Long-Range White Matter Microstructural Alterations in Drug-Naive Children with Attention Deficit Hyperactivity Disorder: A Tract-Based Spatial Statistics Study

Background Microstructural changes might underlie white matter (WM) pathology in attention deficit hyperactivity disorder (ADHD). To investigate WM alterations, particularly the changes in long-range fibers, in drug-naive children with ADHD, we conducted tract-based spatial statistics (TBSS) analysis on diffusion tensor imaging (DTI) data. Materials and Methods In this study, 57 children with ADHD and 41 healthy controls (HCs) were enrolled. None of the enrolled ADHD children received any medication before data collection. The difference in fractional anisotropy (FA), and in mean (MD), axial (AD), and radial diffusivity (RD) between both groups were calculated using TBSS. WM changes were then correlated with clinical symptoms, including the hyperactivity index score and the impulsivity score. Results Whole-skeleton analysis identified several long-range fibers of decreased FA and increased RD in the ADHD group as compared to the HC group. ADHD children demonstrated decreased FA in the right corpus callosum (CC) splenium, left inferior fronto-occipital fasciculus, and intersection of the anterior and posterior internal capsule. Moreover, higher RD was observed in the right CC splenium, superior longitudinal fasciculus, and posterior corona radiata. No regions of increased FA or reduced RD were observed, and no differences in MD or AD were noted. Conclusion Our results demonstrate that microstructural WM alterations and changes in the long-range WM connections are present in children with ADHD. We speculate that these changes may relate to the symptoms of hyperactivity and impulsivity.

تحليل التباين المكاني لتوزيع مستفيدي خدمة الرعاية الصحية المنزلية بغرب مدينة الرياض

تتناول هذه الدراسة تحليل التوزيع المكاني للمستفيدين من خدمة الرعاية الصحية المنزلية التي يقدمها مستشفى الملك سلمان على مستوى أحياء غرب مدينة الرياض، وترجع أسباب اختيار هذا الموضوع للدراسة لسببين رئيسيين هما؛ أهمية هذه الخدمة الصحية المنزلية التي تعد حديثة بالمملكة العربية السعودية لا تقل أهميتها ونوعيتها عن الخدمة المقدمة على مستوى المستشفيات والمراكز الطبية من جهة ونظراً لما توفره للمستفيد من راحة نفسية ومزايا اجتماعية بدلاً عن التكاليف والجهد الذي يتطلبه في مدة إقامته للاستشفاء بالمستشفى في الظروف المعتادة. - أهداف الدراسة: تهدف هذه الدراسة إلى: 1- تحليل التباين المكاني لتوزيع إجمالي السكان وتوزيع السكان بحسب النوع (إناث وذكور) وتوزيع إجمالي المستفيدين من خدمة الرعاية الصحية المنزلية وتوزيع المستفيدين بحسب النوع (إناث وذكور) وبحسب الفئتين العمريتين (أكبر من 65 سنة) و (14-65 ستة) في عام 2020. 2- تحديد نمط التوزيع المكاني لإجمالي السكان وللسكان بحسب النوع (إناث وذكور) ولإجمالي المستفيدين من خدمة الرعاية الصحية المنزلية وللمستفيدين بحسب النوع (إناث وذكور) وبحسب الفئتين العمريتين (أكبر من 65 سنة) و (14-65 ستة) في عام 2020. - منهجية الدراسة: لتحقيق أهداف البحث، اعتمدت هذه الدراسة على المنهج الاستقرائي في تحليل التباين المكاني لتوزيع إجمالي السكان وللسكان بحسب النوع (إناث وذكور) ولإجمالي المستفيدين من خدمة الرعاية الصحية المنزلية وللمستفيدين بحسب النوع (إناث وذكور) وبحسب الفئتين العمريتين (أكبر من 65 سنة) و (14-65 ستة) في عام 2020. وقد اعتمدت منهجية الدراسة على التوظيف المتكامل لكل من: (أ)- الأساليب الإحصائية المتاحة بأدوات التحليل Analyze في شريط أدوات الحزمة الإحصائية SPSS23 بتطبيق اختبار Shapiro-Wilk المناسب لمعالجة وفحص شكل توزيع بيانات السكان والمستفيدين بمجموع 20 حي بهدف تحديد اختبار نوع اختبار الأهمية الإحصائية المناسب لحجم العينات (20 حي) ولشكل توزيع بياناتها. (ب)- تقانات نظم المعلومات الجغرافية باستعمال أدوات الإحصاء المكاني Spatial Statistics Tools المتاحة بشريط الأدوات Arc Tool Box في برنامج نظم المعلومات الجغرافية ArcGIS 10.7، أهمها نموذج الارتباط الذاتي Autocorrelation Model المعروف بمسمى مؤشر موران Moran’s Index) في برامج نظم المعلومات الجغرافية (ArcGIS10.7) لتحديد نمط التوزيع المكاني لإجمالي المستفيدين من خدمة الرعاية الصحية المنزلية وللمستفيدين بحسب النوع (إناث وذكور) وبحسب الفئتين العمريتين (أكبر من 65 سنة) و (14-65 ستة) في عام 2020. - بيانات الدراسة: تتشكل بيانات الدراسة من: (أ)- بيانات توزيع المرضى بحسب النوع (إناث وذكور) وبحسب الفئتين العمريتين (أكبر من 65 سنة) و (14-65 سنة) بمجموع 17 حي وبيانات توزيع السكان بحسب النوع (إناث وذكور) على مجموع 20 حي المستفيدين من برنامج الرعاية الصحية المنزلية المتاحة بمستشفى الملك سلمان لعام 1442هـ (2021م) الذين يبلغ عددهم (322). (ب)- بيانات التعداد الرسمي للسكان في عامي 2010 و2020. (ج)- البيانات المكانية لتوزيع المستفيدين المتاحة من طرف الهيئة الملكية لمدينة الرياض في قاعدة بيانات جغرافية لأحياء مدينة الرياض على هيئة مساحية وشبكة الطرق على هيئة خطية ونقاط مواقع المستشفيات الحكومية والمراكز الصحية التابعة لوزارة الصحة ومواقع المرضى المسجلين للزيارة بمستشفى الملك سلمان في منطقة الدراسة في عام 1441هـ (2020م) على هيئة جداول (Excel). - نتائج الدراسة: أظهرت نتائج اختبار Shapiro-Wilk أن توزيع مساحات الأحياء وتوزيع بيانات السكان والكثافة السكانية والمستفيدين من خدمة الرعاية الصحية المنزلية في عام 2020 بأحياء غرب مدينة الرياض المدروسة يختلف عن التوزيع الطبيعي (المعتدل). ودلت نتائج اختبار Levene أن جميع البيانات تتسم بتجانس تباينها، عدا بيانات الكثافة السكانية والمستفيدين (إناث) والمستفيدين للفئة العمرية أكبر من 65 سنة. وعليه فإن نتائج اختبار ذي الحدين Binomial أظهرت أن تباين توزيع مساحات الأحياء وبيانات السكان المستفيدين لا يحظى بأية أهمية احصائية. وتتوافق نتائج الارتباط الذاتي (مؤشر Moran’s Index) مع هذه النتائج، بحيث تراوحت قيم (Z) المعيارية بين (0.01-) و(1.70+) وقيم مؤشر موران (I) بين (0.06-) و(0.08+) وقيم الاحتمالية (p) بين (0.09) و (0.99)، وهي تدل على أن نمط التوزيع المكاني لإجمالي المستفيدين وتوزيع المستفيدين بحسب النوع (إناث وذكور) وتوزيع المستفيدين من الفئة العمرية (14-65 سنة) هو عشوائي، في حين أن نمط توزيع المستفيدين من الفئة العمرية (أكبر من 65 سنة) هو نمط متجمع. - الخاتمة: انتهت هذه الدراسة بمجموعة من النتائج التي أفرزتها مختلف الخطوات المنهجية المطبقة في فحص شكل توزيع بيانات السكان وبيانات المستفيدين منهم في عام 2020 بأحياء غرب مدينة الرياض مع التأكد من أهميتها الإحصائية من جهة. ومن جهة أخرى كشفت نتائج هذه الدراسة عن نمط التوزيع المكاني العشوائي الذي يتسم به توزيع السكان والمستفيدين من خدمة الرعاية الصحية المنزلية بأحياء غرب مدينة الرياض.

Reduced White Matter Integrity in Patients With End-Stage and Non-end-Stage Chronic Kidney Disease: A Tract-Based Spatial Statistics Study

Background and Purpose: Reduced white matter (WM) integrity has been implicated in chronic kidney disease (CKD), especially in end-stage renal disease (ESRD). However, whether the differences in WM abnormalities exist in ESRD and non-end-stage CKD (NES-CKD) remains unclear. Hence, this study aimed to investigate the WM microstructural changes between the two stages using diffusion tensor imaging (DTI) and explore the related influencing factors.Methods: Diffusion tensor imaging’ images were prospectively acquired from 18 patients with ESRD, 22 patients with NES-CKD, and 19 healthy controls (HCs). Tract-based spatial statistics (TBSS) was performed to assess the voxel-wise differences in WM abnormalities among the three groups. The relationships between DTI parameters and biochemical data were also analyzed.Results: Compared with NES-CKDs, FA value was significantly decreased, and AD value increased in ESRDs mainly in brain regions of bilateral anterior thalamic radiation (ATR), the genu and body of corpus callosum (CC), bilateral anterior corona radiata, superior corona radiata, and superior longitudinal fasciculus. Besides, extensive and symmetrical deep WM damages were observed in patients with ESRD, accompanied by increased MD and RD values. Multiple regression analysis revealed that uric acid and serum phosphorus level can be used as independent predictors of WM microstructural abnormalities in clusters with statistical differences in DTI parameters between ESRD and NES-CKD groups.Conclusion: In the progression of CKD, patients with ESRD have more severe WM microstructural abnormalities than NES-CKDs, and this progressive deterioration may be related to uric acid and phosphate levels.

The Science of Data, Data Science: Perversions and Possibilities in the Anthropocene Through a Spatial Justice Lens

In the Anthropocene statistics, data science, and mathematical models have become a perversion of reality that society has largely chosen to ignore and is embraced as a great savior because people often view numbers as objective purveyors of truth. However, numbers do not interpret themselves, they do not tell their own story; people do that in all their subjective glory. In this chapter, I start by making connections between the Anthropocene and the disciplines of statistics and data science specifically through the context of spatial data. From this discussion I focus on two main points, which I connect to education. The first is that there is a dialectic tension involved in spatial data enquiry between creating new realities using spatial data and using spatial data to make sense of our reality. The second point is that people can choose how to investigate and use spatial data based on their ethics. I believe students should have opportunities to investigate and use spatial statistics through a spatial justice lens both to learn about the world around them and to shape the world around them.
