Abstract Background The effects of group exercise on the physical function of community-dwelling older adults remain unclear. The changes in lower extremity muscle strength, timed up and go (TUG) time, and the motor fitness scale (MFS), over time, among older adults who expressed a willingness to participate in community-based physical exercise groups, were determined using multilevel modelling. Methods We analyzed data of 2407 older adults between April 2010 and December 2019 from the registry of physical tests of community-based physical exercise groups. We conducted a retrospective cohort study to assess the effect of physical exercise on lower extremity muscle strength, TUG time, and MFS scores. The durations of the exercises were evaluated by frequency of physical test’s participate. Results A deterioration in lower extremity muscle strength was found in the short-term participant group only. However, in the mid-term and long-term participation groups, lower extremity muscle strength showed a trend of improvement. The TUG time and the MFS score were negatively correlated with increasing age in both groups divided by the duration of participation. However, there was a slower rate of deterioration in the long-term participation group. Discussion Lower extremity muscle strength, TUG time, and MFS scores decline with increasing age and there were differences in the slope of deterioration that depended on the duration of participation in community-based group exercise. Conclusion Participation in group exercise improved lower extremity muscle strength, TUG time, and MFS scores of older adults living in a community. The positive effects of group exercise were dependent on long-term participation.

Abstract Background Resistance training with pneumatic machines attenuates the age-associated loss in muscle strength and function in older adults. However, effectiveness of scaled-up pneumatic machine resistance training in the community is not known. We evaluated the effectiveness of a multi-site community-delivered 12-week pneumatic machine resistance programme (Gym Tonic (GT)) on muscle strength and physical function in older adults. Methods Three hundred eighteen community-dwelling older adults aged ≥65 years were randomized into 12-week (twice/week) coach-supervised-community-based-GT-programme(n = 168) and wait-list control groups(n = 150). After 12 weeks, the intervention group continued with GT-training and the control group received supervised-GT-programme for further 12 weeks (partial-crossover-design). Fried frailty score, lower-extremity muscle strength and physical function (i.e., fast and habitual gait-speed, balance, repeated-chair-sit-to-stand, short physical performance battery (SPPB)) were determined at baseline, 12 and 24 weeks. Analysis adopted a modified-intention-to-treat-approach. Results After 12 weeks, lower-extremity muscle strength improved by 11–26%(all p < 0.05) and fast gait-speed improved by 7%(p = 0.008) in GT-intervention group(n = 132) than controls(n = 118), regardless of frailty status. Other physical function performance did not differ between control and intervention groups after 12 weeks (all p > 0.05). Frailty score improved by 0.5 in the intervention but not control group(p = 0.004). Within the intervention group, lower-extremity muscle strength and physical function outcomes improved at 24 weeks compared with baseline (all p < 0.001). Within controls, lower-extremity muscle strength, SPPB, repeated-chair-sit-to-stand and fast gait-speed improved post-GT (24-week) compared to both pre-GT (12-week) and baseline. Programme adherence was high in intervention [0–12-weeks,90%(SD,13%); 12–24-weeks,89%(SD,17%)] and control [12–24-weeks,90%(SD,19%)] groups. Conclusion Community-delivered GT resistance training programme with pneumatic machines has high adherence, improves muscle strength and fast gait-speed, and can be effectively implemented at scale for older adults. Future studies could examine if including other multi-modal function-specific training to complement GT can achieve better physical/functional performance in power, balance and endurance tasks. Abstract Background The risk of adverse events associated with peritoneal dialysis (PD) in elderly patients has not been thoroughly investigated. The purpose of this study was to assess the association between physical function and hospitalization in elderly PD patients. Methods This is a single-center prospective observational cohort study. Thirty-three aged patients (74.8 ± 5.9 years) participated in a 6-min walk distance, short physical performance battery (SPPB), lower extremity muscle strength, and 10-m walk speed. All subjects were followed until hospitalization to the end of the follow-up period. Results The 6-min walk distance was 332 ± 112.5 m; SPPB was 11 (8.3–12) points; the lower extremity muscle strength was 36.6 ± 9.6%; 10-m walk speed was 1.1± 0.2 m/s. During the follow-up, 19 patients (57.5%) were hospitalized. In the Kaplan-Meier survival analysis and log-rank test, a lower 6-min walk distance and PD vintage were significantly associated with hospitalization (p<0.05). After adjustment for PD vintage in Cox proportional analysis, the 6-min walk distance remained associated with hospitalization (95% confidence interval, 0.98–0.99). Conclusion Lower exercise tolerance assessed by the 6-min walk distance was significantly associated with hospitalization in elderly PD patients. Our findings indicate that measurement and intervention of exercise intolerance are essential to predict the clinical outcomes of elderly PD patients. Do Single-Leg Balance Control and Lower Extremity Muscle Strength Correlate with Ankle Instability and Leg Injuries in Young Ballet Dancers?

Ankle injuries are common among young ballet dancers. These injuries may be attributed to ankle instability, insufficient lower extremity strength, and poor balance control. The purpose of this study was to explore whether these dancers exhibit functional ankle instability and if their single-leg balance control and lower extremity muscle strength correlate with functional ankle instability and leg injuries. Twenty-one ballet dancers (aged 10 to 17 years) participated in the study. The Cumberland Ankle Instability Tool (CAIT) questionnaire was used to examine functional ankle stability. Isometric muscle strength of the major lower extremity muscles was measured with a digital hand-held dynamometer. Single-leg balance was evaluated with the Y-Balance Test (YBT) and three Athletic Single-leg Stability Test (ASLST) protocols. Lower extremity injuries (self-reported) within 6 months after testing were recorded for correlation analyses. Both dominant and non-dominant ankles of the subjects exhibited functional ankle instability (26.71 and 25.71, respectively). Raising the center of mass (passé and first arm position) during the ASLST did not significantly affect balance performance (p = 0.104). However, removing extrinsic visual feedback significantly decreased single-leg balance (p < 0.001). In general, there was low correlation (r ≤ 0.49) between muscle strength, CAIT, YBT, and ASLST scores with lower extremity injuries. It is concluded that for young ballet dancers lower extremity muscle strength and single-leg balance control may not be strong contributing factors to leg injuries. This study also suggests that functional ankle stability may not have a direct impact on single-leg balance, and ballet dancers rely heavily on extrinsic visual feedback for single-leg balance control. Teachers might consider minimizing extrinsic feedback to challenge ballet dancers when implementing training protocols for single-leg balance control.

Objectives: to know the standard value of lower extremity muscle strength of eight year old children and furthermore to explore the correlation of the muscle strength and body height and weight.Methods: The study design is cross sectional. The target is eight year old children in public elementary school in Jakarta Pusat. The subjects’ characteristics are normal nutritional state, and no neurological normusculoskeletal disorders. Sampling was done by cluster randomization to determine the location and simple randomization on site to determine subjects. There were 171 boys and 180 girls in this tudy. Independentvariables are age, sex, body weight, body height and nutritional state that was determine with Z-score of body mass index. Dependent variables are lower extremity muscle strength that classify as torque. This study usedhand-held dynamometer for muscle strength measurement. Statistical analysis was done with descriptive statistic and Pearson and Spearman correlation test.Results: Standard values of eight year old boy’s lower extremity muscle strength are: right hip flexor 21.86 Nm (SD 3.40), left hip flexor 19.64 Nm (SD 3.19), right hip extensor 17.05 Nm (SD 3.66), left hip extensor16.08 Nm (SD 3.56), right knee extensor 18.19 Nm (SD 3.60), left knee extensor 16.09 Nm (SD 3.55), right knee flexor 15.18 Nm (SD 4.23), left knee flexor 14.48 Nm (SD 3.97), right ankle dorsiflexor 6.58 Nm (SD1.53), left ankle dorsiflexor 6.05 Nm (SD 1.42), right ankle plantarflexor 10.08 Nm (SD 1.69), left ankle plantar flexor 9.13 Nm (SD 1.90).Standard values of eight year old girl’s lower extremity muscle strength are: right hip flexor 21.60 Nm (SD 3.62), left hip flexor 19.62 Nm (SD 3.37), right hip extensor 16.66 Nm (SD 4.06), left hip extensor 15.81 Nm(SD 3.94), right knee extensor 17.43 Nm (SD 3.79), left knee extensor 15.20 Nm (SD 3.38), right knee flexor 14.61 Nm (SD 4.28), left knee flexor 13.51 Nm (SD 4.00), right ankle dorsiflexor 6.34 Nm (SD 1.45), leftankle dorsiflexor 5.97 Nm (SD 1.52), right ankle plantarflexor 9.55 Nm (SD 1.98), left ankle plantar flexor 8.69 Nm (SD 1.83). The boy’s lower extremity muscle strength are stronger than the girl’s in left knee extensor,left knee flexor, right ankle plantarflexor and left ankle plantarflexor. The boy’s muscle strength are moderately correlated to body height except for right hip extensor, left hip extensor and right ankle dorsiflexorthat weakly correlated. The boy’s muscle strength are moderately correlated to body weight except for left hip extensor that weakly correlated. The girl’s muscle strength are moderately correlated to body height. Thegirl’s muscle strength are moderately correlated to body weight except for left hip flexor and left hip extensor that weakly correlated.Conclusions: The muscle strength pattern of boys and girls is similar; the strongest are right hip flexor and the weakest are left ankle dorsiflexor.Keywords: Muscle strength, standard values of eight year old children, torque, hand-held dynamometer
