For the diagnosis and differentiation of chronic cholesistoxolangiitis from the diseases of the biliary system, 20 patients used modern instrumental and laboratory methods. In these methods, excision cholesistorentgenography, retrograde cholangiography, high frequency ultrasound examination, and cholesterol in the blood, manometric tonometry. In the objective examination, patients were disturbed by the pain on the right side of the neck, on the right side of the breast and in the right rib. Dyspeptic symptoms such as lower appetite, nausea, vomiting, and meteorism have been discovered. Dietotherapy, spasmolitics, antibiotics, gallbladder and physiotherapy have also been used in the treatment. Thus, based on the results of our examination, it is recommended that they be used in early diagnosis of chronic cholesistoxolangitis. OBJECTIVE: To determine the frequency of colon cancer, primary hyperparathyroidism, thyroid tumor, and skin cancer in all acromegalic patients in follow-up at the Clinics Hospital - Botucatu Medical School, from 2005 to 2011. SUBJECTS AND METHODS: These patients were evaluated retrospectively for colon cancer, primary hyperparathyroidism, dermatological, and thyroid tumors. RESULTS: Of 29 patients included at the beginning of the study, two were excluded. Among 19 patients submitted to colonoscopy, one presented colon adenocarcinoma (5%). Thyroid nodules were present in 63% of patients, and papilliferous carcinoma was confirmed in two patients (7,7%). Four patients were confirmed as having primary hyperparathyroidism (15%). The most common dermatologic lesions were thickened skin (100%), acrochordons (64%), epidermal cysts (50%), and pseudo-acanthosis nigricans (50%). Only one patient presented basal cell carcinoma. CONCLUSION: Although a small number of acromegalic patients was studied, our findings confirm the high frequency of thyroid neoplasias and primary hyperparathyroidism in this group of patients.

Abstract Background: To compare the necessity between Fine-needle aspiration (FNA) biopsy and ultrasound examination in the diagnosis of different sizes of the thyroid nodules. Does the FNA biopsy have to do it all? Methods: A retrospective analysis was performed to 8352 thyroid patients who underwent thyroid operations between 2011 and 2016 in our hospital. Results: In FNA(+) group, the nodule was more smaller, the increment speed of the amount of operation increased more faster. In no FNA group, the increment speed decreased not obvious in nodules ≥10mm sub-group, but tremendous in both nodules 5mm-10mm sub-group and nodules ≤ 5mm sub-group no matter the nodules were malignant or benign. Over the six years, the total operation number increased, but operation of patients with nodules ≥10mm decreased slightly and operation of patients with nodules <10mm increased markedly especially in nodules ≤ 5mm sub-group. In no FNA group, to compare the malignancy or benign tumor after surgery between nodules 5mm-10mm sub-group and nodules ≥10mm sub-group, χ2=12.000,P=0.001, and between nodules ≤ 5mm sub-group and nodules ≥10mm sub-group, χ2=7.968,P=0.005, but between nodules 5mm-10mm sub-group and nodules ≤ 5mm sub-group, χ2=0.669,P=0.414. Further pairwise comparison showed, in nodules 5mm-10mm sub-group and nodules ≤ 5mm sub-group, the probability of benign tumor was greater than nodules ≥10mm sub-group. Conclusions: In thyroid nodules ≥10mm sub-group, there is no statistical difference between ultrasound diagnosis and biopsy. In nodules <10mm sub-groups, FNA biopsy has the great significance in the diagnosis to add more references for the subsequent treatment.

There is a rise in the incidence of thyroid nodules in pediatric patients. Most of them are benign tissues, but part of them can cause papillary thyroid cancer (PTC). The aim of this study was to detect the mutations in commonly investigated genes as well as in novel PTC-causing genes in thyroid nodules and to correlate the found mutations with clinical and pathological data. The cohort of 113 pediatric samples consisted of 30 benign lesions and 83 PTCs. DNA from samples was used for next-generation sequencing to identify mutations in the following genes: HRAS, KRAS, NRAS, BRAF, IDH1, CHEK2, PPM1D, EIF1AX, EZH1 and for capillary sequencing in case of the TERT promoter. RNA was used for real-time PCR to detect RET/PTC1 and RET/PTC3 rearrangements. Total detection rate of mutations was 5/30 in benign tissues and 35/83 in PTCs. Mutations in RAS genes (HRAS G13R, KRAS G12D, KRAS Q61R, NRAS Q61R) were detected in benign lesions and HRAS Q61R and NRAS Q61K mutations in PTCs. The RET/PTC rearrangement was identified in 18/83 of PTCs and was significantly associated with higher frequency of local and distant metastases. The BRAF V600E mutation was identified in 15/83 of PTCs and significantly correlated with higher age of patients and classical variant of PTC. Germline variants in the genes IDH1, CHEK2 and PPM1D were found. In conclusion, RET/PTC rearrangements and BRAF mutations were associated with different clinical and histopathological features of pediatric PTC. RAS mutations were detected with high frequency in patients with benign nodules; thus, our results suggest that these patients should be followed up intensively.

In order to investigate the characteristics of high-frequency ultrasound for the patients with thyroid nodules (TN), and we use the Thyroid Imaging Reporting And Data System (TI-RADS) to classify all the TN. Combined TIRADS classification with the features of color Doppler ultrasound and Doppler blood flow parameters for each nodule, we tried to diagnose TN between benign and malignant nodules so as to provide clinical physicians a more appropriate method and decision-making strategy for patients with of TN by ultrasound findings. The pulse Doppler technique was used to measure the peak systolic velocity (PSV) and resistance index (RI) of the blood flow within the nodule or closely around. The final pathology results were compared with all the ultrasound test. The diagnostic coincidence rates for B&M-TN were statistically compared by single two-dimensional ultrasound or by single color Doppler flow imaging or by combined technique of both. It can be seen that the combination strategy can optimize the coincidence rate of clinical diagnosis of B&M-TN. Therefore, the differential diagnosis of TN by high-frequency ultrasound TI-RADS classification combined with color Doppler blood flow parameters deserves our attention and further study.

The purpose of this study is to explore whether shear wave elastography has diagnostic value in distinguishing benign and malignant thyroid imaging report and data system (TI-RADS) nodules, so as to provide more accurate information for clinical diagnosis and treatment of thyroid nodules. In this study, 134 patients with thyroid nodules who underwent ultrasound examination from February 2018 to October 2018 are collected. Firstly, according to the diagnostic criteria proposed by Horvath, TI-RADS grading is performed on them, and 78 patients with TI-RADS4 single solid nodules are screened out. Secondly, conventional ultrasound examination is performed on all the researchers, and shear wave elastographies are obtained 1 week after the examination. Thirdly, parameters of each lesion are measured by ultrasound physicians, and the maximum, minimum and average values of Young's modulus of each lesion are recorded. The results show that the mean and maximum of Young's modulus of benign lesions are (26.31 ± 9.88) kPa and (51.36 ± 14.51) kPa, respectively, the mean and maximum of Young's modulus of malignant lesions are (48.36 ± 16.53) and (69.15 ± 19.98) kPa, respectively, and the area under the ROC curve of Emean and Emax is 0.852 and 0.748, respectively. Therefore, shear wave elastography is a new type of ultrasound elastography, which has the advantages of objectivity and little influence from operators without artificial pressure. It has high sensitivity, specificity and accuracy in diagnosing benign and malignant thyroid nodules. Among them, the average value of Young's modulus has the greatest diagnostic value.
