AbstractTonsillectomy is one of the commonest ENT procedures done in paediatric population, the technique of which has evolved over years to decrease the morbidity associated with the surgery. This prospective randomized comparative study is done to evaluate the efficacy of two different techniques of this surgery, conventional cold dissection and laser tonsillectomy based on operative time, blood loss, post-operative pain and occurrence of secondary complications. The study was done in 68 patients of paediatric age group, 34 in each group underwent cold dissection and laser tonsillectomy. Operative time and bleeding were significantly low for laser group. Pain score was comparable in early post-operatives days, but was high towards the end of first week. Our study reported only one incidence of complication in the form of a secondary bleeding.

Ovarian torsion is a true emergency which warrants early diagnosis and timely surgical management to avoid the catastrophic consequences of further adnexal injury. In paediatric population, this is especially dangerous as the condition can go undiagnosed because of its rarity and nonspecific presentation of disease. This leads to delay in surgical exploration and loss of ovarian function. We encountered 6 cases of ovarian torsion in paediatric age group during a period of 2 years, at a tertiary care hospital in Ahmedabad. After enquiry of the symptoms and a series of investigations, a provisional diagnosis of torsion ovary was made and they were taken up for surgery. Intra-operatively all the patients were found to have non-salvageable ovary and fallopian tube on the affected side, and subsequently they underwent salpingo-oophorectomy. Diagnosis of ovarian torsion requires clinician awareness and a high degree of suspicion. Conservative surgery, in the form of ovarian detorsion can be tried in cases of ischemia but if ovarian necrosis has occurred, then salpingo-oophorectomy is performed as the last resort.

SAH in the paediatric population is an uncommon entity accounting for 1-2% of SAH in all age groups & only 18% of ICH in the paediatric population. Trauma is the most common cause of isolated SAH in the paeditric population, with ruptured intracerebral aneurysms although still rare, being the most common cause of spontaneous SAH in children. AVMs are uncommon source of spontaneous SAH. Although they are 10 times more common than cerebral aneurysms in the paediatric population, they rarely present with isolated SAH. This report aims to present such a rare case presented to a tertiary care hospital in Trinidad.

Background: Cystic swellings of the scrotum are common clinical entities that surgeons encounter in daily practice. Males of all ages are affected. Today, considerable heterogeneity exists in their treatment. This study sought to identify the prevalence and patterns of presentation of these swellings in adults and evaluate the different modalities of surgical management.Methods: A cross-sectional study was undertaken from January 2013 to December 2016. A total of 186 patients fulfilling the inclusion criteria were included. Data was collected using a proforma. All patients were managed surgically.Results: Majority of cases (28%) were observed between 41-50 years (mean age 43.9 years). In 71%, mode of presentation was a painless, gradually progressive swelling. Mean duration of symptoms was 17.3 months. Primary vaginal hydrocele was the commonest swelling encountered (in 73.1%). Jaboulay’s procedure was undertaken in 75.2% of patients. Lord’s plication was associated with a comparatively shorter operative time, lesser postoperative scrotal edema and a shorter hospital stay. Epididymal cysts accounted for 19.4% and were managed by excision. The most common complication observed irrespective of surgery performed was scrotal edema (in 36%). Hematoma and infection were less frequently observed. Patients were followed-up for a mean period of 1.8 months. No recurrences were detected during this period.Conclusions: Cystic swellings of the scrotum are conditions predominantly affecting middle-aged males. Among these, primary hydrocele is the commonest. Although Lord's plication is simpler and gives good results with fewer complications, Jaboulay’s procedure remains the gold standard for surgical management of hydrocele.

Background: Children below 14 years of age are vulnerable to many skin diseases mainly due to malnutrition, poor hygiene, less medical care provision, low socio-economic status, etc. The incidence and the spectrum of paediatric dermatological diseases vary from one part of the world to another depending on various factors. This study was aimed to study skin problems in children below 14 years age. Material and Methods: This was a cross-sectional, observational study done with children of age below 14 years attended skin outpatient department (OPD) with skin problems. Results: During study period 310 patients were included in the study of which 178 (57.26%) were male and 132 (42.74%) were female. The ratio of male: female was 1.34:1 in our study. Infectious skin lesions were seen in 163 (52.5%) children, while non-infectious skin lesions were seen in 157 (50.6 %) children. Conclusion: Infections and infestations are commonly noted in our paediatric population. Application of preventable measures by community, early diagnosis and proper counseling by pediatrician or dermatologist is needed to prevent further morbidity of skin lesions.
