Transactional relations across contextual strain, parenting quality, and early childhood regulation and adaptation in a high-risk sample

AbstractThis investigation examined transactional relations across contextual strain, parenting quality, and child adjustment in 209 mothers and children at 24, 42, and 72 months of age. Independent ratings of mothers' stressful life events, social support, and relationship quality provided an objective measure of maternal contextual strain. Observers evaluated parenting quality during parent–child interactions at each time point. Child regulatory functioning during laboratory tasks at 24 and 42 months was evaluated by independent observers based on both behavioral (e.g., noncompliance, distractibility) and emotional (e.g., frustration, anger) indices. At 72 months, teachers reported on children's externalizing behaviors, and children completed objective measures of academic achievement. Nested path analyses were used to evaluate increasingly complex models of influence, including transactional relations between child and parent, effects from contextual strain to parenting and child adaptation, and reciprocal effects from child and parent behavior to contextual strain. Over and above stability within each domain and cross-sectional cross-domain covariation, significant paths emerged from maternal contextual strain to subsequent child adjustment. Bidirectional relations between parenting and child adjustment were especially prominent among boys. These findings counter unidirectional models of parent-mediated contextual effects by highlighting the direct influences of contextual strain and parent–child transactions on early childhood behavioral and academic adjustment, respectively.

One of the driving theories of the development of child antisocial behavior is Patterson’s model of parent-child coercion. Although Patterson hypothesized that coercive processes are established beginning in early childhood, few studies have sought to understand its developmental precursors in early childhood. Even fewer studies have attempted to examine factors that might compromise parenting quality and lead to coercive parent-child interactions during early childhood. One factor repeatedly shown to compromise parenting quality is parental depression. As such, this chapter focuses on how depression among mothers and alternative caregivers, including fathers, is associated with the early onset of coercive family dynamics. The results of the current study have implications for understanding the genesis of coercive processes and for the design of early prevention programs, affirming the importance of including maternal depression in our prevention models.

The chapter highlights results from the Buffalo Longitudinal Study, which began in infancy and was guided by a developmental cascade model. The chapter discusses the importance of the co-occurrence of parent alcohol problems with depression and antisocial behavior beginning in early childhood, and how these parental risks in infancy may predict the quality of parent–child interactions and infant–parent attachment. These processes in early childhood may set the stage for one of the most salient developmental issues at preschool age—the development of self-regulation. Together, the parent–child relationship and child self-regulation may predict one of the most clearly established pathways to adolescence substance use disorders—continuity of externalizing problems from childhood to adolescence. Finally, this chapter presents results from a developmental cascade model from infancy to adolescence, with implications for development of preventive interventions for adolescent substance use disorders.

AbstractPsychological stress has become a major concern, potentially leading to diverse health problems including psychopathology such as depression and anxiety. Transactional Model of Stress and Coping is an established model, conceptualizing stressful experiences via person–environment relationship. This cross-sectional study aimed to explore the pathway from stress to depression/anxiety, with a focus on self-criticism (inadequate-self and hated-self) and self-reassurance (reassured-self) in Czech students who suffered from high prevalence of mental health problems. Convenience sample of 119 undergraduates completed the Depression Anxiety and Stress Scale-21 and the Forms of the Self-Criticizing/Attacking & Self-Reassuring Scale. Correlation and path analyses were conducted. The Strengthening the Reporting of Observational Studies in Epidemiology guidelines were used to aid an accurate and complete report of the study. Depression, anxiety, and stress were positively associated with inadequate-self and hated-self while negatively associated with reassured-self. Both inadequate-self and hated-self partially mediated the stress–depression and stress–anxiety relationships, whereas reassured-self only partially mediated the stress–depression relationship. Inadequate-self had greater impact on the stress–depression/anxiety pathways than hated-self and reassured-self. Findings indicate that clinical treatment may benefit from targeting the feelings of inadequacy to prevent stress progressing to psychopathology. This is particularly relevant as stress levels are rising globally. Our findings offer developments to the Transactional Model, and help practitioners and educators identify solutions to protect mental health of Czech university students.

Introduction: Malnutrition in children is one of the most prevalent global health challenges, and malnourished children have a higher risk of death from childhood diseases. Early childhood caries (ECC) is the most common chronic disease of childhood. Complications from ECC such as pain, loss of tooth/teeth, and infection can undermine a child’s nutrition and growth. Aim: This study aims to evaluate the severity of decay, missing, and filled tooth (dmft) by nutritional status using the z scores of the anthropometric measurements: height for age (HFA), weight for age (WFA), weight for height (WFH), and body mass index for age (BMIA) among children with ECC in Nigeria. Study Design: This is a cross-sectional study conducted in 5 local government areas (LGAs) in Lagos State, Nigeria. A multistage sampling technique was used. Results: A total of 273 cases of ECC were included in the analyses (mean age 4.19 ± 0.96 y). Overall, the mean dmft was 3.04 ± 2.28, and most (96%) were accounted for by untreated decay. The distribution of dmft within the different z score categories of BMIA (<–3 = severely wasted, –2 to –3 = wasted, –2 to +2 = normal, +2 to +3 = overweight and >+3 = obese) showed the highest dmft scores among the combined severely wasted and wasted groups, lowest among children with normal z scores, and intermediate in the overweight and obese groups. There was a significant negative correlation between BMIA z score, WFH z score, and dmft ( r = −0.181, P < 0.05 and r = −0.143, P < 0.05, respectively). However, the correlations between HFA z score, WFA z score, and dmft were positive but not significant ( r = 0.048, P = 0.44 and r = 0.022, P = 0.77, respectively). Conclusion: Our study showed an increased severity of dental caries among severely wasted or wasted children with ECC compared to those of normal or overweight. Knowledge Transfer Statement: The results from this study will raise awareness among clinicians and policy makers on the need for a primary prevention program for early childhood caries in countries with high burden of malnutrition and limited resources. Also, it will help draw the attention of clinicians to the caries status of malnourished children that can be managed to improve the nutritional outcomes.

Abstract Objective The current researches during COVID-19 have not yet addressed somatic distress among cancer patients. The aims of this study are to investigate the somatic distress and psychological symptoms levels of cancer patients, and analyze the influencing factors on somatic distress during the pandemic. Methods This cross-sectional study included consecutively selected 216 eligible cancer patients. The Patient Health Questionnaire-15, Posttraumatic Stress Disorder (PTSD) Checklist for DSM-5, The Depression, Anxiety and Stress Scale-21, the Brief Resilience Scale, the Stressful Life Events List due to Pandemic and, demographic and clinical characteristics form were administered to the participants with face to face interviewing. Results The moderate to severe somatic distress rate was % 38 and probable PTSD rate was 20.4%. Depression, anxiety and stress symptoms were 36.1%, 49.1% and 45.4%, respectively, from mild to extremely severe at any level. There were substantial association between somatic symptoms severity and high PTSD, anxiety, depression, and stress symptoms levels. Low educations levels, high anxiety levels, high experience stressful life events, and low psychological resilience predicted high somatic distress levels. Conclusion This study demonstrates the high risk of somatic distress, PTSD, depression, anxiety and stress in patients with cancer during the pandemic. In addition, somatic distress may indicate high levels of psychological symptoms, high experience stressful life events, and low psychological resilience. It underscores the need to assess psychological status during the pandemic, especially those with high level somatic symptoms.
