Radical surgical treatment, adjuvant chemotherapy and postoperative radiation therapy, used in the complex and combined treatment of breast cancer, inevitably lead to serious functional and psychosomatic disorders. One of the most common complications of breast cancer treatment is postmastectomy syndrome (PMES). That is why the study of the latter and the development of modern methods of rehabilitation of patients with an established diagnosis of PMES remain extremely relevant. To date, the pathogenetic mechanism of the development of upper limb edema and a huge range of possible methods of prevention and treatment have been described in detail. However, the choice of a rehabilitation method requires a personalized approach based on a specific clinical case, and special attention to the psychoemotional status of the patient.

Radical surgery of the breast cancer (ВС) with the radical mastectomy often lead to a dysfunction of the upper lymb on the operation side. The symptom complex of disorders is manifested by swelling, numbness, paresthesias, pain syndrome of varying severity, heaviness, convulsions, spasms, and limited mobility. It is called postmastectomy syndrome (PMS). According to literature sources, the incidence of PMS is 60–80 % of all patients undergoing mastectomy. The severity of postmasectomy syndrome in patients with an inflammatory breast cancer (IBC) and locally advanced (non inflammatory) breast cancer (LABC) was compared. It turned out , that majority of patients with IBC had postmastectomy syndrome of the III degree according to Beltran while the patients with LABC (non inflammatory) – II. The limitation of mobility of the upper limb was also more pronounced in the IBC group, while the severity of pain syndrome was the same in both groups.

Objective. Elaboration of prophylactic measures for complications after radical operations in patients, suffering mammary gland cancer, and estimation of its efficacy. Materials and methods. Immediate results of surgical treatment were analyzed in 147 patients, suffering mammary gland cancer, and distributed, depending on perioperative treatment methods, into: the1st (comparison group) - 92 women-patients, to whom a standard treatment was applied, the second one (main group) - 55 women-patients, in whom complex of prophylactic measures was applied, including organizational, as well as compressive and medicinal therapy. Results. Application of complex of prophylactic measures was accompanied by reduction of the development rate of postoperative oedema from 50% (comparison group) to 25% (p=0.003), the seroma’s formation - from 29 to 11% (p=0.010), development and severity of postmastectomy syndrome, lymphorrhea - from 29 to 18% (p=0,131) and the wounds complications - from 25 to 7.3% (p=0.007). Conclusion. After radical operations with the lymph node dissection in patients, suffering mammary gland cancer, the early morbidity rate is rather high, and mainly connected with lymphovenous drainage outflow. The complex of measures, aiming at minimization of the patients’ stationary stay, early extraction of draining tubes, early activation of the patients, the improved compressive and medicinal therapy permits to reduce the rate and severity of morbidity and to accelerate the patients’ recovery and reconvalescence.

However, significant adverse events such as сardiotoxicity and cardiovascular complications remain high. A lot of women experienced fatigue, arrhythmias, heart failure, ischemic heart disease, and pulmonary side effects induced by breast cancer treatment. The objective: to determine the features of the factor structure of functional parameters in women with postmastectomy syndrome with different levels of functional state of the cardiovascular system. Methods: analysis of sources of literature and empirical data; rheography, spirography, analysis of heart rate variability; anthropometry; mathematical statistics methods. Results. a developed method for estimating the level of functional state of the cardiovascular system of women with postmastectomy syndrome based on age and objective indicators of central hemodynamics: heart rate, minute blood volume and left ventricular work, as well as their significant contribution to overall predicted level. Based on the values of the determined parameters and coefficients of multiple regression, criteria of levels of functional state of the cardiovascular system of patients with postmastectomy syndrome are determined. The proposed method allows to carry out rapid monitoring and with a high degree of probability to assess the functional state of the cardiovascular system of the patients with postmastectomy syndrome, as well as to determine the effectiveness of therapeutic, rehabilitation measures and make adjustments to the program of rehabilitation. Defined factors of the direction of the rehabilitation process with the integral account of the predicted level of the functional state of the cardiovascular system are in the manuscript. It has been identified that the hemodynamic factor has covered 22.37% of the dispersion in women with the low level of functional state, 21.24% – with the lower-than-average level, 22.06 – with the average level; expiratory – 17.15%, 16.74% and 17.48; vegetative – 11.21%, 13.29% and 12.97% at corresponding levels at outpatient rehabilitation stage. Conclusions. obtained results are the basis for choosing priority means of physical rehabilitation during developing a program for the rehabilitation of women with postmastectomy syndrome with various levels of the functional state of the cardiovascular system.
