Osteopoikilosisis a rare inherited benign bone dysplasia incidentally found on radiological exams. It ischaracterized by a specific radiological pattern which consists in diffuse, round or oval, symmetrically shaped sclerotic bone areas distributed throughout the skeleton. It is important to do a correct diagnosis because these lesions could be easily confused with bone metastasis. We reported a case of an osteopoikilosis patient presenting to our clinic with transient loss of consciousness and without any numbness, tingling and weakness in the legs or other parts of the body. The CT scan showed multiple small sclerotic foci bone islands, scattered throughout the thoracic and lumbar spine, ribs, pelvic bone, sacrum and bilateral proximal femur. No significant increase in the activity was detected in Technetium-99m (Tc-99m) whole body bone scintigraphy. The patient was diagnosed with characteristic radiological findings of osteopoikilosis and was followed up.

Abstract Purpose To assess the rate, timing of diagnosis, and repairing strategies of vascular injuries in thoracic and lumbar spine surgery as their relationship to the approach. Methods PubMed, Medline, and Embase databases were utilized for a comprehensive literature search based on keywords and mesh terms to find articles reporting iatrogenic vascular injury during thoracic and lumbar spine surgery. English articles published in the last ten years were selected. The search was refined based on best match and relevance. Results Fifty-six articles were eligible, for a cumulative volume of 261 lesions. Vascular injuries occurred in 82% of instrumented procedures and in 59% during anterior approaches. The common iliac vein (CIV) was the most involved vessel, injured in 49% of anterior lumbar approaches. Common iliac artery, CIV, and aorta were affected in 40%, 28%, and 28% of posterior approaches, respectively. Segmental arteries were injured in 68% of lateral approaches. Direct vessel laceration occurred in 81% of cases and recognized intraoperatively in 39% of cases. Conclusions Incidence of iatrogenic vascular injuries during thoracic and lumbar spine surgery is low but associated with an overall mortality rate up to 65%, of which less than 1% for anterior approaches and more than 50% for posterior ones. Anterior approaches for instrumented procedures are at risk of direct avulsion of CIV. Posterior instrumented fusions are at risk for injuries of iliac vessels and aorta. Lateral routes are frequently associated with lesions of segmental vessels. Suture repair and endovascular techniques are useful in the management of these severe complications.

In scoliosis, most of the deformity is in the disc and occurs during the period of rapid growth. The ring apophyses form the insertion of the disc into the vertebral body, they then ossify and fuse to the vertebrae during that same crucial period. Although this must have important implications for the mechanical properties of the spine, relatively little is known of how this process takes place. This study describes the maturation pattern of the ring apophyses in the thoracic and lumbar spine during normal growth. High-resolution CT scans of the spine for indications not related to this study were included. Ossification and fusion of each ring apophysis from T1 to the sacrum was classified on midsagittal and midcoronal images (4 points per ring) by two observers. The ring apophysis maturation (RAM) was compared between different ages, sexes, and spinal levels. The RAM strongly correlated with age (R = 0.892, p < 0.001). Maturation differed in different regions of the spine and between sexes. High thoracic and low lumbar levels fused earlier in both groups, but, around the peak of the growth spurt, in girls the mid-thoracic levels were less mature than in boys, which may have implications for the development of scoliosis.

The article presents a clinical case of treatment of a patient with spondylodiscitis of two levels – thoracic and lumbar spine spondylodiscitis combined with the thoracic aorta aneurysm. A 68-year-old patient was hospitalized in the Neurosurgical Department of the M.V. Sklifosovsky Poltava Regional Clinical Hospital in August 2019 with complaints of pain and discomfort in the thoracic and lumbar spine, shoulder joints, severe weakness in the lower extremities. The multislice computed tomography with intravenous contrast revealed a sac-like aneurysm of the descending thoracic aorta at the level of vertebrae Th4-Th5 with invasion into the vertebral bodies, spondylodiscitis of vertebrae Th4-Th5 and Th11-Th12 with deformation of the spinal axis, the formation of absolute stenosis of the spinal canal at the level of vertebrae Th11-Th12. At the first stage of the surgery, the thoracic aortic arthroplasty was performed using TAA Stent Graft System (Ankura, China) 34 × 34 × 160 mm at the M.M. Amosov National Institute of Cardiovascular Surgery and the exclusion of the descending aorta aneurysm from the bloodstream was maintained. At the second stage, transpedicular stabilization of the spine at the level of vertebrae Th10-Th11-L2-L3 was performed at the M.V. Sklifosovsky Poltava Regional Clinical Hospital using Legacy system (Medtronic, USA) and spinal cord compression was eliminated. The treatment of spondylodiscitis at the level of the vertebrae Th4-Th5 was conservative with the use of osteotropic antibacterial drugs. The patient was mobilized in the early post-surgical period after stabilization of the spine. Regression of pain syndrome and lower paraparesis was noted. Further observation was performed on an outpatient basis. According to the modified Rankin Scale, which allows assessing the degree of independence of the patient in everyday life, the patient was evaluated with 3 points at discharge. A follow-up examination at 3, 12, and 24 months showed that the functioning of the stent-graft and transpedicular system were satisfactory. The result of 0 points according to the modified Rankin Scale.Patients with the thoracic spine spondylodiscitis require special attention and additional diagnostic procedures. The risk of additional combined pathology in the form of thoracic and abdominal aorta aneurysms should be considered when planning surgical treatment.

Fractures of the thoracic and lumbar spine are the main volume of injuries in the surgical practice of a neurosurgeon. Classification of injuries is important for standardizing the description of vertebral fractures and collecting statistical data. Important classification parameters are completeness, reliability, and reproducibility. In this paper, a detailed description and illustration of the most significant classifications of injuries of the thoracic and lumbar spine is carried out.
