Design/methodology/approach The authors first present the self-evacuation and guided-evacuation models to determine the optimal evacuation plan in ship chamber. Three key performance indicators are put forward to quantitatively assess the evacuation performance within the two fire scenarios. The evacuation model in tower is built to obtain the dividing points of the three different fire evacuation plans. Findings The study shows that the optimal evacuation plan determined by the GAM considering social relationships effectively relieves the congestion or collision of evacuees and improves the evacuation uniformity. The optimal evacuation plan not only solves the crush caused by congestion or collision of evacuees but also can greatly shorten the evacuation time for passenger ship fire. Originality/value This study establishes the GAM considering the interactive evacuee characteristics and the proportion of evacuees guided by the crew members to make the optimal evacuation plan more time-efficient. The self-evacuation process is simulated to assess the performance of the guided-evacuation strategies, which are used to verify the effectiveness and feasibility of the optimal evacuation plan in this research.
