AbstractPsoriasis on the hands and feet often appears on the palms and soles that are called palmopantar psoriasis. Treatment in modern medicine uses local and systematic antibiotics. Homoeopathy has mentioned medicines for plantar psoriasis. A case reported was a female of 7 years of age suffering from symptoms of itching, red scaly plaques, eruption with very offensive odour which aggravates at night, burning in both hands and feet which aggravates at night, weight-bearing area of the sole had painful cracking, scaling, swelling and blister. Also, she had burning on both hands and feet which aggravates at night and was diagnosed as plantar psoriasis. The homoeopathic medicine Syphilinum 1000C was prescribed on the basis of totality of symptoms, Materia Medica and miasm. Plantar psoriasis was reduced considerably within 10 days, and it disappeared completely in 2 months 20 days. No recurrence of plantar psoriasis was observed for another 5 years as well. Photographs during follow-up visits have provided as documentary evidence about the effectiveness of homoeopathic medicine Syphilinum 1000C in plantar psoriasis.

AbstractThis study aimed to describe a staged bilateral z-tenotomy and anastomosis procedure of flexor carpi ulnaris and superficial digital flexor tendons for the correction of carpal valgus and flexural deformity in a 1-year-old male-neutered Akita dog. Bilateral carpal valgus and flexural deformity were observed with palpably taut flexor carpi ulnaris tendons. The dog had a mild, weight-bearing bilateral forelimb lameness with the left forelimb more severely affected. Both forelimbs were treated with staged z-tenotomy and anastomosis procedure of both flexor carpi ulnaris and superficial digital flexor tendons. Successful tendon lengthening and correction of carpal valgus were achieved via z-tenotomy and anastomosis of affected tendons. No complications were observed. An 18-month follow-up revealed no evidence of lameness or carpal valgus. Though tendon injuries commonly occur in small animal patients, there is a shortage of reported cases, especially involving musculotendinous contractures. There are even fewer reports of successful tenotomy procedures. This case report supports successful outcome of the z-tenotomy procedure in a canine patient.

OBJECTIVES: To describe the cutaneous manifestations presented by a patient with pediatric multisystemic inflammatory syndrome (MIS-C) temporarily associated with SARS-CoV-2. METHODS: Female patient, 10 years old, presenting arthralgia in the wrists and ankles, edema in hands and feet, persistent fever, appearance of diffuse, and itchy maculopapular rash associated with odynophagia and productive cough. Evolution of the rash to violaceous lesions on the face, trunk and limbs (upper and lower), in addition to the appearance of vesicles on the face, within 48 hours. CBC suggestive of an infectious condition, with CRP 307mg/L (insert reference value) and ESR 61mm. RT-PCR for SARS-CoV-2 positive. RESULTS: Diagnosis of pediatric multisystemic inflammatory syndrome temporarily associated with SARS-CoV2. Transferred to ICU for monitoring and intravenous human immunoglobulin initiated. Good evolution, with the disappearance of injuries and discharge with outpatient follow-up. CONCLUSION: MIS-C has several dermatological manifestations and pediatricians must be attentive to the diagnosis, not limited to a specific presentation. In this case report, the importance of referral to tertiary referral centers for better case management and timely recognition of the syndrome in these patients is emphasized.

AbstractPhlyctenular conjunctivitis (PC) is characterised by formation of a whitish nodule on bulbar conjunctiva near or at the limbus with a localised conjunctival congestion. It is considered as an allergic reaction to endogenous bacterial protein. Conventional treatment includes use of topical steroids in simple PC, but if there is some degree of secondary infection or corneal involvement then combined steroid-antibiotics are used though there are high chances of recurrence. Homoeopathic literature has medicines listed for PC. The case reported was a female aged 60 years suffering from PC in right eye since 5 days and had no relief from local conventional treatment. She had complaints of redness, foreign body sensation and whitish discharge from the eye. Phlycten was at 6 o'clock position below the limbus with much conjunctival congestion around it and in the lower fornix. Homoeopathic Hepar sulphuris 30C resolved nodule within a week but the patient had itching of eye at the inner canthus, especially in the morning. Homoeopathic Natrum muriaticum 30C helped to resolve conjunctival congestion in the fornix and itching of eye within a week. Both medicines were prescribed on the basis of local signs and symptoms. There was no recurrence in the last 5 months. This case report with ocular photographs during follow-up provides documentary evidence about effectiveness of homoeopathy in PC.

Introduction:Alkaptonuria is a rare autosomal recessive metabolic disorder characterized by accumulation of homogentisic acid (HGA) due to an inherited deficiency of the enzyme HGA oxidase. Unlike rheumatoid arthritis which affects the small joints of the hands and feet, ochronotic arthropathy predominantly involves the large weight-bearing joints such as hips, knees, and spine. The knee is the most common joint to be affected. Ochronotic arthropathy is usually managed conservatively, but for severely affected hip and knee joints, replacement is considered. Case Report:This report describes a case of 57-year-old male who presented with bilateral knee osteoarthritis who was incidentally diagnosed with ochronosis intraoperatively during total knee arthroplasty, its challenges faced and post-operative functional outcome after 18 months follow-up. Conclusion:This case report highlights the need for high index of suspicion for preoperative diagnosis of ochronotic arthropathy. Difficult spinal anesthesia should be anticipated. Friable and stiff tendon due to ochronotic involvement can put the extensor tendon at risk of rupture during retraction of patella intraoperatively. Post-operative active quadriceps rehabilitation should be done with caution due to friable tendon. AbstractCervicothoracic junction tuberculosis involves the C7 to D3 levels of the spine which constitutes 5% of all spinal tuberculosis. As the cervicothoracic junction is transitional zone and a weight-bearing area, the anatomical considerations for treatment of cervicothoracic junction is very difficult. We did a lateral mass screw for C5, 6, and 7 and pedicle screw of D3 and 4 levels, and connected cervical 3.5 mm rod with thoracic 5.5 mm rod using a domino connector. We gave antituberculosis therapy for 18 months. We followed-up the patient routinely and she significantly improved.

Digital fibromyxoma is a rare soft tissue tumor with a propensity to grow in the digits of the hands and feet. While mostly asymptomatic, the tumors can grow and cause discomfort and eventually pain. It is a benign tumor, with treatment revolving around complete excision to prevent recurrence. In this case report, we discuss the case of a 11-year old boy presented with a swelling in the right ring finger that had been increasing in size for over a year. Despite the lack of pain or limitation of movement in the finger, the swelling caused concern to the patient and family due to cosmetic appearance. An excision biopsy with histopathological analysis of the tissue sample suggested the diagnosis of digital fibromyxoma. The high rate of recurrence is due to incomplete excision, and therefore regular follow up is recommended to monitor for recurrence.

This case report describes the positive outcome of a therapeutic intervention that integrated an intensive, residential component with follow-up telepractice for a 21 year old male who stutters. This therapy utilized an eclectic approach to intensive therapy in conjunction with a 12-month follow-up via video telepractice. The results indicated that the client benefited from the program as demonstrated by a reduction in percent stuttered syllables, a reduction in stuttering severity, and a change in attitudes and feelings related to stuttering and speaking.
