Best Practice Injury Compensation Processes Following Intentional Vehicular Assaults and Other Large Scale Transport Incidents: A Delphi Review

Abstract Objective: Intentional vehicular assaults on civilians have become more frequent worldwide, with some resulting in mass casualties, injuries, and traumatized witnesses. Health care costs associated with these vehicular assaults usually fall to compensation agencies. There is, however, little guidance around how compensation agencies should respond to mental and physical injury claims arising from large-scale transport incidents. Methods: A Delphi review methodology was used to establish expert consensus recommendations on the major components of “no fault” injury claim processes for mental and physical injury. Results: Thirty-three international experts participated in a 3-round online survey to rate their agreement on key statements generated from the literature. Consensus was achieved for 45 of 60 (75%) statements, which were synthesized into 36 recommendations falling within the domains of (1) facilitating claims, (2) eligibility rules, (3) payments and benefits for clients, (4) claims management procedures, (5) making and explaining decisions, (6) support and information resources for clients, (7) managing scheme staff and organizational response, (8) clients with special circumstances, and (9) scheme values and integrity. Conclusions: The recommendations present an opportunity for agencies to review their existing claims management systems and procedures. They also provide the basis for the development of best practice guidelines, which may be adapted for application to compensation schemes in different contexts worldwide.

General practice is an ideal setting to be providing nutrition advice; however, it is important that the role of general practitioners (GPs) and practice nurses in providing nutrition advice is acknowledged and defined. This article aims to discuss the role of GPs and practice nurses in the delivery of nutrition advice. Ten general practitioners and 12 practice nurses from a NSW urban Division of General Practice participated in questionnaires and a Lifescripts© implementation study, as well as their consenting patients receiving Lifescripts© (n = 13). An online survey was conducted with 90 Australian private practice dietitians. Semi-structured telephone interviews were conducted with 52 Australian private practice dietitians. The provision of basic nutrition advice is acknowledged to be part of the role of GPs and practice nurses, as they are the first point of contact for patients, allowing them to raise nutrition awareness. However, it is important that this advice is evidence based and able to be delivered in a time-efficient manner. Increased nutrition education and the availability of appropriate resources and nutrition-related best practice guidelines will assist in this process.

Cloud computing is an emerging computing paradigm which introduces novel opportunities to establish large scale, flexible computing infrastructures. However, security underpins extensive adoption of Cloud computing. This paper presents efforts to address one of the significant issues with respect to security of Clouds i.e. intrusion detection and severity analysis. An abstract model for integrated intrusion detection and severity analysis for Clouds is proposed to facilitate minimal intrusion response time while preserving the overall security of the Cloud infrastructures. In order to assess the effectiveness of the proposed model, detailed architectural evaluation using Architectural Trade-off Analysis Model (ATAM) is used. A set of recommendations which can be used as a set of best practice guidelines while implementing the proposed architecture is discussed.

Abstract Background: There is a lack of high quality evidence to guide the optimal management of diabetes-related foot infection, particularly in severe cases of diabetes-related foot infection and diabetes-related foot osteomyelitis. This study examined the opinions of surgeons about the best management of severe diabetes-related foot infection. Methods: Vascular and orthopaedic surgeons in Australia and New Zealand were invited to complete an online survey via email. The survey included multi-choice and open-ended questions on clinical management of diabetes-related foot infection. Responses of vascular surgeons and orthopaedic surgeons were compared using non-parametric statistical tests. Open-text responses were examined using inductive content analysis. Results: 29 vascular and 20 orthopaedic surgeons completed the survey. One-third (28.6%) used best-practice guidelines to assist in decisions about foot infection management. Areas for guideline improvement identified included more specific advice regarding the indications for available treatments, more information about the overall non-surgical patient management and advice on how management can be varied in regions with limited health service resource. The probe-to-bone test and magnetic resonance imaging were the preferred methods of diagnosing osteomyelitis. Approximately half (51.2%) of respondents indicated piperacillin combined with Tazobactam as the preferred antibiotic choice for empirical treatment of severe diabetes-related foot infection. Negative pressure wound therapy was the most common way of managing a wound following debridement. All vascular surgeons (100%) made revascularisation decisions based the severity of ischemia while most orthopaedic surgeons (66.7%) were likely to refer to vascular surgeons to make revascularisation decisions. Vascular surgeons preferred using wound swabs while orthopaedic surgeons favoured tissue or bone biopsies to determine the choice of antibiotic. Respondents perceived a moderate variation in management decisions between specialists and supported the need for further trials to test different management pathways. Conclusions: Most vascular and orthopaedic surgeons do not use best-practice guidelines to assist in decisions about management of diabetes-related foot infection. Vascular and orthopaedic surgeons appear to have different preferences for wound sampling to determine choice of antibiotic. There is a need to advance available evidence supporting different treatments and the related guidelines for the surgical managing of diabetes-related foot infection.

Many academic teams and granting agencies undergo a process of reflection at the completion of research projects to understand lessons learned and develop best practice guidelines. Generally completed at the project’s end, these reviews focus on the actual research work accomplished with little discussion of the work relationships and process involved. As a result, some hard-earned lessons are forgotten or minimized through the passage of time. Additional learning about the nature of collaboration may be gained if this type of reflection occurs during the project’s life. Building on earlier examinations of INKE, this paper contributes to that discussion with an exploration of seventh and final year of a large-scale research project.Implementing New Knowledge Environment (INKE) serves as a case study for this research. Members of the administrative team, researchers, postdoctoral fellows, graduate research assistants, and others are asked about their experiences collaborating within INKE on an annual basis in order to understand the nature of collaboration and ways that it may change over the life of a long-term grant. Interviewees continue to outline benefits for collaboration within INKE while admitting that there continue to be challenges. They also outline several lessons learned which will be applied to the next project.

ABSTRACT INTRODUCTION Patients are interested in receiving text messages (texts) related to their health care. However, anecdotes are emerging of associated problems and it is possible that many of the potential pitfalls are not recognised. AIM To assess clinicians’ attitudes and behaviours towards text messaging (texting) with patients. METHODS A voluntary, anonymous, online survey was created and distributed to general practitioners and physiotherapists in New Zealand and to Sports Medicine Fellows and Sports Medicine Registrars in New Zealand and Australia. RESULTS In total, 322 clinicians completed the survey. Texting behaviours relating to accuracy, privacy and security were identified. A range of sensitive and important medical information was frequently conveyed and at times forwarded to third parties. The clinicians generally felt uneasy communicating this way and some felt pressured into it. Most thought that guidelines are insufficient and that they had not received sufficient education on the issues. Most were interested in further education. CONCLUSION This study has demonstrated frequent texting between clinicians and patients. It has highlighted potential risks to the privacy, accuracy and security of medical information. Current guidelines and education may be insufficient. Clinicians were interested in receiving readily available best practice guidelines and education regarding texting.

Parliamentary speeches present one of the most consistently available sources of information about the political priorities, actor positions, and conflict structures in democratic states. Recent advances of automated text analysis offer more and more tools to tap into this information reservoir in a systematic manner. However, collecting the high-quality text data needed for unleashing the comparative potential of the various text analysis algorithms out there is a costly endeavor and faces various pragmatic hurdles. Against this challenge, this chapter offers three contributions. First, we outline best practice guidelines and useful tools for researchers wishing to collect or to extend existing legislative debate corpora. Second, we present an extended version of the ParlSpeech Corpus. Third, we highlight the difficulties of comparing text-as-data outputs across different parliaments, pointing to varying languages, varying traditions and conventions, and varying metadata availability.

Purpose The purpose of this paper is to provide, through a set of indicators, an overview of the way in which Spanish institutional repositories are run and the services they offer their respective institutions and other users. The selected descriptors are based on aspects related to technology, procedures, content, marketing and the personnel responsible for managing repositories. Design/methodology/approach In order to establish the indicators, a thorough review of the literature was carried out to identify existing indicators that are used to assess repositories. These were divided into five categories (technology, procedures, content, marketing and personnel) with a total of 48 components. An online survey was conducted with the repositories managers of 66 Spanish research institutions in order to verify the degree of fulfilment of the selected indicators. Findings The survey received forty-six responses, which represented a response rate of 69 per cent. Of these, 44 came from universities and two from research centres. In total, 65 per cent of the repositories have the capacity to import data from and export data to other university systems, mainly Current Research Information System (32 per cent). Most repositories have mechanisms for the large-scale import and export of metadata and digital objects (83 per cent). The use of altmetrics in repositories is widespread (44 per cent). Authors and librarians deposit most frequently (37 and 32 per cent, respectively), in spite of the fact that 44 per cent do not have full-time staff working in the repository. In more than 80 per cent of the repositories, between 90 and 100 per cent of the deposits are full-text documents. With respect to the tools used to promote the repository within the institution, these are primarily face-to-face training sessions (82 per cent), followed by support materials such as manuals and help pages (65 per cent). The academic authorities encourage open access among researchers in 56 per cent of cases, a significant element in repository marketing. Originality/value This work proposes a model based on five dimensions and 48 indicators to assess institutional repositories. This approach has been applied to Spanish institutional repositories to provide up-to-date information about their management procedures and promotional methods and the services they offer authors and the university community. This overview of Spanish repositories has provided an insight into the way in which repositories have evolved in recent years and allowed potential improvements to be identified based on the most advanced repositories. This model can also be exported to assess institutional repositories in other countries.

Purpose of the study: Uninterrupted hospital services and medical functions are the keys to functional resilience to cope with mass casualties. This paper presents the important level of resilience indicators for hospital functions to withstand natural disasters. Methodology: For the survey, 21 indicators are grouped into three domains focusing on i) general concerns of healthcare infrastructure planning ii) design and planning of hospital buildings iii) emergency service and management. The corresponding indicators were ranked on a Likert scale of 1 to 5. The authors collected 389 responses through an online survey of the healthcare professionals including disaster management professionals, medical officers, hospital architects, planners, project managers, and engineers. Main Findings: The data were analysed for determining the Relative Importance Index (RII) of each indicator. The top 7 indicators as an outcome of this research are: ‘access to the emergency services (0.861), ‘planning of refugee settlements’ (0.814), ‘uninterrupted supply of MEP services to critical units’ (0.871), ‘signages for internal circulation’ (0.845), ‘adaptive control, command, and communication system’ (0.848), ‘flexible spatial planning in case of a surge of patients’(0.813), ‘ensuring availability of healthcare workers with the provision of support infrastructure’ (0.758). Applications of this study: Assessment of the top indicators highlight the importance of ‘flexible design’ and ‘access to medical functions of a hospital building’. Based on these outcomes, it is proposed to develop a numerical framework for a comprehensive design appraisal of resilient hospital buildings.
