Urinary bladder leiomyoma is a rare tumour accounting for less than 0.5% of all urinary bladder tumours. Till now, less than 250 cases were documented with variable sizes, most of them were less than 10 cm in maximum diameter. Here we present a 68- year-old female patient with urinary bladder giant leiomyoma measuring about 13 cm. She presented with right loin pain. Postcontrast computed tomography of the abdomen and pelvis revealed a large posterolateral right-sided urinary bladder mass with moderate right hydroureteronephrosis. It was managed by partial cystectomy. The patient had an uneventful postoperative course. Postoperative pathological examination of the specimen confirmed giant leiomyoma of the urinary bladder.

We present a unique case of bladder leiomyoma that was mistakenly diagnosed as a ureterocele. The delay in diagnosis meant ongoing significant voiding lower urinary tract symptoms, which could have been avoided. This was eventually successfully treated by transurethral resection. Leiomyoma is the most common benign bladder mass and should be considered in the differential diagnosis when a smooth mass with normal overlying mucosa is seen on cystoscopy or a homogeneous, low density bladder mass is seen on cross-sectional imaging. Despite the benign nature of the lesion, leiomyoma can convey significant morbidity to the patient.

Benign neoplasms of the bladder are a rare pathology. Non-epithelial benign tumors of the bladder (fibromas, fibromyxomas, fibromyomas, hemangiomas, rhabdomyomas, leiomyomas, etc.) account for less than 0.5% of all that affect this organ. The analyzed literature describes about 250 cases of bladder leiomyoma. This article describes the clinical case of surgical treatment of bladder leiomyoma.

Introduction: Bladder leiomyoma which is commonly found in middle aged females is a rare, benign, and mesenchymal tumor of the bladder. Its incidence rate is lower than 0.5% among all types of bladder tumors. There are fewer than 250 reports on bladder leiomyoma. Herein, we report a case of bladder leiomyoma in a 37 years’ female. Case Presentation: A 37-year-old multiparous woman presented with complaints of occasional hematuria and dysuria and irritative symptoms. In the examination and imaging, a mass with size of 8 cm was reported in anterior border of uterus with firm consistency. Surgery was performed and the mass was resected. Postoperative histopathology review confirmed bladder leiomyoma. Conclusion: Leiomyoma should be kept in mind as a differential diagnosis in women presenting with dysuria, frequency or hematuria and a firm consistency mass even in unusual site as the bladder. Complete resection leads to very good outcome with almost no recurrence.
