<p class="abstract"><strong>Background:</strong> Allergic rhinitis (AR) is a common disorder seen in routine otolaryngology practice. There has been an increased interest in researchers worldwide regarding role of vitamin D3 in pathogenesis of allergy. This study intends to compare levels of vitamin D3 in patients with clinically diagnosed allergic rhinitis and subjects without allergic rhinitis. This study was conducted in rural South Karnataka where incidence of allergic rhinitis is on the rise. Main objective of this study was to assess vitamin D3 levels in patients with clinically diagnosed AR and compare it with vitamin D3 levels in control group where normal subjects were included.</p><p class="abstract"><strong>Methods:</strong> Total of 100 patients were included in this study. Case group comprised of 50 patients clinically diagnosed as AR and control group comprised 50 subjects without any symptoms of AR. All the patients underwent serum vitamin D3 estimation. Other parameters such as age, gender, occupation (outdoor vs indoor) and region of residence (urban vs rural) were also compared between the groups.  </p><p class="abstract"><strong>Results:</strong> This study did not find significant difference between two groups when all the parameters compared. Significant number of study subjects both in case and control group were found to be either deficient or inadequate in vitamin D3 levels.</p><strong>Conclusions:</strong> we did not find significant difference in vitamin D3 levels between cases and controls. Further studies with bigger sample size and robust study design may throw more light on association of vitamin D3 with AR.

Background: Depression is a major public health problem and occurs in persons of all ages, and is associated with increased morbidity, soaring costs for treatment and reduced productivity and quality of life. Vitamin D is involved in numerous brain processes including neuroimmunomodulation, neuroprotection, neuroplasticity, regulation of neurotrophic factors, and making it biologically plausible to be associated with depression. Aim of the present study is to compare the therapeutic effects of vitamin D given along with escitalopram versus escitalopram given alone in patients with major depressive disorder.  Methods: In this prospective, randomized, interventional clinical study, 60 patients with a diagnosis of major depressive disorder based on ICD-10 criteria were randomly assigned into two groups, one group received 60000IU vitamin D3 weekly plus 10 mg escitalopram OD daily while the other group received escitalopram10 mg OD daily alone for 8 weeks. Depression severity was assessed at 2-week intervals using the 24-item Hamilton Depression Rating Scale (HDRS). Serum 25(OH) vitamin D levels were measured in all the patients at baseline and after intervention.Result: Fifty nine patients completed the trial. Depression severity based on HDRS decreased significantly after intervention, with a significant difference between the two groups. The vitamin D3+escitalopram combination was significantly better than escitalopram alone from the fourth week of treatment. Conclusion: Role of vitamin D in mood disorder and its dietary supplementation is effective as an adjuvant treatment along with SSRIs in depressive disorders, especially in vitamin D deficient patients.

The present study was aimed to study the effect of Sublingual Vitamin D3 on Serum Vitamin D level in Vitamin D deficiency patients. This was a cross-sectional and interventional study. All the Vitamin D deficiency patients of age 18-60years and either gender, willing to participate in the study were included. Patients who had greater than 20 ng/ml were excluded from the study. The total number of participants in our study was 200, out of these 111 males and 89 females, the mean age in our study was 51.07 ± 7.39Yrs. All volunteers were given sublingual vitamin D3 (60,000IU) in six doses every fifteen days of follow up for 3 months. The subject’s serum 25(OH)D levels were estimated before and after the treatment of sublingual vitamin D3. There was a statistically significant difference in serum vitamin D3 level before 16.61±6.71 ng/ml and after 35.80±7.80 ng/ml after treatment with Sublingual Vitamin D3. Six doses of 60,000IU of Vitamin D3 sublingual route having improved the role of serum 25(OH)D levels in the treatment of Vitamin D3 deficiency patients.Keywords: Vitamin D3; Sublingual route

Ayurveda believes in different pathology which sets in different Srotas to produce different diseases. Whenever a favorable condition and situation arise, diseases will manifest. Nanatmaja Vikara, Anubandha, Gatavata and Avarana are different pathologies explained in Ayurveda. Avarana is one of the most difficult concept to understand teach and incorporate in clinical practice. It is either least observed, diagnosed or goes unidentified due to lack of skill. To understand and analyze the Avarana, Meticulous knowledge of basic concept of Ayurveda is essential. It is assumed that symptoms complex in Avarana leads to misunderstanding, wrong diagnosis and sometimes end with complications. The symptoms produced in Avarana are based on the principles of Rupahani (Karmakshaya), Rupavriddi (Karmavriddhi) and Rupantara (Anya Karma). The present article is an attempt made to have a critical clinical study of role of Avarana in the manifestation of Rupavriddhi, Rupahani, Rupantara.

Protein dimerization facilitates cooperative, high-affinity interactions with DNA. Nuclear hormone receptors, for example, bind either as homodimers or as heterodimers with retinoid X receptors (RXR) to half-site repeats that are stabilized by protein-protein interactions mediated by residues within both the DNA- and ligand-binding domains. In vivo, ligand binding among the subfamily of steroid receptors unmasks the nuclear localization and DNA-binding domains from a complex with auxiliary factors such as the heat shock proteins. However, the role of ligand is less clear among nuclear receptors, since they are constitutively localized to the nucleus and are presumably associated with DNA in the absence of ligand. In this study, we have begun to explore the role of the ligand in vitamin D3 receptor (VDR) function by examining its effect on receptor homodimer and heterodimer formation. Our results demonstrate that VDR is a monomer in solution; VDR binding to a specific DNA element leads to the formation of a homodimeric complex through a monomeric intermediate. We find that 1,25-dihydroxyvitamin D3, the ligand for VDR, decreases the amount of the DNA-bound VDR homodimer complex. It does so by significantly decreasing the rate of conversion of DNA-bound monomer to homodimer and at the same time enhancing the dissociation of the dimeric complex. This effectively stabilizes the bound monomeric species, which in turn serves to favor the formation of a VDR-RXR heterodimer. The ligand for RXR, 9-cis retinoic acid, has the opposite effect of destabilizing the heterodimeric-DNA complex. These results may explain how a nuclear receptor can bind DNA constitutively but still act to regulate transcription in a fully hormone-dependent manner.
