Background: Chronic primary insomnia is a prevalent sleep disorder that is associated with adverse effects on health out- comes. Sleep disturbance is usually associated with abnormal level of systemic inflammation biomarkers. Objective: The aim of this study was to detect changes in sleep quality and inflammatory markers following weight loss among subjects with chronic primary insomnia. Material and Methods: Eighty previously sedentary subjects with chronic primary insomnia subjects enrolled in this study, their age ranged from 32-51 year were randomly assigned to life style intervention group (group A, n=40) or control group (group B, n=40). Polysomnographic recordings for sleep quality assessment, IL-6, IL-10 and TNF-α were measured before and at the end of the study after six months. Results: There was a significant increase in the total sleep duration, sleep efficiency, sleep onset latency and IL-10 in addi- tion to significant reduction in awake time after sleep onset, REM latency, IL-6 and TNF-α after 6 months of in group(A) as a result of weight loss program; while the results of the control group (group B) were not significant. Also, there were significant differences between both groups at the end of the study. Conclusion: Life style intervention modulates systemic inflammatory parameters and sleep quality among subjects with chronic primary insomnia. Exercise is often considered a non-pharmacological approach that could have beneficial effects on sleep. Objective: The aim of the study was to compare the impact of aerobic and resistance exercise training on quality of sleep and inflammatory markers among subjects with chronic primary insomnia. Material and Methods: Sixty previously sedentary subjects with chronic primary insomnia subjects enrolled in this study, their age ranged from 31-52 years. All participants were randomly assigned to aerobic exercise intervention group (group A, n=35) or resistance exercise intervention group (group B, n=35). Polysomnographic recordings for sleep quality assessment, IL-6, IL-10 and TNF-α were measured before and at the end of the study after six months. Results: There was a significant increase in the total sleep duration, sleep efficiency, sleep onset latency and IL-10 in group(A) and group (B) in addition to significant reduction in awake time after sleep onset, REM latency, IL-6 and TNF-α after 6 months of aerbic and resistance exercise training. However, there were significant differences between both groups at the end of the study. Conclusion: Aerobic exercise training is more appropriately than resistance exercise training in modulation of inflammatory and sleep quality among subjects with chronic primary insomnia. We aimed to assess which quantitative EEG changes during daytime testing in patients with sleep disorder (primary insomnia and excessive daytime sleepiness groups). All experimental study participants were subjected to a long-term test for maintaining attention to sound stimuli, and their EEGs were recorded and then processed, using wavelet analysis, in order to estimate the power and frequency structure of alpha activity. In healthy subjects, the maximum increase in the alpha rhythm occurred near 9 Hz. Patients with primary insomnia were characterized by an increase in the amplitude of the alpha rhythm near 11 Hz. For subjects with sleep disorders, an increase in the amplitude of the alpha rhythm was observed in the entire frequency range (7.5–12.5 Hz), with a maximum increase at 9–10 Hz. Significant differences (p≤0.001) for changes in the alpha rhythm dynamics in the course of performing the attention test were observed in the frequency range of 7.5–10.5 Hz between the control group and patients with sleep disorders. The ratios of the alpha rhythm power values for passive stages with closed eyes before and after active stage were significantly different among the groups of healthy sleep volunteers, patients with primary insomnia, and patients with impaired sleep hygiene within the range of 9.5 to 12.5 Hz. The results of the current study supported the notion of a 24-h hyperarousal in primary insomnia.

Based on multimodal functional magnetic resonance imaging technology, explore the changes of local brain function in the whole brain range of patients with primary insomnia at rest, and conduct correlation analysis to explore the relationship between this locality and function and clinical features. Provide further imaging evidence for the exploration of the neural mechanism of primary insomnia. Using multimodal functional magnetic resonance imaging, self-rating anxiety scale, self-rating depression scale to assess the status anxiety factor and trait anxiety of STAI in patients with primary insomnia (88 cases) and normal sleepers (82 cases). Factors and total scores, depressive mental disorders and total scores were statistically significantly different from the normal sleep group. The study found that patients with primary insomnia under the multimodal functional magnetic resonance imaging assisted treatment have abnormal local functional activities in multiple brain regions such as emotions and sensorimotor regions. We explored the brain of patients with primary insomnia from the perspective of functional differentiation. Changes in nerve activity are conducive to further understanding the characteristics of nerve activity in primary insomnia.
