Motor variability in gait is frequently linked to fall risk, yet field-based biomechanical joint evaluations are scarce. We evaluated the validity and sensitivity of an inertial measurement unit (IMU)-driven biomechanical model of joint angle variability for gait. Fourteen healthy young adults completed seven-minute trials of treadmill gait at several speeds and arm swing amplitudes. Trunk, pelvis, and lower-limb joint kinematics were estimated by IMU- and optoelectronic-based models using OpenSim. We calculated range of motion (ROM), magnitude of variability (meanSD), local dynamic stability (λmax), persistence of ROM fluctuations (DFAα), and regularity (SaEn) of each angle over 200 continuous strides, and evaluated model accuracy (RMSD: root mean square difference), consistency (ICC2,1: intraclass correlation), biases, limits of agreement, and sensitivity to within-participant gait responses (effects of speed and swing). RMSDs of joint angles were 1.7–7.5° (pooled mean of 4.8°), excluding ankle inversion. ICCs were mostly good to excellent in the primary plane of motion for ROM and in all planes for meanSD and λmax, but were poor to moderate for DFAα and SaEn. Modelled speed and swing responses for ROM, meanSD, and λmax were similar. Results suggest that the IMU-driven model is valid and sensitive for field-based assessments of joint angle time series, ROM in the primary plane of motion, magnitude of variability, and local dynamic stability.

Various factors are responsible for injuries that occur in the U.S. Army soldiers. In particular, rucksack load carriage equipment influences the stability of the lower extremities and possibly affects gait balance. The objective of this investigation was to assess the gait and local dynamic stability of the lower extremity of five subjects as they performed a simulated rucksack march on a treadmill. The Motek Gait Real-time Interactive Laboratory (GRAIL) was utilized to replicate the environment of the rucksack march. The first walking trial was without a rucksack and the second set was executed with the All-Purpose Lightweight Individual Carrying Equipment (ALICE), an older version of the rucksack, and the third set was executed with the newer rucksack version, Modular Lightweight Load Carrying Equipment (MOLLE). In this experiment, the Inertial Measurement Unit (IMU) system, Dynaport was used to measure the ambulatory data of the subject. This experiment required subjects to walk continuously for 200 seconds with a 20kg rucksack, which simulates the real rucksack march training. To determine the dynamic stability of different load carriage and normal walking condition, Local Dynamic Stability (LDS) was calculated to quantify its stability. The results presented that comparing Maximum Lyapunov Exponent (LyE) of normal walking was significantly lower compared to ALICE (P=0.000007) and MOLLE (P=0.00003), however, between ALICE and MOLLE rucksack walking showed no significant difference (P=0.441). The five subjects showed significantly improved dynamic stability when walking without a rucksack in comparison with wearing the equipment. In conclusion, we discovered wearing a rucksack result in a significant (P <  0.0001) reduction in dynamic stability.

Accurate coordination of mediolateral foot placement, relative to the center of mass kinematic state, is one of the mechanisms which ensures mediolateral stability during human walking. Previously, we found that shoes constraining ankle moments decreased foot placement accuracy, presumably by impairing control over movement of the swing leg. As such, ankle moment constraints can be seen as a perturbation of foot placement. Direct mechanical perturbations of the swing leg trajectory can improve foot placement accuracy as an after-effect. Here, we asked whether constrained ankle moments could have a similar effect. If confirmed, this would offer a simple training tool for individuals with impaired foot placement control. Participants (n=19) walked in three conditions; normal (baseline, 10 minutes), while wearing shoes constraining ankle moments (training, 15 minutes), and normal again (after-effects, 10 minutes). Foot placement accuracy was calculated as the percentage of variance in foot placement that could be predicted based on the center of mass kinematic state in the preceding swing phase. When walking with constrained ankle moments, foot placement accuracy decreased initially compared to baseline, but it gradually improved over time. In the after-effect condition, foot placement accuracy was higher than during baseline, but this difference was not significant. When walking with constrained ankle moments, we observed increased step width, decreased stride time and reduced local dynamic stability. In conclusion, constraining ankle moment control deteriorates foot placement accuracy. A non-significant trend towards improved foot placement accuracy after prolonged exposure to constrained ankle moments, allows for speculation on a training potential.

Since most sporting injuries occur at the lower extremity (50% to 66%) and many of those injuries occur at the knee (30% to 45%), it is important to have robust metrics to measure risk of knee injury. Dynamic measures of knee stability are not commonly used in existing metrics but could provide important context to knee health and improve injury screening effectiveness. This study used the Local Dynamic Stability (LDS) of knee kinematics during a repetitive vertical jump to perform a post-hoc previous injury classification of participants. This study analyzed the kinematics from twenty-seven female collegiate division 1 (D1) soccer, D1 basketball, and club soccer athletes from Auburn University (height = 171 ± 8.9cm, weight = 66.3 ± 8.6kg, age = 19.8 ± 1.9yr), with 7 subjects having sustained previous knee injury requiring surgery and 20 subjects with no history of injury. This study showed that LDS correctly identified 84% of previously injured and uninjured subjects using a multivariate logistic regression during a fatigue jump task. Findings showed no statistical difference in kinematic position at maximum knee flexion during all jumps between previously injured and uninjured subjects. Additionally, kinematic positioning at maximum knee flexion was not indicative of LDS values, which would indicate that future studies should look specifically at LDS with respect to injury prevention as it cannot be effectively inferred from kinematics. These points suggest that the LDS preserves information about subtle changes in movement patterns that traditional screening methods do not, and this information could allow for more effective injury screening tests in the future.

AbstractUnderstanding the modulations of motor control in the presence of perturbations in task conditions of varying complexity is a key element towards the design of effective perturbation-based balance exercise programs. In this study we investigated the effect of mechanical perturbations, induced by an unstable surface, on muscle activation and visuo-postural coupling, when actively tracking target motion cues of different complexity. Four postural tasks following a visual oscillating target of varying target complexity (periodic-sinusoidal vs. chaotic-Lorenz) and surface (stable-floor vs. unstable-foam) were performed. The electromyographic activity of the main plantarflexor and dorsiflexor muscles was captured. The coupling between sway and target was assessed through spectral analysis and the system’s local dynamic stability through the short-term maximum Lyapunov exponent. We found that external perturbations increased local instability and deteriorated visuo-motor coupling. Visuo-motor deterioration was greater for the chaotic target, implying that the effect of the induced perturbations depends on target complexity. There was a modulation of the neuromotor system towards amplification of muscle activity and coactivation to compensate surface-related perturbations and to ensure robust motor control. Our findings provide evidence that, in the presence of perturbations, target complexity induces specific modulations in the neuromotor system while controlling balance and posture.

AbstractWalking while using a mobile phone has been shown to affect the walking dynamics of young adults. However, this has only been investigated using treadmill walking at a fixed walking speed. In this study, the dynamics of over ground walking were investigated using lower trunk acceleration measured over 12 consecutive trials, following differing walking speed and mobile phone use instructions. Higher walking speed significantly increased the proportion of acceleration along the vertical measurement axis, while decreasing the proportion of acceleration along the anteroposterior axis (p < 0.001). Moreover, higher walking speed also resulted in increased sample entropy along all measurement axes (p < 0.05). When walking while texting, the maximum Lyapunov exponent increased along the anteroposterior and vertical measurement axes (p < 0.05), while sample entropy decreased significantly along the vertical axis (p < 0.001). Walking speed and mobile phone use both affect the walking dynamics of young adults. Walking while texting appears to produce a reduction in local dynamic stability and an increase in regularity, however, caution is required when interpreting the extent of this task effect, since walking speed also affected walking dynamics.

AbstractStable walking relies critically on motor responses to signals of head motion provided by the vestibular system, which are phase-dependent and modulated differently within each muscle. It is unclear, however, whether these vestibular contributions also vary according to the stability of the walking task. Here we investigate how vestibular signals influence muscles relevant for gait stability (medial gastrocnemius, gluteus medius and erector spinae) – as well as their net effect on ground reaction forces – while humans walked normally, with mediolateral stabilization, wide and narrow steps. We estimated coherence of electrical vestibular stimulation (EVS) with muscle activity and mediolateral ground reaction forces, together with local dynamic stability of trunk kinematics. Walking with external stabilization increased local dynamic stability and decreased coherence between EVS and all muscles/forces compared to normal walking. Wide-base walking also decreased vestibulo-motor coherence, though gait stability did not differ. Conversely, narrow-base walking increased local dynamic stability, but produced muscle-specific increases and decreases in coherence that resulted in a net increase in vestibulo-motor coherence with ground reaction forces. Overall, our results show that while vestibular contributions may vary with gait stability, they more critically depend on the stabilization demands (i.e. control effort) needed to maintain a stable walking pattern.
