Nanohybrid vs. fine hybrid composite in Class II cavities: Clinical results and margin analysis after four years

ABSTRACTObjectives: The purpose of this study was to evaluate and compare the 12 month clinical performances of two different posterior composites in Class I and Class II restorations.Methods: Thirty-one patients (10 male, 21 female) were recruited into the study. A total of 82 Class I and Class II cavities were restored with either a nanohybrid composite (Grandio) or a low-shrinkage composite (Quixfil), using their self etch adhesives (Futura Bond and Xeno III) according to manufacturers’ instructions. The restorations were clinically evaluated 1 week after placement as baseline, and after 6 and 12 months post-operatively using modified USPHS criteria by two previously calibrated operators. Statistical analysis were performed using Pearson Chi-square and Fisher’s Exact Test (P<.05).Results: All patients attended the 12-month recall. Lack of retention was not observed in any of the restorations. With respect to color match, marginal adaptation, secondary caries and surface texture, no significant differences were found between two restorative materials tested after 12 months (P>.05). None of the restorations had marginal discoloration and anatomic form loss on the 12 month follow-up. Restorations did not exhibit post-operative sensitivity at any evaluation period.Conclusions: Clinical assessment of nanohybrid (Grandio) and low-shrinkage posterior composite (Quixfil) exhibited good clinical results with predominating alpha scores after 12 months. However; further evaluations are necessary for the long-term clinical performance of these materials. (Eur J Dent 2010;4:57-65)

Background: Optimal management of hypodivergent growing patients demands a strict control of vertical dimension and to exploit the growth potential. If a deep bite malocclusion causes a traumatic contact between the upper and lower incisors and affects the facial appearance, an early interceptive treatment is recommended. The aim of this case report is to outline the clinical management of the occlusal plane of a growing Class II division 1 deep bite patient treated with aligners and Class II elastics. Methods: The treatment lasted 11 months and was divided into two phases. Treatment goals included improvement of the soft tissue profile and basal bone relationships through an increase in the mandibular third of the face and a sagittal advancement of the mandible. The correction of the curve of Spee involved intrusion of the mandibular incisors and extrusion of the mandibular premolars. Results: The cephalometric analysis at the end of the treatment displayed significant differences in the skeletal and occlusal pattern along with aesthetic improvements. Conclusion: The final cephalogram showed a consistency between the planned tooth movement and the clinical results. Although definitive recommendations must be withheld until longer follow-up is available, the patient presented here shows that the treatment protocol yielded positive mandibular growth.

Introduction: Evaluation of microleakage is important for assessing the success of new restorative materials and methods. The aim of this study was to assess the microleakage of class II restorations with different flowable composites as liners and two different polymerization techniques classic and soft start. Materials and Methods: 40 extracted human premolars teeth with class II cavity preparation medial and distal (80 cavities) were divided into four groups: 1.Vertise Flow (VF)+micro hybrid composite Herculite 2.Surefil SDR Flow (SDRF)+micro-hybrid composite Herculite 3.Tetric Flow (TF)+micro-hybrid composite Herculite 4.control group micro-hybrid composite Herculite. Mesial cavities are polymerized with classic and distal cavity with soft start technique of polymerization. After that, the samples were immersed in 0.5% AgNO3 solution and sectioned into the mesiodistal direction. Using a stereomicroscope (Nikon - Japan), with a magnification of 40x, the gingival microleakage of cavities was examined. Data were analyzed using Fisher's and Student's tests. Results: After using the classical polymerization technique, all three used flowable composites VF+Herculite, SDRF+Herculite, TF+Herculite showed less gingival microleakage than the control group. This difference was statistically significant. After the application of the soft-start technique of polymerization, VF+Herculite and SDRF+Hercules showed a statistically significant reduction in gingival microleakage, while TF+Herculite showed a comparable result with control group, without a statistically significant difference. There was no statistically significant difference between classical and soft start polymerization techniques. Conclusion: Flowable composites in this study have reduced gingival microleakage and can be used as liners in the restoration of II class cavities.

Abstract This prospective study assessed the dual-curing self-adhesive composite hybrid Surefil one. The restorations were placed and reviewed by dental practitioners who are members of a practice-based research network in the United States. Seven practitioners filled 60 cavities (20 class I, 19 class II and 21 class V) in 41 patients with Surefil one without adhesive, according to the manufacturer’s instructions. The restorations were evaluated using modified rating criteria at baseline and after 3 months and 1 year. Patients were also contacted to report postoperative hypersensitivity 1 to 4 weeks after placement. Recall rates were 98% after 3 months and 82% after 1 year. The only patient that showed moderate hypersensitvity after 1 year had previously reported symptoms that were unlikely associated to the class I molar restoration. One class II restoration in a broken maxillary molar was partially lost. The remaining 48 restorations were found to be in clinically acceptable condition resulting in an annual failure rate of 2%. The lowest number of acceptable scores (88%) was for color match. Self-adhesive composite hybrid restorations showed promising results in stress-bearing class I and II as well as non-retentive class V cavities at 1-year recall.

ABSTRACTObjectives: The objective of this in vitro study was to evaluate the marginal sealing ability of an ormocer in comparison with a hybrid composite when using thermocycling and load cycling procedures together in the study design.Methods: Modified proximal Class II cavities were prepared on the mesial and distal surfaces of twenty-two human premolars. Each tooth was restored with Admira and contralaterally with TPH Spectrum. Half of all of the teeth were subjected to thermo-mechanical load cycling. After dye penetration, longitudinal sections in mesio-distal direction were prepared and examined under a stereomicroscope. Data were analyzed with Mann Whitney U test.Results: The results showed that the thermo-mechanical load cycling caused a statistically significant increase in gingival microleakage when compared with the non-thermocycled and nonloaded restorations for both the materials Admira (P=0.006) and TPH Spectrum (P=0.023).Conclusions: Simultaneous load cycling and thermocycling are decisive factors in the in vitro assessment of gingival microleakage, which still remains to be a clinical problem even with the ormocer system Admira. OBJECTIVE: The aim of this case report was to evaluate the results of deep overbite correction by the segmented arch technique. METHODS: The first stage of the treatment was performed with this technique to intrude incisors and canines. A maxillary transpalatal bar and a mandibular lingual arch using 0.051-in archwire were employed associated to a facial bow, anchored primarily with occipital pull and, after molar relationship correction, changed to high pull. RESULTS: Upper incisors presented a great amount of intrusion and some palatal root inclination as programmed in the treatment plan. The skeletal mandibular response was favorable to achieve profile balance. Periapical radiograph showed rounded apex of incisor roots, expected in any orthodontic intrusion movement. CONCLUSION: The clinical results present the successful approach to deep overbite malocclusion.

Insulin-dependant (Type I) diabetes mellitus (IDDM) is a metabolic disorder resulting from the lack of endogenous insulin secretion. The disease is thought to result from the autoimmune mediated destruction of the insulin producing ß cells within the islets of Langerhans. The disease process is probably triggered by environmental agents, e.g. virus or chemical toxins on a background of genetic susceptibility associated with particular alleles within the major histocompatiblity complex (MHC). The relation between IDDM and the MHC locus has been reinforced by the demonstration of both class I and class II MHC proteins on the surface of ß cells from newly diagnosed patients as well as mounting evidence that IDDM has an autoimmune pathogenesis. In 1984, a series of observations were used to advance a hypothesis, in which it was suggested that aberrant expression of class II MHC molecules, perhaps induced by gamma-interferon (IFN γ) could present self antigens and initiate an autoimmune disease. We have tested some aspects of this model and demonstrated that expression of IFN γ by pancreatic ß cells can initiate an inflammatory destruction of both the islets and pancreas and does lead to IDDM.
