Self-inflicted injury, the most common form of intentional injury, disproportionately affects low-income countries, but is poorly described in this setting. This retrospective review of the 2008–2018 trauma registry at a referral hospital in Malawi included all victims of intentional injury ≥10 years. Self-inflicted injuries were compared to assaults. The primary outcome was in-hospital mortality. Common mechanisms of self-inflicted injuries were fall from height, poisoning, and penetrating injury. In-hospital mortality from self-inflicted injury was 8.8% vs. 1.9% for assault. Those who died from self-inflicted injury were more often older (median 34 vs. 26 years, p < 0.001), male (91.9% vs. 67.8%, p < 0.001), unemployed (32.8% vs. 6.4%, p < 0.001), and most commonly died by hanging (60%). The odds of in-hospital mortality after self-inflicted injury was four times assault (OR 4.0 [95% CI 1.4–11.5], p = 0.01). The trauma registry proved useful for describing self-inflicted injury in this setting.

Background: Fractures of distal end of femur are complex injuries which are difficult to manage. These fractures often are unstable and comminuted. Despite advances in surgical techniques and improvement in implants, treatment of distal femoral fractures remains a challenge in many situations. This study was done to analyse the functional outcome and this management of distal femoral fractures. Material & Methods: This study has been done between August 2008 to March 2010 on patients coming to Orthopaedics Department at Jawaharlal Nehru Hospital and Research Center, Bhilai. It is a prospective study which includes 25 patients with 10 Type A, 7 Type B and 8 Type C fractures of distal femur after fulfilling the inclusion and exclusion criteria. Results: Mean age of 47.08 years with sex incidence of 3.17:1(M:F). Mode of injury was RTA in 16, simple fall in 6, fall from height in 2 and railway track accident in 1 patient. There were 5-A1, 1-A2, 4-A3, 3-B1, 1-B2, 3-B3, 1-C1, 4-C2 and 3-C3 fractures. Results were found to be excellent in 17, good in 1, moderate in 3 and poor in 4 patients. Conclusion: We conclude that minimally invasive surgical techniques and with the availability of locking type of plates distal femoral fractures can now be dealt with more precessions and more satisfactory results can be produced. Shipyard industry is one of the growing industry in Malaysia. There is still a lot of room for improvement with regards to the safety aspects within the industry. This study presents the finding of a survey on the safety management in shipyard operation of class C and D in Peninsular Malaysia registered with Ministry of Finance Malaysia. Shipyard operation is considered a hazardous job. Most of the cases were due to fall from height, fire and explosion. Similar accidents happened in South Korea, where 8 fatalities and 31 injuries were reported in January to August 2017. The accidents were due to crane collapsed and explosion during welding works inside a hull of a Liquefied Petroleum Gas (LPG) carrier. This paper aims to study the Health, Safety and Environment Management System (HSEMS) of shipyard operations in Malaysia particularly in Class C and Class D shipyard. This study was conducted using a survey method following the Glenn D’s equation. The survey was targeted at the people who are involved in the industry including the staffs of clients, consultants and contractors. The findings of this study show that almost all the shipyards of Class C and D have HSEMS in placed but some are incomplete. Accidents still happen in the Malaysian shipyard industry from time to time. The gaps in the HSEMS that lead to the accident are highlighted in this paper. Nearly 10% of Class C and D shipyards do not have clear HSE Policy and this one of possible causes of accidents due to low priority given to HSE matters in this particular Class C and D shipyard.

ABSTRACT Objective: To describe the epidemiology of patients with thoracolumbar spine fracture submitted to surgery at Hospital Cristo Redentor and the related costs. Methods: Prospective epidemiological study between July 2014 and August 2015 of patients with thoracolumbar spine fracture with indication of surgery. The variables analyzed were sex, age, cost of hospitalization, fractured levels, levels of arthrodesis, surgical site infection, UTI or BCP, spinal cord injury, etiology, length of stay, procedure time, and visual analog scale (VAS) . Results: Thirty-two patients were evaluated in the study period, with a mean age of 38.68 years. Male-female ratio was 4:1 and the most frequent causes were fall from height (46.87%) and traffic accidents (46.87%). The thoracolumbar transition was the most affected (40.62%), with L1 vertebra involved in 23.8% of the time. Neurological deficit was present in 40.62% of patients. Hospital stay had a median of 14 days and patients with neurological deficit were hospitalized for a longer period (p<0.001), with an increase in hospital costs (p= 0.015). The average cost of hospitalization was U$2,874.80. The presence of BCP increased the cost of hospitalization, and patients with spinal cord injury had more BCP (p= 0.014) . Conclusion: Public policies with an emphasis on reducing traffic accidents and falls can help reduce the incidence of these injuries and studies focusing on hospital costs and rehabilitation need to be conducted in Brazil to determinate the burden of spinal trauma and spinal cord injury.

This prospective study was carried out during the period of January 2007 to December 2011 in Mymensingh Medical College Hospital and other private clinics of Mymensingh town to assess the results of Ilizarov technice for nonunion of the tibia. Here total operated patient were 30, of them 24 were male and 6 were female. 10 cases are hypertrophic, 8 cases are atrophic and 12 cases are infected nonunion. In this series age of the patients was 10 ? 60 years. Motor vehicle accident was the commonest cause of fracture found in 66.67% cases. Second most common cause of injury was fall from height. Among 30 patients proximal corticotomy was done in 24 patient and distal corticomy in 2 patient. Union time was 4-12 months. Among 30 patients 27 were united at initial procedure. From nonunited 3 cases again refreshening of the ends along with bone grafting was done and ultimately 2 were united and 1 remains nonunited. In this series limb length discrepancy reduced to maximum 1.5 cm with contralateral limbs. Average total hospital stay was 15 days. Pin site infection and pain were the commonest complications in our study. Close adherences to Ilizarov principles make it now possible to successfully treat a most of orthopaedic conditions that previously were fraught with high morbidity rates and poor results. Transverse sacral fracture is a rare entity in the pediatric age group, primarily resulting from a fall from height. This case report stresses the importance of proper neurological assessment, adequate imaging and highlights different modalities of management of this fracture. We report a case of a 13-year-old child with type III Roy-Camille sacral fracture with associated bladder and bowel dysfunction. The patient underwent spinopelvic fixation and decompression of the sacral canal. The patient partially regained bowel and urinary function and had improved motor function after three months of follow-up. Operative management is generally advocated for such fractures.
