What will it take to end AIDS in Asia and the Pacific region by 2030?

The 2016 global commitments towards ending the AIDS epidemic by 2030 require the Asia–Pacific region to reach the Fast-Track targets by 2020. Despite early successes, the region is well short of meeting these targets. The overall stalled progress in the HIV response has been further undermined by rising new infections among young key populations and the unprecedented COVID-19 pandemic. This paper examines the HIV situation, assesses the gaps, and analyses what it would take the region to end AIDS by 2030. Political will and commitments for ending AIDS must be reaffirmed and reinforced. Focused regional strategic direction that answers the specific regional context and guides countries to respond to their specific needs must be put in place. The region must harness the power of innovative tools and technology in both prevention and treatment. Community activism and meaningful community engagement across the spectrum of HIV response must be ensured. Punitive laws, stigma, and discrimination that deter key populations and people living with HIV from accessing health services must be effectively tackled. The people-centred public health approach must be fully integrated into national universal health coverage while ensuring domestic resources are available for community-led service delivery. The region must utilise its full potential and draw upon lessons that have been learnt to address common challenges of the HIV and COVID-19 pandemics and achieve the goal of ending AIDS by 2030, in fulfillment of the United Nations’ Sustainable Development Goals.

The Asia–Pacific region is home to nearly 6 million people living with HIV. Across the region, key populations – men who have sex with men, transgender women, people who inject drugs, sex workers, prisoners – and their sexual partners make up the majority of those living with HIV. While significant progress has been made in the past 5 years towards UNAIDS’s 90–90–90 goals (90% of people with HIV diagnosed, 90% on antiretroviral therapy, 90% virologically suppressed), significant gaps remain. The papers in this Special Issue address important questions: are we on track to end the AIDS epidemic in the Asia–Pacific region? And can countries in this region reach the new UNAIDS targets for 2030?

Certain of the dynamism and resilience of the region, we underline our resolve to achieve sustainable growth and equitable development and to unlock the full potential of the people who live here. We agree that the prospects for economic growth in the region are strong, and that Asia-Pacific will continue to play a leading role in the global economy. The goals we have set, including the achievement of free and open trade and investment in the region by the dates set out in the Bogor Declaration, are ambitious and unequivocal.

A focus on crisis is a mainstay of the literature on contemporary populism. However, the links between populism and crisis remain under-theorized and undeveloped. This article puts forward a novel perspective for understanding this relationship, arguing that crisis does not just trigger populism, but that populism also attempts to act as a trigger for crisis. This is because crises are always mediated and ‘performed’. The article presents a six-step model of how populist actors ‘perform’ crisis, drawing on empirical examples from Europe, Latin America, North America and the Asia-Pacific region. It explains how the performance of crisis allows populist actors to pit ‘the people’ against a dangerous other, radically simplify the terrain of political debate and advocate strong leadership. It ultimately suggests that we should move from thinking of crisis as something purely external to populism, towards thinking about the performance of crisis as an internal core feature of populism.

The coconut industry is undeniably an important feature in the economies of the Asia‑Pacific region. It is an important source of income and food. In Vanuatu, this industry contributes 41% to its total export earnings; 7.10% in the Philippines; and 8.91% in the Solomon Islands in 1992. This industry also provides sustenance to the people of Indonesia, Malaysia, India and Sri Lanka. Coconuts are major ingredients in many food preparations in these countries.

Over the last 25 years, the global area of certified forests has grown rapidly and voluntary forest certification has become recognized as an effective tool to engage international markets in improving sustainability within forest management units. However, the bulk of this growth has occurred in North America, Northern Europe, Australia, and New Zealand, with relatively limited uptake in the tropics. Since its creation, forest certification has been largely understood as a “market-based” mechanism, in contrast to government-led policies and regulations. Through the experience of the Responsible Asia Forestry and Trade (RAFT) partnership in the Asia Pacific region, we find that the framing of forest certification as voluntary and market-based, and as a mechanism to overcome governance failure, has created an artificial dichotomy. In this dichotomy, voluntary certification and regulatory measures to promote sustainable forest management are conceived of and pursued largely independently. We argue that it is more constructive to view them as complementary approaches that share a common goal of increasing sustainability across the forestry sector. In practice, forest certification interacts with conventional governance institutions and mechanisms. Understanding these interactions and their implications, as well as additional possibilities for interaction, will help in realizing the full potential of forest certification.

The publication of information is essential for expanding the benefits of public health throughout the Asia-Pacific region. All academics are familiar with the adage “Publish or perish,” but for public health, this should be rewritten as “Publish or the population perishes.” Publication and communication in public health continues to undergo rapid change. There are many challenges that a regional journal faces. The number of printed journals is beginning to decline. Digital publishing and extensive databases and search engines have become the only practical way to access data. But the advent of digital publishing has seen the arrival of profit-making entrepreneurs. Whether a Journal is in the printed form or digital, it remains the key communication in public health and ultimately improve the health of the people. The Asia-Pacific Journal of Public Health (APJPH), which is owned by Asia Pacific Academic Consortium for Public Health ( APACPH) and published by Sage ensures that your data gets maximum digital exposure in the region where it can make a difference. Over the years, APJPH has increased its impact factor and gained in prestige. Although impact factor of a journal is important in documenting the recognition of work by academics, it does not necessarily reflect importance to our region. There needs to be a re-evaluation of publication and communication strategies so that all citizens of the Asia-Pacific region benefit.

Health system planners in low- and middle-income countries (LMIC) of the Asia-Pacific region seeking to reorient primary health care (PHC) systems to achieve universal health coverage may be hindered by lack of knowledge of what works in their setting. With limited resources for research available, it is important to identify evidence-based strategies for reorganising PHC delivery, determine where relevant evidence gaps exist and prioritise these for future study. This paper describes an approach for doing this using the best available evidence combined with consultation to establish evidence priorities. We first reviewed PHC organisational interventions in Asia-Pacific LMICs and ascertained evidence gaps. The largest gaps related to interventions to promote access to essential medicines, patient management tools, effective health promotion strategies and service planning and accountability. Evidence from Pacific Island countries was particularly scant. We then engaged an expert panel of 22 PHC stakeholders from seven Asia-Pacific LMICs in a Delphi exercise to identify priority questions for future research. Research priorities were: (1) identifying effective PHC service delivery models for chronic diseases; (2) devising sustainable models of disease integration; (3) optimising task shifting; (4) understanding barriers to care continuity; (5) projecting future PHC needs; and (6) designing appropriate PHC service packages. Notably, stakeholder-determined priorities reflected large, context-dependent system issues, while evidence gaps centred on discrete interventions. Future research on the organisation of PHC services in Asia-Pacific LMICs should incorporate codesign principles to engage researchers and national PHC system stakeholders, and innovative methods that build on existing evidence and account for system complexity.
