Comparison of four procedures for separating apolipoprotein A- and apolipoprotein B-containing lipoproteins in plasma.

Abstract Because lipoproteins containing apolipoprotein A (ApoA-I + ApoA-II) or apolipoprotein B (ApoB) seem to exert opposite effects as risk factors for coronary heart disease, we decided to determine the separability of these two major plasma lipoproteins by procedures originally designed to separate high-density from low- and very-low-density lipoproteins. The presumably ApoB-free lipoproteins isolated from normal plasma by (a) ultracentrifugation at d = 1.063; precipitation with (b) heparin-Mn2+ or (c) phosphotungstate-Mg2+; or (d) immunoprecipitation with antibodies to ApoB were characterized by quantifying cholesterol and apolipoproteins A-I, A-II, B, C-II, C-III, D, E, F, and Lp(a). ApoA- and ApoB-containing lipoproteins were completely separated only by immunoprecipitation with antibodies to ApoB. The ApoB-containing lipoproteins isolated by other procedures always contained 4% to 20% of total plasma ApoA-I and differed substantially from one another with respect to the content of some of the minor apolipoproteins. Measuring apolipoproteins was more reliable than measuring cholesterol for monitoring this separation and for expressing the concentrations of ApoA- and ApoB-containing lipoproteins.

In the total fraction of low and very low density lipoproteins (LDL+VLDL) isolated from serum by precipitation in the presence of heparin-Mn the copper-induced lipid peroxidation was accompanied by accumulation of LPO products, a decrease ANS fluorescence intensity (FANS) and an increase in probe - cation DSP-6, a fluorescence intensity decrease of intrinsic in the ultraviolet area (Fuv) and an increase in the visible area (Fvis). The degree of lipoprotein modification was estimated by calculating the Fvis / Fuv and FDSP-6 / FANS ratio. Strong positive correlation was found between these ratios and concentration of thiobarbituric acid-reactive substances (TBARS) of LDL+VLDL samples isolated from sera of 49 donors and incubated at 37°С in the presence of 50 M CuSO4 during 0, 3 and 24 hr (Fvis / Fuv (r=0,75; p<0,001) and FDSP-6 / FANS (r=0,73; p<0,001)). Very strong positive correlation was also found between both fluorescent parameters Fvis / Fuv and FDSP-6 / FANS (r=0,95, p<0,001). Changes in the values of Fvis / Fuv, FDSP-6 / FANS, concentration of TBARS in 75 patients with documented coronary heart disease (CHD) and 49 apparently healthy donors were studied. No significant differences of these parameters in LDL+VLDL of patients with CHD and donors were found.

For the treatment of familial hypercholesterolemia, Liposorber LA-40 was clinically applied. The Liposorber is a commercially developed affinity adsorbent for plasma perfusion which selectivily adsorbs low density lipoproteins and very low density lipoproteins and is specially designed for plasmapheretic treatment of hypercholesterolemia. The Liposorber column, containing activated cellulose beads having an affinity for liporpotein containing apolipoprotein-B, has an excellent adsorption capacity, excellent selectivity, minimum albumin loss. This new apheresis system was applied to 2 clinical cases. After seven months of trial perfusion every 2 weeks, patient condition was good, with a level of total cholesterol under 300 mg/dl. No replacement fluids were given during or after treatment. In this paper, clinical results of these patients were shown and the mechanism of adsorption of this specific adsorbent was discussed.
