AbstractBackgroundThe effect of alcohol consumption on cognitive decline is not clear. We aimed to study the association between alcohol consumption and cognitive functioning controlling for functional heath status.MethodsA total of 1610 older adults with a score ≥26 on the Mini-Mental State Examination (MMSE) were followed to assess the change in scores at the 3-year follow-up. Information on alcohol consumption as well as socio-demographic, lifestyle, psychosocial and clinical factors, as well as health service use were assessed at baseline and 3-year follow-up interviews. Linear mixed models with repeated measures were used stratifying by functional status.ResultsClose to 73% reported consuming alcohol in the past 6 months, of which 11% were heavy drinkers (≥11 and ≥16 drinks for women and men). A significant decrease in MMSE scores was observed in low functioning non-drinkers (−1.48; 95% CI: −2.06, −0.89) and light to moderate drinkers (−0.99; 95% CI: −1.54, −0.44) and high functioning non-drinkers (−0.51; 95% CI: −0.91, −0.10).ConclusionsAlcohol consumption did not contribute to cognitive decline. Cognitive decline was greater in individuals reporting low functional status. Research should focus on the interaction between changing patterns of alcohol consumption and social participation in individuals with low and high functioning status.

Matrix-induced autologous chondrocyte implantation (MACI) is a viable procedure that can be used as both a primary or revision cartilage regenerative procedure in high-functioning individuals without tibiotalar arthritis. Both short-term and midterm follow-up results demonstrate clinical, radiographic, and functional improvements with high rates of return to full activities. Cost remains a chief concern with the use of this technique, but theoretical improvements in the durability of repair with type II cartilage replacement may offer long-term benefits. Level of Evidence: Level V, expert opinion.

Anak dengan autism spectrum disorder (ASD) memiliki karakteristik utama yaitu perilaku repetitif dan minat yang terbatas, serta defisit dalam kemampuan berkomunikasi dan berinteraksi sosial sehari-hari. Anak dengan ASD memiliki risiko lebih besar untuk mengembangkan perilaku bermasalah, seperti perilaku tidak patuh yang berdampak buruk pada keberfungsian anak sehari-hari baik dalam aspek akademis maupun dalam lingkungan sosial. Perilaku tidak patuh yang berlebihan dapat dikurangi melalui program modifikasi perilaku. Tujuan penelitian ini adalah mengukur efektivitas program modifikasi perilaku untuk meningkatkan kepatuhan pada anak dengan High Functioning Autism Spectrum Disorder (HF-ASD) melalui single case A-B with follow-up design. Partisipan dalam penelitian ini adalah seorang anak laki-laki dengan kondisi HF-ASD yang berusia 8 tahun dan menunjukkan perilaku tidak patuh. Intervensi dilakukan sebanyak 19 sesi dengan menerapkan prinsip errorless compliance training seperti behavioral momentum, errorless learning, penyampaian instruksi yang efektif, dan positive reinforcement. Hasil menunjukkan bahwa program modifikasi perilaku efektif meningkatkan perilaku kepatuhan pada anak dengan HF-ASD dari 8% menjadi 81% serta terdapat efek generalisasi pada instruksi yang tidak dilatih selama intervensi.  Kata kunci: modifikasi perilaku; kepatuhan; high functioning autism spectrum disorder

Abstract. Background: There have been very few prospective studies of death by suicide in the general population. Rather, studies of suicide have generally used psychological autopsies, a method that has the potential weakness of recall bias. Aims: To examine correlates of death by suicide among a community-based nonclinical sample prospectively assessed years before death by suicide. Method: We analyzed data from an epidemiological study of a 10-year birth cohort (n = 4,914) conducted in Israel in the 1980s, with follow-up mortality data over 25 years. Results: Eight participants died by suicide during follow-up (6/100,000 per year; mean follow-up to suicide = 18.3 ± 2.0 years), the majority of whom were rated as functioning relatively well at baseline. Male sex, psychiatric hospitalizations, major depressive disorder, and previous suicide attempts were associated with later suicide. Conclusion: In this nonclinical sample of persons assessed between ages 25 and 34, several correlates of suicide were identified, but the majority of persons who died by suicide were relatively high functioning at baseline. Major precursors of suicide may be more proximal factors of acute or chronic negative changes in life circumstances.
