Objectives The quality of sexual life of elderly people represents an understudied topic of sexual medicine and of psycho-sexology. Hence, we aimed to evaluate the association of specific psycho-relational variables, such as intimacy, increased affective gestures towards a partner (AGtP), daily shared activities, and dyadic satisfaction, with the psychosexual wellbeing of elderly people, expressed in terms of sexual satisfaction. Methods: A cohort of elderly people was selected from a sample of a broader study evaluating the role of sexual activity in protecting the emotional wellbeing of a population subjected to quarantine due to the COVID-19 pandemic. Specifically, the presence of sexual dysfunctions, the emotional wellbeing (i.e., absence of anxiety and/or depression), and the quality of the partners’ relationships were studied. For the study’s purpose, the Sexual Health Inventory for Males (SHIM), the Female Sexual Function Index (FSFI), the Orgasmometer and the Orgasmometer-F, the Generalized Anxiety Disorder scale (GAD-7), the Patient Health Questionnaire (PHQ-9), and the Dyadic Adjustment Scale were adopted. Results: A group of 124 elderly subjects (≥60) was selected for the study’s purposes. Among these, 84% were males (120/124), and 16% were females (20/124). All the subjects declared to be in a stable relationship and to be sexually active during the first lockdown period. Gender differences were found for the Dyadic Satisfaction subscale (males: 37.04 ± 6.57; females: 32.85 ± 10.04; p < 0.05) and the Orgasmometer (males: 7.64 ± 1.30; females: 6.60 ± 2.46; p < 0.01). Linear regression analysis showed the association between higher Orgasmometer scores and: (i) the absence of sexual dysfunctions (β = −1.213; SE = 0.271; p < 0.0001), (ii) higher dyadic satisfaction (β = 0.042; SE = 0.019; p < 0.05), and (iii) reduced shared activities with partner (β = −0.463; SE = 0.143; p < 0.01) and increased affective gestures towards partner (DAS measured AGtP) (β = 0.595; SE = 0.065; p < 0.0001). Post hoc analysis of ANCOVA with the Bonferroni correction method showed a significant difference in the Orgasmometer scores between subjects with and without sexual dysfunction (mean difference: 2.102; SE = 0.340; pBonf < 0.001), with healthy subjects reporting higher scores compared to dysfunctional ones. Conclusions: It is reasonable to suppose that, beyond the presence of sexual dysfunctions, the sexual health of elderly people may benefit from the quality of the relationship, and, specifically, from the presence of affective gestures towards the partner and the dyadic satisfaction. To the contrary, the quantity of time spent together, sharing specific activities, may be considered a factor worsening relational and sexual health. These data should be considered during the evaluation of sexual health among elderly people.

: Female sexual dysfunction (FSD) is an underinvestigated comorbidity of diabetes mellitus, often not evaluated in diabetes clinics. Diabetic women should be encouraged to talk about this topic by their diabetologist, because these problems could be comorbid to cardio-metabolic alterations, as it happens in the male counterpart. This review summarizes evidence on sexual dysfunction characteristics in diabetic women, exploring possible underlying pathogenic mechanisms. The role of hypoglycemic drugs in this context was also evaluated. To date, no specific questionnaire has been designed for the assessment of sexual dysfunctions in diabetic female patients but the use of colour-doppler ultrasound of clitoral arteries has been highlighted as a useful tool for the assessment of cardiovascular risk in these women. Similarly, no specific guidelines are available for the treatment of FSD in the diabetic population but patients should be supported to have a healthy lifestyle and, in the absence of contraindications, can benefit from already approved treatments for FSD.

Male reproductive health depends upon the normal structures and functions of Shukrava srotas. Shukra is composed of both sperm and semen and considered one of the important factors for fertilization. It is stated as Phalavat shukra. Preceptors of Ayurveda have described eight types of Shukra dosha and their treatments. Low sperm count, decrease sperm motility, abnormal sperm morphology, ejaculatory problems, sexual dysfunctions, environmental exposures (radiation, pollution, and stress etc), lifestyle habits (smoking, alcohol, recreational drugs etc), varicocele, hormonal imbalances, DNA damage, and reactive oxygen species (ROS) etc are causative factors for Male infertility. Thousands of years back, ancient system of medicine has mentioned the word Purusha vandya (male infertility) and different pharmacological activities, medicinal formulas, therapies to treat sperm disorders, semen impairments, and sexual dysfunctions. Vajeekarana is one of the special branches of Astanga Ayurveda which maintains the fertility and management of male infertility. Shukrala, Shukra janana, Shukra shodhana, Shukra rechaka, Shukra pravataka and Shukra sthambhaka etc., are the pharmacological activities mentioned to treat Shukradoshas. Shukrala karma mainly composed of Shukra vrudhikara and Shukra srutikara means which enhances the Shukra (semen and sperm) quantitatively and qualitatively and facilitates its ejaculation. Therefore, an attempt has been made to establish the concept of Shukrala karma and its therapeutic applicability in the management of male infertility.

Most of the Hakeems are practice on sexual disorders like Decrease in sperm count, decrease of viscosity of semen, erectile dysfunction, pre mature ejaculation and fluidity of sperms are common sexual dysfunctions of males and Leucorrhoea, Amenorrhea and Lactational amenorrhea (Postpartum infertility) are the common sexual dysfunctions of females of human being. The present study on these diseases is based on the collection of data from Hakeems of Malegaon region who practicing on these diseases from generations about 90 to 150 years.

Abstract Age and sexual difficulties show a relevant influence in older adults’ sexual well-being (SWB). The objective of this study was to assess sexual issues that affect SWB, revealed by older adults in person-centered therapy, using qualitative research. Interviews with 38 older participants, aged 65 to 82 years, living in the community were submitted to content analysis. Eight main themes emerged from the results of the content analysis: Partner unavailability, family issues, physical changes, worries about hygiene, sexual dysfunctions, fear of physical abuse, and sexual transmitted diseases. This study was relevant towards identifying the sexual issues older adults feel regarding their SWB, as shared in therapy. Older adults referred their greatest issues to be partner unavailability, sexual dysfunctions and physical changes due to aging.

Introduction: Sexuality is way people experience and express themselves sexually. It influences psychological, physical and social well-being of both men and women. Sexual dysfunctions are believed to be among the commonly prevalent psychological disorders in the general population but there is a lack of published research papers and literature related to sexuality and sexual medicine from Nepal. The aim of the present study was to descriptively analyze the nature of sexual dysfunctions in a teaching hospital. Material And Method: Retrospective analysis of data from subjects attending the psychiatry outpatient department in Nepalgunj Medical College Teaching Hospital, Kohalpur over the calendar year 2019 was done. Sociodemographic and clinical parameters of the subjects were obtained from the OPD register. Diagnosis was made using the DSM-5. Results: Out of total 54 subjects, 92.6% were male while 7.4% were female. Married subjects constituted 79.6% while single/unmarried 20.4%. Premature ejaculation was the most common sexual dysfunction encountered (55.5%) followed by erectile disorder (33.3%) and female sexual interest/arousal disorder (5.5%). Male hypoactive sexual desire disorder was seen in 3.7% and genito-pelvic pain/penetration disorder was seen in 2%. Conclusion: Males more commonly seek medical help for sexual dysfunction than females. People especially females, hesitate to discuss and seek medical help for sexual dysfunctions. There is lack of research on sexual medicine and sexuality from Nepal and prevalence of various sexual dysfunctions is not known.
