Stapedectomy remains a joint key-indicator case with ossiculoplasty for otolaryngology residents in the United States. Yet, residents consistently report feeling inadequately prepared to perform stapes surgery following graduation. Applying recently described age- and sex-standardized incidence rates of surgically confirmed cases of otosclerosis to the US populace, upper and lower estimates of residents’ case exposure to stapedectomy can be approximated. With this, uppermost projections estimate 6484 new cases of stapes surgery are performed annually nationwide. With approximately 1424 otolaryngology residents nationally, the average case exposure is 7.8 stapedectomies throughout their training, with upper and lower estimates of 17.1 and 4.2 cases, respectively. As such, proficiency in stapedectomy is no longer a realistic expectation for US graduating residents. This reality supports the removal of “stapedectomy” from the list of 14 key-indicator case requirements, leaving ossiculoplasty as its own key-indicator case, thereby reinforcing true competence in this fundamental procedure for the graduating otolaryngologist.

Culturally relevant teaching and learning is a good approach for complementing equity work, but educators are not particularly familiar with it and intentionally employing it in schools although the concept of culturally relevant pedagogy or CRP has been introduced to the professional literature for more than 20 years ago. It is vital that educators understand the role of culturally relevant teaching and learning in supporting equity as a remedy for eliciting more active and productive student engagement. All educators involved in promoting and implementing equity as a solution toward student engagement and achievement must accept the reality that it needs not only understanding of what educational practice works and does not work but also patience and realistic expectation that equity's challenging work is an on-going and long-lasting advocacy. As educators who are social justice advocates, we must reimagine teaching and learning as a socially and culturally situated pedagogy to increase motivation, engagement, and hope to meet the needs of all our students, when they need them.

This chapter explains why the only value of understanding justice does not lie in whatever practical value it has. It argues that the value of understanding full justice, even if it is an unrealistic goal, is over-determined: it has several kinds of practical value, but its value does not depend on that in any case. The chapter supposes that full or great justice is not a realistic expectation, not a standard people are at all likely or inclined ever to meet, and, moreover, that there is no other way in which understanding full justice would be available for the production of something of value. It then considers a claim: in that case, there is no great value in understanding those upper reaches of justice, or in understanding whether it has such upper reaches. This is broader than the claim that such understanding is useless, which would leave the possibility of worth other than usefulness. Not only is it useless, on this view, it is, for that reason, worthless (or close enough). The chapter argues against this.

If the neonate with protruding ears, mildly constricted ears, a Stahl’s ear, or even cryptotia is seen by the plastic surgeon during the child’s first days of life, the timing is auspicious. By initiating immediate steps to mold the ear with tapes and splints, complete correction of the problem without surgery is a realistic expectation. The urgency and the effectiveness of early nonsurgical treatment of such ears is not yet widely appreciated by those responsible for primary medical care of neonates. It is the plastic surgeon’s task to heighten awareness of such treatment.

In doing the analysis on how much there is scope of multilateral diplomacy as basis for global governance, an overview on historical facts, international organizations and elements contributing to the global governance achievement will be touched upon. Impact and historical aspects of international organisations such as United Nations, European Union and World Trade organisations in setting the first pillars of global governance will be also elaborated, with an emphasis on impact and potential that these institutions have in global governance evolution.
