Position of the mandibular canal before and after bilateral sagittal split ramus osteotomy: a cone beam computed tomography study

Objectives. To investigate the availability of correction of mandibular plane using software for vertical measurements in cone beam computed tomography (CBCT) according to the sites of the mandible.Methods. CBCT scans of six dry mandibles were performed at 0-, 5-, 10-, 15-, and 20-degree angles relative to CBCT scanning table. Using the imaging software, mandibular planes of the different angles were corrected to that of 0-degree angle on the CBCT images. Before and after correction of the mandibular planes, the distance from the mandibular canal to the alveolar crest was measured at M1, M2, and M3 areas of the mandible and vertical measurements were statistically compared with those of 0-angle location using the pairedt-test.Results. Prior to correction, the vertical measurements increased as the angle increased. The greatest differences of measurements were observed in M3 areasP<0.05. After correction, a strong correlation was found in measurements between the 0-degree angle and the other angles in all sites of the mandibleP>0.05.Conclusions. The vertical measurements of CBCT were significantly influenced by mandibular positioning. When CBCT scans are performed at angles other than 0-degree angle, software-based correction of the mandibular plane can be a reliable tool for the accurate vertical measurements in CBCT.

ABSTRACT Objective: To compare the condylar displacement following sagittal split ramus osteotomy (SSRO) in asymmetric setback patients between the conventional approach and surgery-first approach and to determine whether the condylar displacement is affected by asymmetric setback in SSRO patients. Materials and Methods: This was a retrospective study. The subjects consisted of patients with facial asymmetry who underwent SSRO and had cone-beam computed tomography taken before and 1 month after surgery. They were allocated into the conventional (n = 18) and surgery-first (SF) groups (n = 20). Descriptive, independent t-tests and Pearson correlation analysis were computed. Results: The amount of condylar displacement in x-, y-, and z-directions and Euclidean distance showed no statistically significant differences between the conventional and SF groups. Comparing the postoperative condylar position with the preoperative position, the condylar displacement occurred in posterior (P &lt; .05) and downward (P &lt; .05) directions in both groups except on the deviated side in the conventional group. The condylar displacement occurred in a posterior (P &lt; .05) direction on the deviated side of the conventional group. However, the condylar displacement in three dimensions showed no statistically significant differences between the two groups. In the correlation analysis, the condylar displacement in both the deviated and contralateral sides showed no significant correlation with asymmetric setback in either group. Conclusion: The condylar displacement in three dimensions and the distance of condylar displacement in SSRO patients with facial asymmetry showed no significant difference between conventional and SF groups. Condylar displacement was not associated with asymmetric setback.

ABSTRACT Objectives Exact location of the inferior alveolar nerve (IAN) bundle is very important. The aim of this study is to evaluate the relationship between the mandibular third molar and the mandibular canal by cone-beam computed tomography. Study design This was a cross-sectional study with convenience sampling. 94 mandibular CBCTs performed with CSANEX 3D machine (Soredex, Finland) and 3D system chosen. Vertical and horizontal relationship between the mandibular canal and the third molar depicted by 3D, panoramic reformat view of CBCT and cross-sectional view. Cross-sectional view was our gold standard and other view evaluated by it. Results There were significant differences between the vertical and horizontal relation of nerve and tooth in all views (p < 0.001). Conclusion The results showed differences in the position of the inferior alveolar nerve with different views of CBCT, so CBCT images are not quite reliable and have possibility of error. During the course of this experimental study tomograms of extracted teeth were analyzed before and after filling the root canals with an endodontic material and fragments of broken metal instruments for root canal treatment. During the first stage of the experiment, untreated extracted teeth were scanned using conebeam computed tomography and microfocus cone-beam computed tomography. A comparative assessment of capabilities of the two methods of cone-beam computed tomography based on examination of untreated root canals was carried out. The second part of the study is dedicated to visualization of root canals that contain foreign high-density materials.

This study aimed to compare the canal transportation and canal centering ability in the preparation of curved root canals after instrumentation with TruNatomy (TN) (TN; Dentsply Sirona, Maillefer, Ballaigues, Switzerland) and ProTaper Gold (PG) (PG; Dentsply, Tulsa Dental Specialties, Tulsa, OK, USA) files using cone‑beam computed tomography (CBCT). 30 Single rooted extracted human teeth with root curvature ranging from 20-30° according to Schneider’s method were selected. Teeth with any visible cracks or fractures, calcifications, previous root canal treatments were excluded. The teeth were randomly assigned into two groups i.e. Group 1-TN and Group 2-PG (n = 15 each). The teeth were instrumented according to manufacturer’s guidelines for both the groups. Canals were scanned using a CBCT scanner before and after preparation to evaluate the transportation and centering ratio at 3 mm, 5 mm, and 7 mm from the apex. The data analysis was done using SPSS software and the test used was independent sample t test for comparison between the 2 groups.Data obtained suggested that TN group presented lesser canal transportation at the middle third of the root. The PG group showed better centering abitily at apical third of the root canal when both the groups were compared. TN resulted in less transportation than PG at the middle third, and PG showed better centering ability at the apical third. Overall, both systems safely prepared root canals, causing minimal errors.

The filling material should be restricted to the root canal, and not extend to the periradicular tissues. Overextension occurs when there is an overflow of gutta-percha and sealer, whereas overfilling refers to the overflow only of sealer beyond the apical foramen. Both may cause several negative clinical consequences. Nevertheless, an accurate diagnosis of where they occurred cannot always be performed by conventional radiographic examination, because of the two-dimensional aspect of the image. This paper describes a clinical case of labiomandibular paraesthesia after overfilling into the mandibular canal (MC), as diagnosed by cone-beam computed tomography (CBCT), later used to perform the treatment planning. A 34-year-old Caucasian female patient sought a private dental clinic complaining of pain in the right mandibular posterior region. After taking the anamnesis and performing clinical and radiographic exams, the patient was diagnosed with pulp necrosis in the second right mandibular molar, and underwent root canal treatment. The final radiography showed overextension or overfilling, probably into the MC. About 2 hours after the procedure, the patient reported paraesthesia of her lower right lip and chin. A CBCT confirmed a small overfilling into the MC. For this reason, vitamin B12 was prescribed as the first treatment option. After 7 days, the patient reported a significant decrease in paraesthesia, and was completely normal after 15 days. This case report shows that CBCT is an effective radiographic diagnostic tool that can be used as an alternative in clinical cases of labiomandibular paraesthesia caused by overextension or overfilling.   Keywords Endodontic treatment; Overfilling; Paraesthesia; Conebeam computed tomography.
