Introdução: A diabetes mellitus (DM) é uma doença crônica no qual o corpo não produz insulina ou não consegue empregar de forma adequada a insulina que produz. É uma das enfermidades mais diagnosticadas nos últimos anos no mundo todo e afeta uma grande quantidade de indivíduos. a Polineuropatia diabética é uma das complicações mais comuns ocasionadas pela DM gerando diversos sintomas como o déficit de sensibilidade periférica dos MMII, que consequentemente aumenta o risco de quedas entre os portadores da doença. Objetivo: Analisar tratamentos de cinesioterapia e propriocepção, e sua eficácia com déficit sensórios motores causados pela polineuropatia diabética. Materiais e Métodos: Por meio de uma revisão sistemática, através de buscas em bases de dados, foram admitidos apenas ensaios clínicos publicados entre os anos de 2010 e 2020. A busca envolveu as bases de dados SciELO, BVS, PubMed, PEDro e Google acadêmico, utilizando os descritores “Physical Therapy Specialty”; “Diabetic Neuropathies”; “Exercise Therapy”; “Proprioception”; “Adult”; “Aged”. A triagem dos artigos, foi realizada por dois pesquisadores, e foram incluídos ensaios clínicos, teses e doutorados que abordassem trabalho proprioceptivo e de equilíbrio em pacientes neuropatas diabéticos. Resultados: Na análise inicial, a busca resultou em 69 registros potencialmente relevantes, porém 57 foram excluídos e apenas 12 contemplaram os critérios de seleção e destes todos atingiram a pontuação estabelecida na escala PEDro. Conclusão: Por meio desta análise metodológica, foi possível concluir que um trabalho postural, de equilíbrio estático e dinâmico com propriocepção é de fato efetivo, trazendo melhora das disfunções dos portadores de Polineuropatia diabética. Porém, alguns protocolos ainda necessitam de estudos e maior tempo de aplicação para concluir sua eficácia.

Abstract Introduction: Diabetic neuropathies are thought to result from diabetic microvascular injury involving small blood vessels that supply nerves. Cardiac autonomic neuropathy (CAN) represents a serious complication associated with Diabetic neuropathies. Objectives: The aim of the study is to investigate the presence of cardiovascular diabetic autonomic neuropathy in a group of adult diabetic Sudanese patients with ischemic heart disease. Methodology: This is a descriptive prospective cross sectional hospital based study. The study population included type 1 and type 2 diabetic patients admitted to the cardiac care unit in ELshaab Teaching Hospital with acute coronary syndrome over a period of two years, from April 2017to April 2019. Results: A total of 49 males (65.33%) and 26 females (34.67%) were included in the study. CAN was detected in 94.67% of the studied group Conclusion: An incidence of 94.67% cardiovascular autonomic neuropathy in our sample indicates a high occurrence of this syndrome among our diabetic patients. Poor glycemic control is a common feature among our studied group. Sub-clinical autonomic neuropathy can be detected early using autonomic function tests.

: The various manifestations of diabetic neuropathy including distal symmetric sensorimotor polyneuropathy (DSPN) and cardiovascular autonomic neuropathy (CAN) are among to the most prevalent chronic complications of diabetes. Major clinical complications of diabetic neuropathies such as neuropathic pain, chronic foot ulcers, and orthostatic hypotension are associated with considerable morbidity, increased mortality, and diminished quality of life. Despite the substantial individual and socioeconomic burden, the strategies to diagnose and treat diabetic neuropathies remain insufficient. This review provides an overview on the current clinical aspects and recent advances in exploring local and systemic biomarkers of both DSPN and CAN assessed in human studies (such as biomarkers of inflammation and oxidative stress) to better understand the underlying pathophysiology and to improve early detection. Current therapeutic options for DSPN are (I) causal treatment including lifestyle modification, optimal glycemic control, and multifactorial risk intervention; (II) pharmacotherapy derived from pathogenetic concepts; and (III) analgesic treatment against neuropathic pain. Recent advances in each category are discussed including non-pharmacological approaches such as electrical stimulation. Finally, the current therapeutic options for cardiovascular autonomic complications are provided. These insights should contribute to a broader understanding of the various manifestations of diabetic neuropathies from both the research and clinical perspectives.

Objetivo: Investigar a associação entre neuropatia diabética e sintomas do trato urinário inferior nas pessoas com diabetes mellitus. Métodos: Revisão integrativa da literatura, que seguiu seis etapas; a busca ocorreu no mês de outubro de 2019, no Portal da Biblioteca Virtual de Saúde e nas bases de dados National Library of Medicine, Medical Literature Analysis and Retrieval System Online e Scopus,  utilizando os descritores e operadores booleanos “urinary incontinence” AND “diabetes mellitus” AND “diabetic neuropathies”; os critérios de inclusão foram publicações de 2009 a 2019, nos idiomas português, inglês e espanhol. Resultados: Dos 52 estudos provenientes da busca, oito compuseram a amostra final; as mulheres foram majoritárias nos estudos; a neuropatia diabética estava associada aos sintomas de trato urinário inferior, sendo a incontinência urinária, incontinência de urgência, noctúria, bexiga hiperativa e disfunção sexual feminina; a obesidade, cor branca, tempo e descontrole da doença foram fatores associados aos sintomas urinários. Considerações finais: A avaliação da sensibilidade protetiva dos pés e detecção precoce de neuropatias, oferecem uma oportunidade para a prevenção dos sintomas de trato urinário inferior. As limitações deste estudo foram a heterogeneidade metodológica dos artigos, a estratégia de busca e a falta de dados com relação ao sexo masculino.

Background: Background: Diabetic neuropathies (DN) are the most common complications in diabetic patients, affecting about 70% of them. Patients with DN may lose sensation in certain areas of the body, facilitating the onset of foot ulcers, as well as chronic pain. This is due to the progressive degeneration of nerve fibers, demyelination, and axonopathy. Recent studies about stem cell therapies for the treatment of DN show promising potential for tissue regeneration. Results with mesenchymal stem cells derived from various organs/tissues demonstrate great therapeutic potential considering their easy obtainment, as well as their immunomodulatory and pro-regenerative effects. However, problems such as cell transplant rejection, tumor formation, transplantation safety, and effectiveness still need to be solved. Developmental biology lacks detailed insights into some aspects of cell mechanisms, like the genetic components of cell growth or differentiation. These gaps can limit a rapid advance in stem cell therapy research and put it in the future a little bit farther from the expectations that have emerged in recent media. Conclusion: In the present review, we attempt to discuss the potential of most studied types of stem cells, their application for the treatment of experimental diabetic neuropathies and associated clinical manifestations including future perspectives around these themes.
