The aim of this study was to develop and apply an evaluation method for assessing the accuracy of a novel 3D EPID back-projection algorithm for in vivo dosimetry. The novel algorithm of Dosimetry Check (DC) 5.8 was evaluated. A slab phantom homogeneously filled, or with air and bone inserts, was used for fluence reconstruction of different squared fields. VMAT plans in different anatomical sites were delivered on an anthropomorphic phantom. Dose distributions were measured with radiochromic films. The 2D Gamma Agreement Index (GAI) between the DC and the film dose distributions (3%, 3 mm) was computed for assessing the accuracy of the algorithm. GAIs between films and TPS and between DC and TPS were also computed. The fluence reconstruction accuracy was within 2% for all squared fields in the three slabs’ configurations. The GAI between the DC and the film was 92.7% in the prostate, 92.9% in the lung, 96.6% in the head and the neck, and 94.6% in the brain. An evaluation method for assessing the accuracy of a novel EPID algorithm was developed. The DC algorithm was shown to be able to accurately reconstruct doses in all anatomic sites, including the lung. The methodology described in the present study can be applied to any EPID back-projection in vivo algorithm.

Abstract Background Radiochromic films have many applications in radiology and radiation therapy. Generally, the dosimetry system for radiochromic film dosimetry is composed of radiochromic films, flatbed scanner, and film analysis software. The purpose of this work is to present the effectiveness of a protocol for accurate radiochromic film dosimetry using Radiochromic.com as software for film analysis. Materials and methods Procedures for image acquisition, lot calibration, and dose calculation are explained and analyzed. Radiochromic.com enables state-of-the-art models and corrections for radiochromic film dosimetry, such as the Multigaussian model for multichannel film dosimetry, and lateral, inter-scan, and re-calibration corrections of the response. Results The protocol presented here provides accurate dose results by mitigating the sources of uncertainty that affect radiochromic film dosimetry. Conclusions Appropriate procedures for film and scanner handling in combination with Radiochromic.com as software for film analysis make easy and accurate radiochromic film dosimetry feasible.

Evaluation of the dose distribution delivered in a radiosurgery treatment of an acoustic neuroma using an adapted Alderson phantom and radiochromic film to assess absolute dose. A system was developed to perform the dose measurements, using the Alderson Rando phantom´s head carrying a modified slice, where a material reproducing the tumor was inserted. The phantom’s head with the modified slice was irradiated in a medical linear accelerator, using a procedure similar to that adopted in real cases, and radiochromic films were used to assess dose values. The suitability of the dose distribution delivered in the treatment was evaluated by comparing the Dose Volume Histograms, obtained from the Treatment Planning System (TPS) and by radiochromic film measurements. An agreement around 3% between experimental data and TPS´s calculations, showing an acceptable concordance with the planning results. The established approach of transforming 2D arrays of dose in a 3D one is satisfactory, validating it. The developed method shows to be an excellent alternative to quality control in radiosurgery using the radiochromic film to assess absolute dose.

AbstractThis work presents the first tests performed with radiochromic films and a new Micro‒Opto‒Electro-Mechanical system (MOEMS) for in situ dosimetry evaluation in radiotherapy in real time. We present a new device and methodology that overcomes the traditional limitation of time-delay in radiochromic film analysis by turning a passive detector into an active sensor. The proposed system consists mainly of an optical sensor based on light emitting diodes and photodetectors controlled by both customized electronic circuit and graphical user interface, which enables optical measurements directly. We show the first trials performed in a low‒energy proton cyclotron with this MOEMS by using gafchromic EBT3 films. Results show the feasibility of using this system for in situ dose evaluations. Further adaptation is ongoing to develop a full real‒time active detector by integrating MOEM multi‒arrays and films in flexible printed circuits. Hence, we point to improve the clinical application of radiochromic films with the aim to optimize radiotherapy treatment verifications.

Radiochromic films are mainly used for two-dimensional dose verification in photon, electron, and proton therapy treatments. Moreover, the radiochromic film types available today allow their use in a wide dose range, corresponding to applications from low-medical diagnostics to high-dose beam profile measurements in charged particle medical accelerators. An in-depth knowledge of the characteristics of radiochromic films, of their operating principles, and of the dose reading techniques is of paramount importance to exploit all the features of this interesting and versatile radiation detection system. This short review focuses on these main aspects by considering the most recent works on the subject.
