Understanding evacuation preferences and wildfire mitigations among Northwest Montana residents

There is currently insufficient information in the United States about residents’ planned evacuation actions during wildfire events, including any intent to remain at or near home during fire events. This is incompatible with growing evidence that select populations at risk from wildfire are considering alternatives to evacuation. This study explores the evacuation preferences of wildland–urban interface residents in Flathead County, Montana, USA. We compare the performance of wildfire mitigation and fuel reduction actions across groups of residents with different primary evacuation preferences. We also explore what factors (e.g. actions, demographics, attitudes towards government, risk perceptions) help explain residents’ preferences for evacuation. Results suggest that relatively high proportions of residents are interested in staying and defending their homes, with smaller proportions favouring evacuation or passively sheltering in their homes during wildfire. Vegetation management behaviour differs significantly among residents with different evacuation preferences, including significantly higher rates of forest thinning among those intending to remain at home and actively defend their residence. Other results suggest that sex, part-time residency, income and attitudes towards loss from fire are statistically associated with differences in evacuation preferences.

AbstractIntroduction:On 18 July 2001, a train hauling hazardous materials, including hydrochloric acid, hydrofluoric acid, and acetic acid, derailed in the city of Baltimore, Maryland, resulting in a fire that burned under a downtown street for five days. Firefighters were stymied in their efforts to extinguish the fire, and the city was subjected to thick smoke for several days.Objectives:To determine whether an urban chemical fire with a hazardous materials spill resulted in a detectable public health impact, and to demo-graphically describe the at-risk population for potential smoke and chemical exposure.Methods:The United States Centers for Disease Control and Prevention (CDC) was consulted about possible side effects from chemical exposure. Total numbers of emergency department (ED) patients and admissions from 15:00 hours (h), 15 July 2001 to 15:00 h, 21 July 2001 were collected from five local hospitals. Patient encounters citing specified chief complaints from 15:00 h, 15 July to 15:00 h, 18 July (pre-accident) were compared with the period from 15:00 h, July 18 to 15:00 h, 21 July (post-accident). Data were analyzed using Fisher's exact test. The United States Census Bureau's Topologically Integrated Geographic Encoding and Referencing (TIGER) digital database of geographic features and ArcView Geographic Information Systems (GIS) were used to create maps of Baltimore and to identify populations at-risk using attribute census data. Results: There were 62,808 people residing in the immediate, affected area. The mean of the values for age was 33.7 ±3.2 years (standard deviation; range = 16 yrs) with 49% (30,927) males and 51% (31,881) females. A total of 2,922 ED patient encounters were screened. Chief complaints included shortness of breath, pre-event = 109 vs. post-event = 148; chest complaints = 90 vs. 113; burns and/or skin irritation = 45 vs. 42; eye irritation 26 vs. 34; throat irritation = 33 vs. 27; and smoke exposure = 0 vs. 15. There was a statistically significant increase (p <0.05) for shortness of breath and smoke exposure-related complaints. No statistically significant increase in numbers of admitted patients with these complaints was found.Conclusions:In the setting of a large-scale urban chemical fire, local EDs can expect a significant increase in the number of patients presenting to EDs with shortness of breath and/or smoke inhalation. Most do not require inpatient hospitalization. Careful assessment of impact on local EDs should be considered in future city-accident planning. Some official warnings were widely misinterpreted or ignored. Public education on potential hazards and disaster preparedness targeted to populations at-risk should receive a high priority. Geographic information systems (GIS) may serve as useful tools for identifying demographics of populations at-risk for disaster planning and responses.

The Debate Question 1: Various commentators have frequently invoked the importance of moderate Muslims and the role that they can play in fighting extremism in the Muslim world. But it is not clear who is a moderate Muslim. The recent cancellation of Tariq Ramadan’s visa to the United States, the raids on several American Muslim organizations, and the near marginalization of mainstream American Muslims in North America pose the following question: If moderate Muslims are critical to an American victory in the war on terror, then why does the American government frequently take steps that undermine moderate Muslims? Perhaps there is a lack of clarity about who the moderate Muslims are. In your view, who are these moderate Muslims and what are their beliefs and politics? AUJ: The promotion of “moderate” Muslims is part of an extremist tendency sweeping the United States, unlike the situation in the Muslim world. It is the result of a war between two Americas: the America of ideals (e.g., of equality and justice) and the America of extremism, which has succumbed to self-interest groups and individuals. For the America of ideals, the Tariq Ramadan episode is a dark spot, one among many such episodes in recent times. Periodic episodes of tragedy are the hallmark of the America that has shifted its priorities under the pressure and manipulation of the extremists. These forces use all expedient means to sacrifice the wellbeing of the United States for self-interest and promotion of the Zionist state. This extremism entails a morbid dread of Islam. It never regards any Muslim as moderate unless one publicly rejects the Qur’an as “the final manifesto of God,”1 considering this belief a “disturbing cornerstone of Islam,”2 and submitting to the rejection of key parts of the Qur’an.3 Unquestioning support for Israel, along with all other American-approved dictatorships, is the minimum criterion.4 All other factors are irrelevant. The fascistic American track record of accepting “moderates” and rejecting “radicals” is clear.5 The final distinction is not defined by their adherence to Islam, but by the assumed threat they pose to the interests of these extremists. For example, a devout man, fervent in all of his personal rituals but not participating in political affairs, would be a “moderate,” whereas a marginally practicing Muslim with the zeal to voice his opposition to the injustice perpetrated by the extremists’America is classified as a “radical.” In the current political context, a moderate is one who is passive like the devout man, or active like the extremist “moderates” – the Muslim neomods – who openly promote the extremist agenda using Islamic interpretations or “Project Ijthihad”6 as a cover. Hence, the distinction is not academic or religious, but political. Two opposing factors prove this point. First, there are clear commands for Muslims to be moderate by default.7 Moderateness is a prerequisite for all Muslims, not a label of identity for some. Accordingly, Muslims cannot be part-time or partial Muslims (Qur’an 2:208) or reject part of the Qur’an (Qur’an 2:85).8 Hence, such religious labelling is irrelevant. Second, the extremists insist that strong belief in the totality of the Qur’an makes Muslims “Islamists.”9 That is why they believe themselves to be “absolutely at war with the vision of life that is prescribed to all Muslims in the Koran.”10 It means that the standards of “moderateness,” as set by the American extremists, are directed at neutralizing a preconceived threat. Under these circumstances, mere claims of being a “moderate” do not make any difference at all, as long as a Muslim is presented as a threat, however baseless, to the interests of extremist America. Similarly, the so-called extremism in the Muslim world is not the result of Muslims’faith. Rather, it is a function of the perpetually colonized and oppressed people due to the lack of true independence and a central authority to control and productively channel their energies. It is naïve to suggest that a few ill-informed “moderate” individuals or puppet regimes can emulate the abilities of an entire central authority (i.e., the Islamic state) and effect progress and positive meaningful change.

Abstract Food insecurity is an under-recognized geriatric syndrome that has extensive implications in the overall health and well-being of older adults. Understanding the impact of food insecurity in older adults is a first step in identifying at-risk populations and provides a framework for potential interventions in both hospital and community-based settings. This symposium will provide an overview of current prevalence rates of food insecurity using large population-based datasets. We will present a summary indicator that expands measurement to include the functional and social support limitations (e.g., community disability, social isolation, frailty, and being homebound), which disproportionately impact older adults, and in turn their rate and experience of food insecurity and inadequate food access. We will illustrate using an example of at-risk seniors the association between sarcopenia, the age-related loss of muscle mass and function, with rates of food security in the United States. The translational aspect of the symposium will then focus on identification of psychosocial and environmental risk factors including food insecurity in older veterans preparing for surgery within the Veterans Affairs Perioperative Optimization of Senior Health clinic. Gaining insights into the importance of food insecurity will lay the foundation for an intervention for food insecurity in the deep south. Our discussant will provide an overview of the implications of these results from a public health standpoint. By highlighting the importance of food insecurity, such data can potentially become a framework to allow policy makers to expand nutritional programs as a line of defense against hunger in this high-risk population.

The United States is abrogating the Anti-Ballistic Missile Treaty in order to deploy a limited missile defence shield. Amongst other developments, this is prompting a reconsideration of the global security framework. However, a crucial element is missing from the current missile defence proposals: a clearly articulated concept of peaceful use, applicable both to outer space and to earth-space. The deployment of missile defence runs counter to emerging norms. It has effects going far beyond the abandonment or re-configuration of specific Cold War agreements. In a community of nations committed to the maintenance of international peace and security (cf. national or plurilateral security), sustainable meaning for widely used and accepted norms of peaceful use and peaceful purposes is at risk.

More than one-third of young people in the United States are either obese or at risk of becoming obese. The authors consider how food marketers have contributed to this problem and how they might help resolve it. The article organizes the marketing activities of food-related companies around the classic four Ps. The authors first discuss product, price, and promotion in terms of past, present, and potential future industry actions. They then discuss place as a function of four key commercial end points in the food channel: (1) supermarkets, (2) convenience stores, (3) restaurants, and (4) schools. The authors consider government actions in terms of how they affect the actions of both the food industry and consumers. Throughout the article, the authors consider how extant research can be extended in an effort to better understand and address the youth obesity problem.

Objective With the completion of the Human Genome Project and swift development of genomic technologies, public health practitioners can use these advancements to more precisely target disease interventions to populations at risk. To integrate these innovations into better health outcomes, public health professionals need to have at least a basic understanding of genomics within various disciplines of public health. This descriptive study focused on the current level of genomics content in accredited master of public health (MPH) programs in the United States. Methods We conducted an internet search on all 171 Council on Education for Public Health (CEPH)–accredited MPH programs in the United States for genomics content in required and elective courses using the search terms “genetics,” “genomics,” and “molecular.” Results Of the 171 CEPH-accredited MPH programs examined, 52 (30.4%) schools and programs in 34 states offered some type of genomics education. Thirty-five (20.5%) schools and programs had a course in genetic epidemiology, 29 (16.9%) had a course in genetic biostatistics or bioinformatics, and 17 (9.9%) had a course in general public health genomics. The remaining 119 offered no course with a focus on genetics or genomics. In addition, some electives or specifically focused courses related to genomics were offered. Conclusion We found inadequate training in public health genomics for MPH students. To realize the promise of precision public health and to increase the understanding of genomics among the public health workforce, MPH programs need to find ways to integrate genomics education into their curricula.
