Differential semantic processing in patients with schizophrenia versus bipolar disorder: an N400 study

AbstractObjective:Both bipolar disorder (BD) and schizophrenia (SZ) are associated with language and thought symptoms that probably reflect a semantic memory-related impairment. We conducted a preliminary study to explore the nature of semantic processing in these disorders, using event-related potentials (ERPs).Methods:Twelve patients with BD, 10 patients with SZ and a matched group of 21 healthy controls (HC) underwent EEG recording while they heard sentences containing homophones or control words and performed a semantic ambiguity resolution task on congruent or incongruent targets.Results:Mean N400 amplitude differed between groups for homophones. Patients with SZ made more resolution errors than HC and exhibited a greater N400 congruity effect in ambiguous conditions than BD. In BD, the opposite N400 congruity effect was observed in ambiguous conditions.Conclusion:Results indicated differences in semantic processing between BD and SZ. Further studies with larger populations are needed in order to develop neurophysiological markers of these disorders.Significant OutcomesIn ambiguous conditions, patients with SZ exhibited a greater N400 difference between congruent and incongruent conditions than patients with BD.In ambiguous conditions, patients with SZ exhibited greater N400 amplitude in incongruent conditions than in congruent ones, whereas patients with BD exhibited the opposite N400 congruity effect.Ambiguity resolution results suggest that patients with SZ have difficulty considering the context, while patients with BD overactivate the dominant meaning of homophones and have difficulty inhibiting it.

Socioeconomic status (SES) refers to the social position or class according to their material and non-material social resources. We conducted a study with 60 college students to explore whether SES affects past self-evaluation and used event-related potentials (ERPs) in a self-reference task that required participants to judge whether the trait adjectives (positive or negative) describing themselves 5 years ago were appropriate for them. Behavioral data showed that individuals’ positive past self-evaluations were significantly higher than individuals’ negative past self-evaluations, regardless of high or low SES. Individuals with high SES had significantly higher positive past self-evaluations than those with low SES. ERP data showed that in the low SES group, negative adjectives elicited a marginally greater N400 amplitude than positive adjectives; in the high SES group, negative adjectives elicited a greater late positive potential (LPP) amplitude than positive adjectives. N400 is an index of the accessibility of semantic processing, and a larger N400 amplitude reflects less fluent semantic processing. LPP is an index of continuous attention during late processing; the larger LPP amplitude is elicited, the more attention resources are invested. Our results indicated that compared with college students with low SES, the past self-evaluations of college students with high SES were more positive; college students with high SES paid more attention to negative adjectives. However, college students with low SES were marginally less fluent in processing negative adjectives.

Recent neuroimaging and electrophysiological studies reveal how the reading system successfully adapts when phonological codes are relatively coarse-grained due to reduced auditory input during development. New evidence suggests that the optimal end-state for the reading system may differ for deaf versus hearing adults and indicates that certain neural patterns that are maladaptive for hearing readers may be beneficial for deaf readers. This chapter focuses on deaf adults who are signers and have achieved reading success. Although the left-hemisphere-dominant reading circuit is largely similar in both deaf and hearing individuals, skilled deaf readers exhibit a more bilateral neural response to written words and sentences than their hearing peers, as measured by event-related potentials and functional magnetic resonance imaging. Skilled deaf readers may also rely more on neural regions involved in semantic processing than hearing readers do. Overall, emerging evidence indicates that the neural markers for reading skill may differ for deaf and hearing adults.

The present study investigated the interaction of emotional prosody and word valence during emotional comprehension in men and women. In a prosody-word interference task, participants listened to positive, neutral, and negative words that were spoken with a happy, neutral, and angry prosody. Participants were asked to rate word valence while ignoring emotional prosody, or vice versa. Congruent stimuli were responded faster and more accurately as compared to incongruent emotional stimuli. This behavioral effect was more salient for the word valence task than for the prosodic task and was comparable between men and women. The event-related potentials (ERPs) revealed a smaller N400 amplitude for congruent as compared to emotionally incongruent stimuli. This ERP effect, however, was significant only for the word valence judgment and only for female listeners. The present data suggest that the word valence judgment was more difficult and more easily influenced by task-irrelevant emotional information than the prosodic task in both men and women. Furthermore, although emotional prosody and word valence may have a similar influence on an emotional judgment in both sexes, ERPs indicate sex differences in the underlying processing. Women, but not men, show an interaction between prosody and word valence during a semantic processing stage.

AbstractObjectivesTo evaluate alterations of top-down and/or bottom-up attention in migraine and their cortical underpinnings.Methods19 migraineurs between attacks and 19 matched control participants performed a task evaluating jointly top-down and bottom-up attention, using visually-cued target sounds and unexpected task-irrelevant distracting sounds. Behavioral responses and MEG/EEG were recorded. Event-related potentials and fields (ERPs/ERFs) were processed and source reconstruction was applied to ERFs.ResultsAt the behavioral level, neither top-down nor bottom-up attentional processes appeared to be altered in migraine. However, migraineurs presented heightened evoked responses following distracting sounds (orienting component of the N1 and Re-Orienting Negativity, RON) and following target sounds (orienting component of the N1), concomitant to an increased recruitment of the right temporo-parietal junction. They also displayed an increased effect of the cue informational value on target processing resulting in the elicitation of a negative difference (Nd).ConclusionsMigraineurs appear to display increased bottom-up orienting response to all incoming sounds, and an enhanced recruitment of top-down attention.SignificanceThe interictal state in migraine is characterized by an exacerbation of the orienting response to attended and unattended sounds. These attentional alterations might participate to the peculiar vulnerability of the migraine brain to all incoming stimuli.HighlightsMigraineurs performed as well as healthy participants in an attention task.However, EEG markers of both bottom-up and top-down attention are increased.Migraine is also associated with a facilitated recruitment of the right temporo-parietal junction.

Abstract Background Deficits in event-related potential (ERP) including duration mismatch negativity (MMN) and P3a have been demonstrated widely in chronic schizophrenia (SZ) but inconsistent findings were reported in first-episode patients. Psychotropic medications and diagnosis might contribute to different findings on MMN/P3a ERP in first-episode patients. The present study examined MMN and P3a in first episode drug naïve SZ and bipolar disorder (BPD) patients and explored the relationships among ERPs, neurocognition and global functioning. Methods Twenty SZ, 24 BPD and 49 age and sex-matched healthy controls were enrolled in this study. Data of clinical symptoms [Positive and Negative Symptoms Scale (PANSS), Young Manic Rating Scale (YMRS), Hamilton Depression Rating Scale (HAMD)], neurocognition [Wechsler Adult Intelligence Scale (WAIS), Cattell's Culture Fair Intelligence Test (CCFT), Delay Matching to Sample (DMS), Rapid Visual Information Processing (RVP)], and functioning [Functioning Assessment Short Test (FAST)] were collected. P3a and MMN were elicited using a passive auditory oddball paradigm. Results Significant MMN and P3a deficits and impaired neurocognition were found in both SZ and BPD patients. In SZ, MMN was significantly correlated with FAST (r = 0.48) and CCFT (r = −0.31). In BPD, MMN was significantly correlated with DMS (r = −0.54). For P3a, RVP and FAST scores were significant predictors in SZ, whereas RVP, WAIS and FAST were significant predictors in BPD. Conclusions The present study found deficits in MMN, P3a, neurocognition in drug naïve SZ and BPD patients. These deficits appeared to link with levels of higher-order cognition and functioning.

AbstractThis article examines the role of attention and automaticity in auditory processing as revealed by event-related potential (ERP) research. An ERP component called the mismatch negativity, generated by the brain's automatic response to changes in repetitive auditory input, reveals that physical features of auditory stimuli are fully processed whether or not they are attended. It also suggests that there exist precise neuronal representations of the physical features of recent auditory stimuli, perhaps the traces underlying acoustic sensory (“echoic”) memory. A mechanism of passive attention switching in response to changes in repetitive input is also implicated.Conscious perception of discrete acoustic stimuli might be mediated by some of the mechanisms underlying another ERP component (NI), one sensitive to stimulus onset and offset. Frequent passive attentional shifts might accountforthe effect cognitive psychologists describe as “the breakthrough of the unattended” (Broadbent 1982), that is, that even unattended stimuli may be semantically processed, without assuming automatic semantic processing or late selection in selective attention.The processing negativity supports the early-selection theory and may arise from a mechanism for selectively attending to stimuli defined by certain features. This stimulus selection occurs in the form ofa matching process in which each input is compared with the “attentional trace,” a voluntarily maintained representation of the task-relevant features of the stimulus to be attended. The attentional mechanism described might underlie the stimulus-set mode of attention proposed by Broadbent. Finally, a model of automatic and attentional processing in audition is proposed that is based mainly on the aforementioned ERP components and some other physiological measures.
