AbstrakKesehatan Ibu dan Anak (KIA) merupakan permasalahan kesehatan yang belum tuntas di Indonesia. Ibu dan anak adalah anggota keluarga yang perlu mendapatkan perhatian serta prioritas dalam usaha penyelenggaran kesehatan. Tahun 2012 terdapat enam provinsi yang menjadi fokus pemerintah dalam mengatasi permasalahan kematian ibu yang salah satunya Jawa Timur. Kabupaten Jember menjadi salah satu Kabupaten di Jawa Timur yang masih mengalami permasalahan KIA. Penambahan anggaran dalam upaya peningkatan kesehatan ibu dan anak belum berjalan secara optimal di Kabupaten Jember. Oleh karena itu, perlu dilakukan penelitian dengan pendekatan Health Account terkait KIA di Kabupaten Jember. Jenis penelitian yang digunakan adalah penelitian deskriptif. Unit analisis pada penelitian ini adalah intitusi yang melakukan kegiatan langsung maupun tidak langsung terhadap program KIA pada tahun 2018 dengan fokus anggaran yang berada pada tingkat daerah. Hasil penelitian menunjukkan pola belanja pada setiap program kesehatan banyak digunakan untuk belanja investasi dan anggaran banyak diprioritaskan untuk belanja investasi pada kegiatan tidak langsung. Saran yang diberikan adalah pemerintah Kabupaten Jember menyusun kebijakan yang diarahkan pada pembangunan berparadigma sehat dan membangun sistem informasi manajemen keuangan daerah yang terintegrasi, dan juga harus membuat payung hukum untuk melembagakan pelaksanaan Health Account ditingkat daerah.Abstract Maternal and Child Health (MCH) is unresolved health problems in Indonesia. Mother and child are family members who need attention and priority in delivery of health. In 2012, there were six provinces that were focus of government in resolve problems of maternal mortality, one of them was East Java. Jember District is one of district in East Java that is still having problems with MCH. The addition of the budget in increasing maternal and child health has not been running optimally in Jember. Therefore, it is necessary to research with Health Account approach related to MCH in Jember District. This type of research was descriptive research. The unit of analysis in this research was the institution that carried out direct or indirect activities on the MCH program in 2018 with an appropriate budget focus at the regional level. The results of research showed that expenditure patterns on every program were widely used for investment expenditure and the budget was prioritized for investment expenditure in indirect activities. The advice given is that Jember District Government develops policies directed at healthy paradigm development and builds an integrated financial management information system, and must also create a legal protection to institutionalize implementation of Health Accounts at distrcit level.

This chapter is a critical review of the complexities of breastfeeding and its attending effects on the health of the Mother and Child. The researcher employs a historical research approach to gather both theoretical and empirical facts on this issue. It was discovered that in spite of the pragmatic and frantic efforts by different individuals, organizations, governments and non-governmental bodies in establishing a universal, healthy and safest method of breastfeeding through research, publications, workshops and so on, based on the unavoidable and uncompromising importance and benefits to maternal and child health, it is on record that the maternal behaviour is still obviously at variance to the acceptability of full breastfeeding as promulgated and declared by World Health Organization (WHO). A serious, dedicated, pragmatic and coordinated counseling approach is therefore recommended to be adopted to revive and strengthening a positive behaviour in women to foster positive attitude towards breastfeeding to guarantee 100% morbidity and mortality rate in Maternal and Child health.

Abstract Background Preconception education is one of the challenges in maternal and child health in Latin America, in countries such as Colombia. That is why the objective of the following research was to develop a pilot educational intervention aimed at pregnant and breastfeeding women, and their support network, to strengthen practices, skills and knowledge, for informed decision-making and the strengthening of self-care behaviors of women and childcare, beyond childbirth preparation in the municipality of Soraca, located in Boyacá, Colombia in 2017. Methods Quasi-experimental study with an intervention group (municipality of Soraca) and a control group (municipalities of Oicata and Sotaquira) which linked a process and impact evaluation to assess prioritized indicators in terms of maternal and child health and nutrition, before, during and after an educational intervention. Results The main results found that women considered the methodologies used in the educational sessions to be appropriate and relevant and recognized the importance of the call and home visit postpartum nurse. The strategy increased the involvement of the support network in training processes. In turn, the proportion of children who were breastfed was higher in the intervention group than in the control group (88% vs. 60% p = 0.037). The proportion of children who received a bottle was higher in the control group compared to the intervention group (57.1% vs. 16% p = 0.006). Conclusion Strategies such as the one developed in Soraca allow for improved health outcomes for the mother and child and their support network. The strategy 123 in Soraca was a clear example of how a prenatal educational intervention generates positive outcomes for mothers, infants, and their support networks in the areas of food and nutrition, emotional well-being, and physical health.

This study seeks to make a deep systematic analysis of the involvement of the community as an additional implementing element in the implementation of public health policy, especially in the field of mother and child in West Java, Indonesia. This descriptive-qualitative study presents a discussion of how the elements of public health practitioners interact with communities in West Java Province so that their knowledge and skills are needed. With the theory of health services, data obtained that (a) service and network cooperation in public health institutions, within the Provincial Government of West Java is quite uniform and well implemented; (b) in carrying out its obligations as the implementing element of the community deeply understand public policy in the field of public health, especially maternal and child health; so they can carry out their functions to serve the community. Nevertheless, the lack of community volunteers directly addressing maternal and child health is felt so that this review recommends incentives for health workers coming from ordinary people.

Abstract Objectives: to present the main evidence, recommendations and challenges for maternal and child health in the context of COVID-19 pandemic. Methods: narrative review of national and international documents and reflections on the theme. Results: the coexistence ofpregnancy/puerperium and COVID-19 infection establishes many challenges. It is extremely important that the conduct should be individually adopted, covering all aspects of health in the mother-child binomial, estimating risks and benefits of each decision. Until now, it is recognized that natural childbirth should be encouraged and breastfeeding maintained, if adequate hygienic-sanitary care is ensured. Cesarean delivery and the isolation and separation of the mother-child contact without breastfeeding, will only be eligible when the clinical status of the mother or child is critical. The child must be included in all stages of health care, as this commonly asymptomatic group plays an important role in the family's transmissibility of the disease. Routine immunization should be provided, as well as clinical assistance when necessary, and families must be assisted in favor of their well-being. Conclusion: at the moment, it is not possible to measure the consequences of this new pandemic on maternal and child health, demanding attention to its evolution and new evidences about the implications in mother and child care.

Despite high rates of breastfeeding initiation in Australia, there is a significant drop in breastfeeding rates in the early postpartum period, and Australian government breastfeeding targets are not being met. The Supporting breastfeeding In Local Communities (SILC) trial was a three-arm cluster randomised trial implemented in 10 Victorian local government areas (LGAs). It aimed to determine whether early home-based breastfeeding support by a maternal and child health nurse (MCH nurse) with or without access to a community-based breastfeeding drop-in centre increased the proportion of infants receiving ‘any’ breast milk at four months. Focus groups, a written questionnaire and semi-structured interviews were undertaken to explore the interventions from the perspective of the SILC-MCH nurses (n=13) and coordinators (n=6), who established and implemented the interventions. Inductive thematic analysis was used to identify themes, then findings further examined using Diffusion of Innovations Theory as a framework. SILC-MCH nurses and coordinators reported high levels of satisfaction, valuing the opportunity to improve breastfeeding in our community; and having focused breastfeeding time with women in their own homes. They felt the SILC interventions offered benefits to women, nurses and the MCH service. Implementing new interventions into existing, complex community health services presented unforeseen challenges, which were different in each LGA and were in part due to the complexity of the individual LGAs and not the interventions themselves. These findings will help inform the planning and development of future programs aimed at improving breastfeeding and other interventions in MCH.
