Profunda Femoris Artery (PFA) arises from lateral aspect of femoral artery 3.5 cm distal to inguinal ligament. It gives lateral and medial circumflex femoral arteries from lateral and medial aspect respectively. Following variation was reported in right lower limb of a 60 year male cadaver, during routine dissection for medical students. Profunda Femoris Artery (PFA) arose from lateral aspect of femoral artery 1 cm distal to inguinal ligament, running laterally and down words parallel to femoral artery the profunda femoris passed beneath rectus femoris, sartorius and vastus medialis successively, finally pierced adductor magnus as forth perforator artery, 6 cm above knee joint.

Panchakarma is a fruitful measure and its extensive efficacies are classified in different dimensions to cure different types of complicated diseases. Vamana, Virechana, Niruha, Anuvasana and Nasya are the five procedures and subsequently even Vamana, Virechana, Basti, Nasya and Rakta Mokshana are broadly termed as Panchakarma in day to day practices.The new diseases are evolving with intense gravity and therefore to combat those, Panchakarma is a very effective measure. In present era competition and advancement have led to hectic and stressful life, man has eventually forgotten the absolute enjoyment and solitary affection of nature and as a result is facing many health problems. Gridhrasi is no exception, it is correlated to sciatica in modern science where low back ache radiating to toes in the lateral aspect of the thigh associated with stiffness is the main complaint.

We present the case of a 66-year-old, type II diabetic male with a deep wound to the plantar-lateral aspect of his right hallux. On examination, the central plantar compartment of his right foot was moderately erythematous and tender on palpation. After obtaining a deep wound culture, treatment was complicated by a progression of a group B and F beta streptococcus, necrotizing infection. The patient underwent a right hallux amputation, followed by a plantar medial incision for drainage of an abscess to the medial and central plantar compartments of the foot. Due to the extent and limb threat of the infection, the patient ultimately underwent a transmetatarsal amputation. Advanced healing modalities were also employed to decrease wound healing times, which allowed the patient to achieve early weightbearing and return to activities of daily living. This study depicts how the astute podiatric surgeon needs to make a decision in a timely manner to surgically debride all nonviable and necrotic tissue in order to minimize further amputation and preserve foot function.

Diplopodia, being a rare congenital disorder, is infrequently discussed in published texts. Most reported cases have accounted the involvement of duplicated preaxial digits with other associated organ system and physical deformities. Here, we present an unusual case of isolated diplopodia involving postaxial toes in a child with no other organ and physical abnormalities. Radiological studies revealed a set of 10-digit-duplicated foot over the lateral aspect of the native foot, complete with phalanges and its corresponding metatarsals as well as tarsals, supplied by an anomalous posterior branch of the popliteal artery. Definitive surgery was performed just before the child was learning to walk.

Objectives The aim of this study was to describe the use of an external landmark that defines the attachment of the tentorium ossium for planning a craniectomy to access the cerebellar fossa. The external landmark was defined by a line where the caudal aspect of the convexity of the cranium transitions to a flat surface in the caudal aspect of the temporal fossa. We also aimed to determine if this external landmark was present and readily visualized, and to establish its relationship to the nuchal crest using three-dimensional (3D) volume-rendered CT reconstructions created from cats with normal cranial morphology. Methods First, a case is presented for the description of an approach in a cat with a meningioma located dorsolateral to the cerebellum. Second, CT studies of five cats with normal cranial morphology were selected. Regions of interest (ROIs) were drawn at the attachment of the tentorium ossium to the cranium and nuchal crest. Three-dimensional reconstructions were developed with colored ROI overlays. The external landmark defined the tentorial attachment on all 3D reconstructions. Additionally, using the postoperative CT of the clinical case described herein, ROIs of the tentorial attachment and nuchal crest along with a third ROI, the craniectomy, were drawn and overlaid on the 3D reconstruction to illustrate the position of the craniectomy in relation to the tentorium ossium attachment and nuchal crest. Results The use of the external landmark provided for a craniectomy that enabled adequate visualization for excision of a meningioma. On all 3D reconstructions, the external landmark was present and readily visualized. Conclusions and relevance Between the attachment of the tentorium ossium and nuchal crest exists an area adequately sized for a craniectomy in cats. Clinicians can use an identifiable external landmark on the lateral aspect of the cranium to plan the rostral boundary for a craniectomy to access the cerebellar fossa in cats.

Introduction The development of stenoses after correction of an esophageal atresia or acid and lye burn of the esophagus are well-known problems in pediatric surgery. Currently, stenoses are treated in the majority of cases by repeated balloon dilatations. The diameter of the balloons used is not standardized; standard curves do not exist. The aim of this study was to evaluate the diameter of the esophagus correlated to the body weight of the children as measured in upper gastrointestinal (GI) studies to answer the important question to what extent a stenosis should be dilated. Materials and Methods Within the time period from 2011 through 2016, 60 patients with upper GI studies were selected. Evaluations were blinded to two different examiners. The diameters were measured under maximum contrast filling between the second and third rib (cranial point of measurement) and between the seventh and eighth rib (caudal point of measurement). For both, the anteroposterior and lateral aspect was examined. The diameter was calculated as the arithmetic average of both measurements within one level. The diameters were correlated to the weight of the children. Results All children (n = 38) within the 3rd to 97th weight percentile were analyzed. Linear correlation and coefficients of 0.67 at the cranial point and 0.70 at the caudal point were found. Mean diameter at the cranial point of measurement was 6.75 mm at the lowest weight (2.6 kg) and 14 mm at 74 kg. Mean weight of these children (standard deviation [SD]) was 25.3 (18.8) kg and median age was 7 years. Within weight groups (0–10 kg; 10–20 kg; 20–35 kg; 35–50 kg; >50 kg), we calculated SD and two side tested critical 95% confidence interval for all measurements (n = 74). Conclusion Although the variation in measurements is considerable, this evaluation gives a reliable hint to which extent esophageal stenoses should be dilated in relation to the body weight. To the best of our knowledge, this is the first investigation to evaluate the diameter of the esophagus in children in relation to the body weight.

Background: The volumetric and biological behaviors of equine block grafts compared with autogenous block grafts have not yet been assessed. Hence, the aim of the present study was to compare—by means of histomorphometry, immunohistochemistry and microtomography—the graft incorporation and remodeling processes of autogenous and equine xenogenous bone blocks used for mandibular lateral augmentation in rabbits. Methods: Autogenous bone grafts harvested from the iliac bony crest and equine block grafts were secured to the lateral aspect of the mandible angle of eighteen rabbits. The healing after 7, 20 and 60 days was assessed in six animals each period. Results: After 60 days, new bone was present 24.2 ± 11.2% and 31.6 ± 13.3% in the autograft and xenograft groups, respectively. A better integration to the recipient sites was observed in the autogenous compared with the xenogenous blocks. Conclusions: Both xenogenous and autogenous bone blocks presented similar percentages of newly formed bone over time. However, bone volume, the quality of the grafted area and graft incorporation to the recipient sites were superior in the autogenous compared with the equine xenogenous graft sites.

Stress-induced release of dynorphins (Dyn) activates kappa opioid receptors (KOR) in monoaminergic neurons to produce dysphoria and potentiate drug reward; however, the circuit mechanisms responsible for this effect are not known. We found that conditional deletion of KOR from Slc6a4 (SERT)-expressing neurons blocked stress-induced potentiation of cocaine conditioned place preference (CPP). Within the dorsal raphe nucleus (DRN), two overlapping populations of KOR-expressing neurons: Slc17a8 (VGluT3) and SERT, were distinguished functionally and anatomically. Optogenetic inhibition of these SERT+ neurons potentiated subsequent cocaine CPP, whereas optical inhibition of the VGluT3+ neurons blocked subsequent cocaine CPP. SERT+/VGluT3- expressing neurons were concentrated in the lateral aspect of the DRN. SERT projections from the DRN were observed in the medial nucleus accumbens (mNAc), but VGluT3 projections were not. Optical inhibition of SERT+ neurons produced place aversion, whereas optical stimulation of SERT+ terminals in the mNAc attenuated stress-induced increases in forced swim immobility and subsequent cocaine CPP. KOR neurons projecting to mNAc were confined to the lateral aspect of the DRN, and the principal source of dynorphinergic (Pdyn) afferents in the mNAc was from local neurons. Excision of Pdyn from the mNAc blocked stress-potentiation of cocaine CPP. Prior studies suggested that stress-induced dynorphin release within the mNAc activates KOR to potentiate cocaine preference by a reduction in 5-HT tone. Consistent with this hypothesis, a transient pharmacological blockade of mNAc 5-HT1B receptors potentiated subsequent cocaine CPP. 5-HT1B is known to be expressed on 5-HT terminals in NAc, and 5-HT1B transcript was also detected in Pdyn+, Adora2a+ and ChAT+ (markers for direct pathway, indirect pathway, and cholinergic interneurons, respectively). Following stress exposure, 5-HT1B transcript was selectively elevated in Pdyn+ cells of the mNAc. These findings suggest that Dyn/KOR regulates serotonin activation of 5HT1B receptors within the mNAc and dynamically controls stress response, affect, and drug reward.
