Despite considerable rhetoric, comprehensive primary health care remains largely a matter of a paper exercise. The theory promotes horizontal and vertical integration and the active participation of people in planning. Experience in Australia and elsewhere indicates that what is in place in health services is often primary medical care: the management of the needs of presenting individuals. The arguments for upstream interventions remain valid, bolstered by research on the social determinants of health. Two examples are given of primary health care that attempt to work upstream, before clinical interventions become necessary and illustrate the need for both horizontal and vertical integration. Consequences for policy and training are drawn.

This study was designed to test the hypothesis that health care professionals would differ in their evaluations of a patient contemplating suicide, in their acceptance of suicide for the patient, and in their projected behavioral response to the patient, as a function of their belief that the patient has cancer, a terminal illness, or terminal cancer, and as a function of the degree of their death anxiety. Health care professionals ( N = 138) across a variety of disciplines completed Templer's Death Anxiety Scale, read one of four randomly distributed written introductions, then viewed taped segments of counseling sessions with a woman who was contemplating suicide. Results demonstrated that health care professionals' evaluations, acceptance, and behavior in the case of a patient contemplating suicide are affected by their belief that the patient has cancer and/or is dying, and by the degree of death anxiety experienced by the practitioner. The importance of awareness regarding personal feelings about suicide among terminal cancer patients is discussed.

Abstract The primary health care in the Spanish National Health System is organised in health centres with multi-professional teams, composed of doctors and nurses specialised in family and community health, in addition to other professionals. This article analyses the role of primary health care nurses in the Spanish National Health System. In the last decade, new concepts of task sharing between doctors and nurses as well as advanced nursing roles have been evolved in the health centres that focus on improving care for chronically ill patients and access to primary care. With shared responsibility, nurses are responsible for chronic patients in stable conditions, health prevention and promotion. The scaling up of advanced nursing tasks is limited by uncertainties of roles, disparities between states, and legislations that do not cover the full extent of advanced nursing tasks. The case study of Spain indicates that a strong multi-professional model of primary health care teams is a crucial basis for the evolvement of advanced nursing practice and its acceptance in daily routines. However, advantageous education structures and legislations are needed to allow nurses to develop their contribution in the full potential.

ABSTRACT Objective: to analyze the perceptions of professors, students and service professionals of primary health care, regarding their experiences in the Reorientation Program for Professional Training in Health in the Nursing Courses. Method: this is a qualitative research, in the form of a case study. The scenarios consisted of five nursing courses in higher education institutions, in the state of Santa Catarina. Twenty-two teachers and fourteen Primary Health Care professionals were interviewed. In order to understand the students' perception five focus groups were developed. Results: are presented from three general categories called: Pró-Saúde: evaluations about the experiences, Pró-Saúde - considerations about the advances and Pró-Saúde - considerations about the challenges. Conclusion: there are positive advances motivated by the Program throughout a contextual movement by an articulated, reflexive and critical training to the reality of health. However, there are challenges to reduce the gaps between services and training objectives.

Objetivo: Conhecer as percepções de enfermeiros sobre o cuidado a pacientes oncológicos, na Atenção Primária à Saúde. Metodologia: Pesquisa descritiva, com abordagem qualitativa, realizada entre março e abril de 2019, em dez Unidades de Atenção Primária à Saúde, no município de Aracati, Ceará, Brasil. Os sujeitos da pesquisa foram 10 enfermeiros, sendo realizada entrevista semiestruturada registrada em formulário. Resultados: A partir das falas, emergiram três categorias: Percepções de enfermeiros sobre câncer e cuidados a pacientes oncológicos; Ações realizadas na atenção primária no cuidado a pacientes oncológicos; Dificuldades na assistência oncológica, na atenção primária. Conclusão: Os enfermeiros participantes visualizaram o câncer como doença estigmatizante, que interfere na qualidade de vida de pacientes e familiares, sendo importante a assistência oncológica no âmbito da atenção primária, principalmente, com auxílio de equipe multiprofissional.Descritores: Atenção Primária à saúde; Oncologia; Cuidados de Enfermagem. ABSTRACTObjective: To know the perceptions of nurses about the care of cancer patients in Primary Health Care.
