AbstractThe quality of life (QoL) of men with optimal outcomes after robot-assisted radical prostatectomy (RARP) is largely unexplored. Thus we assessed meaningful changes of QoL measured with the EORTC QLQ-C30 24 months after RARP according to postsurgical Cancer of the Prostate Risk Assessment score (CAPRA-S) and pentafecta criteria. 2871 prostate cancer (PCa) patients with completed EORTC QLQ-C30 were stratified according to CAPRA-S, pentafecta (erectile function recovery, urinary continence recovery, biochemical-recurrence-free survival (BFS), negative surgical margins) and 90-day Clavien–Dindo-complications (CDC) ≤ 3a. Multivariable logistic regression analyses (LRM) aimed to predict improvement of EORTC QoL. Mean preoperative QoL values did not significantly differ between CAPRA-S low- (LR) vs. high-risk (HR, 75.7 vs. 75.2; p = 0.7) and pentafecta vs. non-pentafecta groups (75.6 vs. 75.2; p = 0.6). After RARP, stable QoL rates for CAPRA-S LR vs. HR and pentafecta were 30, 26 and 30%, respectively. Corresponding improved QoL rates were 44, 32 and 47%. In LRM, CAPRA-S and pentafecta criteria were independent predictors of improved QoL. We conclude that most favourable combined outcomes after RARP might confer stable or even improved QoL but up to one third of patients might experience deterioration. This warrants further investigation how to capture the underlying cause and to address and potentially solve these perceived negative effects despite successful RARP.

Background: Increased facility surgical treatment volume is sometimes associated with improved survival in patients with cancer; however, published studies evaluating volume are heterogeneous and disparate in their patient inclusion and definition of volume. The purpose of this work was to evaluate uniformly the impact of surgical facility volume on survival in patients with cancer. Methods: The National Cancer Database was searched for patients diagnosed in 2004 through 2013 with the 12 cancers most commonly treated surgically. Facilities were stratified by 4 categories using the overall population (low, intermediate, high, and very high), each including 25% of patients, and then stratified by each individual disease site. Five-year postsurgery survival was estimated using both the Kaplan-Meier method and corresponding log-rank tests for group comparisons. Cox proportional hazard models were used to evaluate the effects of facility volume on 5-year postsurgery survival further, adjusted for multiple covariates. Results: A total of 3,923,618 patients who underwent surgery were included from 1,139 facilities. Of these, 40.4% had breast cancer, 12.8% prostate cancer, and 10.0% colon cancer. Most patients were female (65.0%), White (86.4%), and privately insured (51.6%) with stage 0–III disease (64.8%). For all cancers, the risk of death for patients undergoing surgery at very high-volume facilities was 88% of that for those treated at low-volume facilities. Hazard ratios (HRs) were greatest (very high vs low volume) for cancer of the prostate (HR, 0.66; 95% CI, 0.63–0.69), pancreas (HR, 0.75; 95% CI, 0.71–0.78), and esophagus (HR, 0.78; 95% CI, 0.73–0.83), and for melanoma (HR, 0.81; 95% CI, 0.78–0.84); differences were smallest for uterine and non–small cell lung cancers. Overall survival differences were greatest for cancers of the brain, pancreas, and esophagus. Conclusions: Patients treated surgically at higher-volume facilities consistently had improved overall survival compared with those treated at low-volume centers, although the magnitude of difference was cancer-specific.

ObjectivesTo externally validate the extraprostatic extension (EPE) grade criteria on MRI and analyze the incremental value of EPE grade to clinical models of prostate cancer.MethodsA consecutive 130 patients who underwent preoperative prostate MRI followed by radical prostatectomy between January 2015 to January 2020 in our institution were retrospectively enrolled. The EPE grade, Cancer of the Prostate Risk Assessment (CAPRA), and Memorial Sloan Kettering Cancer Center nomogram (MSKCCn) score for each patient were assigned. Significant clinicopathological factors in univariate and multivariate analyses were combined with EPE grade to build the Clinical + EPE grade model, and the CAPRA and MSKCCn score were also combined with EPE grade to build the CAPRA + EPE grade and MSKCCn + EPE grade model, respectively. The area under the curve (AUC), sensitivity and specificity of these models were calculated to evaluate their diagnostic performance. Calibration and decision curve analyses were used to analyze their calibration performance and clinical utility.ResultsThe AUC for predicting EPE was 0.767–0.778 for EPE grade, 0.704 for CAPRA, and 0.723 for MSKCCn. After combination with EPE grade, the AUCs of these clinical models increased significantly than using clinical models along (P < 0.05), but was comparable with using EPE grade alone (P > 0.05). The calibration curves of EPE grade, clinical models and combined models showed that these models are well-calibrated for EPE. In the decision curve analysis, EPE grade showed slightly higher net benefit than MSKCCn and CAPRA.ConclusionThe EPE grade showed good performance for evaluating EPE in our cohort and possessed well clinical utility. Further combinations with the EPE grade could improve the diagnostic performance of clinical models.

Aim: The aim of the study is to determine the pre-treatment prevalence and associated factors of anxiety and depression in men with cancer of the prostate. Study Design: This is a prospective, cross-sectional study. Place and Duration:  The urology unit of Ekiti State University Teaching Hospital, Ado-Ekiti. Ekiti State, Nigeria. The study period was from 1st January 2018 to 31st December 2019. Methodology: A total of 46 consented male participants recently diagnosed with cancer of the prostate (CAP) were assessed for depression and anxiety disorders using the Hospital Anxiety and Depression Scale (HADS). Participants also completed a sociodemographic questionnaire, an 8-item International Prostate Symptoms Score (IPSS) and a quality of life questionnaire (BS). Results: Forty-three and half percent (43.5%) of the participants suffered depression; anxiety disorder occurred in 21.7% of the participants. Only 2.2% of the participants had anxiety disorder without depression, while 23.9% had depression without anxiety disorder. Significant level of poor quality of life (QoL) was found among participants with depression and anxiety disorder. Having one psychiatric morbidity was associated with having another. Age of participants and other sociodemographic parameters were not associated with psychiatric morbidity. Conclusion: There is a high prevalence of anxiety disorder and depression among patients with cancer of the prostate. Assessment of these disorders should be included in the management of patients with prostate cancer to enhance early detection and prompt treatment for better patients’ clinical outcome.

The TMPRSS2:ERG fusion is considered prostate specific and has been rarely described in other tumors. We describe the case of a patient who developed lung and prostate cancers, both harboring the TMPRSS2:ERG fusion. The patient developed a cancer of the prostate with lymph node metastases and after two years a nodule of the thoracic wall. The histology and immunohistochemical profile of the two tumors were typical of prostate and lung cancers. The presence of the TMPRSS2:ERG fusion was demonstrated by next-generation sequencing on both malignancies, leading to the assumption that the lung nodule was a metastasis from the prostate cancer. The patient failed to respond to antiandrogen therapy, while chemotherapy for lung cancer led to a significant objective response. To our knowledge, this is the first case of a lung cancer harboring the TMPRSS2:ERG fusion, widening the spectrum of lung cancer-associated molecular alterations.
