Abstract Background Visual impairment in children is a significant public health problem affecting millions of children globally. Many eye problems experienced by children can be easily diagnosed and treated. We conducted a qualitative study with teachers and optometrists involved in a school-based vision screening programme in Quetta district of Pakistan to explore their experiences of training, vision screening and referrals and to identify factors impacting on the effectiveness of the programme. Methods Between April 2018 and June 2018, we conducted semi-structured in-depth interviews with 14 teachers from eight purposefully selected schools with high rates of inaccurate (false positive) referrals. Interviews were also conducted with three optometrists from a not-for profit private eye care hospital that had trained the teachers. Interviews were audio recorded and professionally transcribed. NVIVO software version 12 was used to code and thematically analyze the data. Results Findings suggest that the importance of school-based vision screening was well understood and appreciated by the teachers and optometrists. Most participants felt that there was a strong level of support for the vision screening programme within the participating schools. However, there were a number of operational issues undermining the quality of screening. Eight teachers felt that the duration of the training was insufficient; the training was rushed; six teachers said that the procedures were not sufficiently explained, and the teachers had no time to practice. The screening protocol was not always followed by the teachers. Additionally, many teachers reported being overburdened with other work, which affected both their levels of participation in the training and the time they spent on the screening. Conclusions School-based vision screening by teachers is a cost-effective strategy to detect and treat children’s vision impairment early on. In the programme reviewed here however, a significant number of teachers over referred children to ophthalmic services, overwhelming their capacity and undermining the efficiency of the approach. To maximise the effectiveness and efficiency of school-based screening, future initiatives should give sufficient attention to the duration of the teacher training, experience of trainers, support supervision, refresher trainings, regular use of the screening guidelines, and the workload and motivation of those trained.

Background/AimTo investigate the prevalence, causes and risk factors of visual impairment (VI) among the elderly in ‘home for the aged’ in Hyderabad, India.MethodsIndividuals aged ≥60 years were recruited from 41 ‘homes for the aged’. All participants had complete eye examinations including presenting visual acuity, refraction, slit-lamp examination, intraocular pressure measurement and fundus imaging by trained clinicians. VI was defined as presenting visual acuity worse than 6/18 in the better eye. Multivariate logistic regression was used to determine the risk factors associated with VI.Results1512 elderly residents from 41 homes for the aged were enumerated, of whom 1182 (78.1%) were examined. The mean age of examined participants was 75.0 years (SD 8.8 years; range: 60–108 years); 35.4% of those examined were men. The prevalence of VI was 30.1% (95% CI 27.5 to 32.8). The leading cause of VI was cataract (46.3%, n=165), followed by uncorrected refractive error (27.0%, n=96), posterior capsular opacification (14.9%, n=53) and posterior segment disease (6.5%, n=23). Overall, 88.2% of the VI was either treatable or correctable. In multiple logistic regression, those aged 80 years and older (OR: 1.7, p<0.01), living in ‘free’ homes (OR: 1.5, p<0.01) and who were immobile/bedridden (OR: 3.02, p<0.01) had significantly higher odds of VI. Gender was not associated with VI.ConclusionsVI was common and largely avoidable in residents of ‘homes for the aged’ in Hyderabad, India. Screening for vision loss in ‘homes for aged’ and the provision of appropriate services should become routine practice to achieve the goal of healthy ageing in India.

ObjectiveTo determine the rate of falls reported by older community dwellers in Shenzhen, China and to identify fall-related risk factors.MethodParticipants were community dwellers residing in Shenzhen, China, who were aged 60 years and over and were recruited using multistage random sampling. All participants were surveyed about demographic and health-related information, mood, vision and hearing impairment, self-rated health and retrospective falls, and a test of balance was administered. Univariate and multivariate negative binomial regression was used to identify factors associated with a greater number of falls.ResultStudy participants were 1290 people aged 60–98 years (mean 68.2 years, SD ±6.5). One hundred and seventy-seven falls were reported. One hundred and eleven (8.6%) participants reported one fall in the past year, 17 (1.3%) participants reported two falls and 10 (0.8%) participants reported three or more falls. Univariate analysis showed that age, living alone, presence of a medical condition, medication usage, visual impairment, poor subjective body sense perception, low mood, poor self-rated health and poor balance were associated with a greater number of falls in the past year. Multivariate analysis identified presence of a medical condition (incidence rate ratio (IRR)=1.40, 95% CI 1.19 to 1.67), living alone (IRR=2.46, 95% CI 1.12 to 5.41), visual impairment (IRR=1.46, 95% CI 1.03 to 2.08), walking aid use (IRR=2.29, 95% CI 1.12 to 4.69) and impaired balance (IRR=1.05, 95% CI 1.00 to 1.10) to be significantly associated with a greater number of falls in the past year.ConclusionMore falls occurred in older Chinese people with presence of a medical condition, living alone, visual impairment, used a walking aid and impaired balance.
