Abstract Diagnosis and management of nerve impairment in adults can be difficult due to the myriad of other diagnoses that must be considered. Accurate diagnosis of severity of a nerve impairment and determination of prognostic indicators for return of adequate function are paramount to best management. The strongest prognostic test of nerve function is electromyography. Team management by an otolaryngologist and speech-language pathologist of vocal fold paralysis needs to be sensitive to the patient's needs, both short-term and long-term. This article summarizes the current diagnostic tests and options. Case studies are included to guide the reader through team management and surgical options.

Objective To examine the association of laryngoplasty, voice therapy, and pneumonia rate in patients with unilateral vocal fold paralysis (UVFP). Study Design Population-based retrospective cohort study. Setting Data were collected from the LHID2000 (Longitudinal Health Insurance Database 2000), containing the information of 1 million randomly selected patients in Taiwan. Methods In the LHID2000, we identified 439 patients having new diagnoses of UVFP from 1997 to 2013. We grouped the aforementioned patients according to UVFP treatment and probed the occurrence of pneumonia: 305 patients underwent laryngoplasty or voice therapy, and 134 patients did not undergo treatment. Follow-up procedures were executed for the enrollees until death or December 31, 2013, representing the end of the study period. We assessed the association of UVFP treatment and pneumonia by executing Cox proportional hazards regression. Results The pneumonia cumulative incidence was significantly higher among enrolled patients without treatment than in those receiving treatment ( P < .001). The pneumonia incidence was significantly lower in patients receiving UVFP treatment (hazard ratio, 0.49; 95% CI, 0.27-0.88; P = .018), as validated by the Cox proportional hazards model after adjustment. Patients undergoing laryngoplasty with or without voice therapy had a significantly lower incidence of pneumonia at 6 months and 1, 3, and 5 years, whereas those undergoing voice therapy alone did not. Conclusion Laryngoplasty was associated with a lower incidence of short- and long-term pneumonia in patients with UVFP. Physicians should encourage patients with UVFP at risk of aspiration to receive prompt evaluation as well as treatment.

Comprehensive environmental observation, eco-innovation and smartization are essential to ensure the delivery of the long-term data and information required to address the shift towards smart, green and integrated raw materials efficiency. For this reason we need the mine-wide digitalization and informatization base model, an advanced mine-wide decision support system and a smart supervision system to supervise and control the production, back to predefined short-term production targets with most likelihood and optimal approaches. There are three main steps to be taken: analysis, evaluation and determination of the shift requirements, development of the models as well as modeling of the scenarios and connection to the smart platform for the support of the decision makers. The paper aims to consider what would be required for a raw materials area to operate as a modern smart technology-supported business. It attempts to provide a vision of some future smart architectures scenarios.

Many techniques have been developed for medialization of the paralyzed vocal fold. The purpose of this study is to evaluate autologous fat as an alternative to alloplastic substances for use in vocal fold medialization. Eight dogs underwent left recurrent laryngeal nerve sectioning. Autologous fat was harvested, and the paralyzed vocal fold was medialized by injecting the fat into the thyroarytenoid muscle. The animals were divided into three groups for evaluation at 1, 3, and 6 months. Videolaryngoscopy was performed prior to sacrificing the animals. The larynges were sectioned coronally, and histologic studies were performed. The studies confirmed the preservation of viable fat at the injected site in all animals. Only a minimal inflammatory response was observed in the 1-month group. It would appear that fat injection is a viable alternative to Teflon injection and thyroplasty; it eliminates the need for alloplastic materials, does not appear to migrate, and does not require an open procedure.

OBJECTIVE: To histologically evaluate the long-term outcomes of autologous fat grafts after injection laryngoplasty in the human larynx. STUDY DESIGN AND SETTING: We injected liposuctioned fat for vocal fold augmentation in patients with vocal fold paralysis. We suctioned autologous fat from the low abdomen with an 18-G disposable needle and a 20-mL disposable syringe under negative pressure. This is different from the conventional liposuction technique and avoids the use of special equipment. In this article, we report the histological evaluation of 2 patients (patient 1: 12 months, patient 2: 41 months) who required total laryngectomy after autologous fat injection into the vocal folds. RESULTS: Histological examination revealed normal-appearing viable adipocytes with minimal inflammatory response in both patients. CONCLUSIONS: Our liposuctioned autologous fat injection histologically offered long-term improvement in patients with impaired glottal closure from vocal fold paralysis. In this study, the deposition of three heavy metals (Mn, Mo, and Ni) in the county of Obrenovac (Serbia) in four moss taxa (Bryum argenteum, Bryum capillare, Brachythecium sp., and Hypnum cupressiforme) is presented. The distribution of average heavy metal content in all mosses in the county of Obrenovac is presented on maps, while the long-term atmospheric deposition (in the mosses Bryum argenteum and B. capillare) and short term atmospheric deposition (in the mosses Brachythecium sp. and Hypnum cupressiforme) are discussed and given in tabular form. Areas of the highest contaminations are highlighted.
