Abstract. Voluntary incident reports by commercial pilots provide essential data for airline efforts in learning from incidents (LFI). Because LFI is frequently limited by pilots not reporting incidents voluntarily, we interviewed seven commercial aviation safety experts in a focus group to derive factors influencing the voluntary reporting behavior of pilots’ self-inflicted incidents. As a result, we derived 36 factors and integrated them into a motivational framework by van den Broeck et al. (2019) . Pilots pursue various goals when voluntarily reporting incidents, such as enabling safety-related change or organizational learning. This behavior is influenced by personal antecedents, such as shame, and contextual antecedents, such as feedback. Our work expands the understanding of motivational aspects of voluntary incident reporting and discusses practical interrelations.

Abstract. The aim of the present study was to examine commercial pilots’ reporting behavior and confidence in their airlines’ just culture. In pursuit of this aim, 539 European-based pilots participated in the study by answering an online questionnaire. The results are compared with an earlier study comprising Australian-based pilots. The results reveal that 84% and 57% of the European and Australian pilots, respectively, trust their airlines’ just culture. When comparing reporting behaviors, it was found that 53% of the Australian pilots and 33% of the European pilots stated they had failed to report, or had under-reported, safety information in their airlines’ safety management system. A distinct difference with the aviation regulatory backdrop that the two pilot groups operate within is the legal legitimization of just culture in European law. It is unknown whether this difference influences confidence in just culture or has an effect on reporting behavior.

The complexity of commercial aviation operations has grown substantially in recent years, together with a diversification of techniques for collecting and analyzing flight data. As a result, data-driven frameworks for enhancing flight safety have grown in popularity. Data-driven techniques offer efficient and repeatable exploration of patterns and anomalies in large datasets. Text-based flight safety data presents a unique challenge in its subjectivity, and relies on natural language processing tools to extract underlying trends from narratives. In this paper, a methodology is presented for the analysis of aviation safety narratives based on text-based accounts of in-flight events and categorical metadata parameters which accompany them. An extensive pre-processing routine is presented, including a comparison between numeric models of textual representation for the purposes of document classification. A framework for categorizing and visualizing narratives is presented through a combination of k-means clustering and 2-D mapping with t-Distributed Stochastic Neighbor Embedding (t-SNE). A cluster post-processing routine is developed for identifying driving factors in each cluster and building a hierarchical structure of cluster and sub-cluster labels. The Aviation Safety Reporting System (ASRS), which includes over a million de-identified voluntarily submitted reports describing aviation safety incidents for commercial flights, is analyzed as a case study for the methodology. The method results in the identification of 10 major clusters and a total of 31 sub-clusters. The identified groupings are post-processed through metadata-based statistical analysis of the learned clusters. The developed method shows promise in uncovering trends from clusters that are not evident in existing anomaly labels in the data and offers a new tool for obtaining insights from text-based safety data that complement existing approaches.

Surgical castration is a painful husbandry procedure performed on piglets in the United States (US) to improve meat quality. Veterinarians play a crucial role in developing pain management protocols. However, providing pain management for castration is not common practice in US swine production systems. Therefore, the objective of the present study is to identify factors influencing swine veterinarian decision-making in regard to pain management protocols for piglet castration using focus group methodologies. Swine veterinarians (n = 21) were recruited to participate in one of three focus groups. Audio recordings were transcribed verbatim and analyzed by two independent coders who identified three areas of focus, including (1) the lack of approved products validated for efficacy, (2) economic limitations and challenges, and (3) deficient guidelines and training for veterinarians to develop protocols. Although participating veterinarians acknowledged the importance of pain management from an animal welfare standpoint, these barriers must be addressed to ensure that castration pain can be successfully mitigated on-farm.

Why is it that, despite the large amount of studies on IT governance, many professionals still fail to put these theories into practice? Building on the previous question, we reviewed the literature to examine how organizational learning theories can help in understanding the gap between the ‘theory' and ‘practice' of IT governance. Thereafter, a focus group discussed this gap within the context of Industry 4.0. The findings indicate important concerns that might hinder learning in organizations, including: lack of shared domain knowledge between business and IT stakeholders, lack of shared understanding and poor diffusion of IT governance practices in the organization. Unless these concerns are successfully resolved, executives will experience difficulties to govern IT successfully within the context of Industry 4.0. We provide recommendations to practitioners with regards to the capabilities which should be present in the organisation to overcome such concerns. These capabilities represent a T-shaped portfolio of skills.
