Abstract Background Quality indicators (QI) are mandatory in French hospitals. After a decade of use, the Ministry of Health set up an expert workgroup to enhance informed decision-making regarding currently used national QI, i.e. to propose a decision of withdrawing, revising or continuing their use. We report the development of an integrated method for a comprehensive appraisal of quality/safety indicators (QI) during their life cycle, for three purposes, quality improvement, public disclosure and regulation purposes. The method was tested on 10 national QI on use for up to 10 years to identify operational issues. Methods A modified Delphi technique to select relevant criteria and a development of a mixed evaluation method by the workgroup. A ‘real-life’ test on 10 national QI. Results Twelve criteria were selected for the appraisal of QI used for regulation goals, 11 were selected for hospital improvement and seven for public disclosure. The perceived feasibility and relevance were studied including hospital workers, patients and health authorities professionals; the scientific soundness of the indicator development phase was reviewed by analyzing reference documents; the metrological performance (limited to the discriminatory power and dynamics of change during the life cycle dimensions) was analyzed on the national datasets. Applied to the 10 QI, the workgroup proposed to withdraw four of them and to modify or suspend the six others. Conclusions The value of the method was supported by the clear-cut conclusions and endorsement of the proposed decisions by the health authorities.

AbstractIn this paper, a CAD integrated method is proposed for automatic recognition of potential weld locations in large assembly structures predominantly comprised of weld joints. The intention is to reduce the total man-hours spent on manually locating, assigning, and maintaining weld-related information throughout the product life cycle. The method utilizes spatial analysis of extracted stereolithographic data in combination with available CAD functions to determine whether the accessibility surrounding a given intersection edge is sufficient for welding. To demonstrate the method, a system is developed in Siemens NX using their NXOpen Python API. The paper presents the application of the method to real-life use cases in varying complexity in cooperation with industrial partners. The system is able to correctly recognize almost all weld lines for the parts considered within a few minutes. Some exceptions are known for particular intersection lines located deep within notched joints and geometries weldable through sequential assembly, which are left as a subject to further works.

The analysis of ventilation strategies is fundamentally affected by regional climate conditions and local cost databases, in terms of energy consumption, CO2 emission and cost-effective analysis. A systematic approach is covered in this paper to estimate a local economic and environmental impact on a medium-sized space located in two regions during supply-and-installation and operation phases. Three ventilation strategies, including mixing ventilation (MV), displacement ventilation (DV) and stratum ventilation (SV) were applied to medium-sized air-conditioned space with this approach. The trend of the results for three ventilation systems in the life cycle assessment (LCA) and life cycle cost (LCC) analysis is SV < DV < MV. The result of CO2 emission and regional LCC shows that SV is the lowest one in both regional studies. In comparison with the Hong Kong Special Administrative Region (HKSAR) during 20 Service years, the case analysis demonstrates that the percentage differences in LCC analysis of MV, DV & SV in Guangdong are less than 20.5%, 19.4% and 18.82% respectively. Their CO2 emission of MV, DV and SV in Guangdong are more than HKSAR in 10.69%, 11.22% and 12.05%, respectively. The present study could provide information about regional effects in the LCA and LCC analysis of three ventilation strategies emissions, and thereby help set up models for decision-making on high efficiency and cost-effective ventilation strategy plans.

A working party was set up by the Scottish Home and Health Department to consider the system of allocation of money to health boards. The system advocated (SHARE Report) is predictable from the NHS structure and the analogous report (RAWP) in England. The Secretary of State has announced his decision to accept SHARE in general principle. In principle, the Report is to be welcomed as a first step toward rationalising the distribution of health service money. However, work already published suggests the assumptions in SHARE require examination. SHARE would encourage individual health boards to plan for their own populations although thereby the greatest improvement in health services in Scotland may not be achieved. The SHARE objective of equal opportunity of securing access is open to various interpretations. Emphasis is laid on the recommendation in the Report for a comprehensive examination of the inter-relationship of social circumstances (in particular, urban deprivation), morbidity and mortality.

Since the introduction of open innovation (OI), both firms and academics have widely acknowledged the potential of unlocking large firms’ innovation potential through interactions with external parties, such as young ventures. These asymmetric partnerships are prone to several problems related to communication, roles and responsibilities, cultural differences, and operational issues, for which solutions and best practices have been proposed. However, all these solutions focus on the partnership itself; hence, on the “Get & Manage (GM)” stages. Unfortunately, the processes leading to a partnership; i.e., the “Want & Find (WF)” stages before the partnership, have largely been overlooked. The central thesis of this manuscript is that solutions that are implemented in the early “WF” stages have a positive impact on the outcomes of an asymmetric large firm — young venture partnership. We will show that attention to set-up and communication efforts in these early stages is needed, and discuss how our detailed explanations of such fruitful solutions contribute to the extant literature on asymmetric OI collaborations.

This paper describes efforts taken to further transition life cycle analysis techniques from the latter, more detailed phases of design, to the early-on conceptual phase of product development. By using modern design methodologies such as automated concept generation and an archive of product design knowledge, known as the Design Repository, virtual concepts are created and specified. Streamlined life cycle analysis techniques are then used to determine the environmental impacts of the virtual concepts. As a means to benchmark the virtual results, analogous real-life products that have functional and component similarities are identified. The identified products are then scrutinized to determine their material composition and manufacturing attributes in order to perform an additional round of life cycle analysis for the actual products. The results of this research show that enough information exists within the conceptual phase of design (utilizing the Design Repository) to reasonably predict the relative environmental impacts of actual products based on virtual concepts.

PurposeBandim Health Project (BHP) monitors health and survival of women and children in a nationally representative rural Health and Demographic Surveillance System (HDSS) in Guinea-Bissau. The HDSS was set up in 1989–1990 to collect data on health interventions and child mortality.ParticipantsThe HDSS covers 182 randomly selected clusters across the whole country. The cohort is open, and women and children enter the cohort, when they move into the selected clusters, and leave the cohort, when they move out or die, or when children reach 5 years of age. Data are collected through biannual or more frequent household visits. At all village visits, information on pregnancies, vital status, vaccination status, arm circumference, use of bed nets and other basic information is collected for women and children. Today, more than 25 000 women and 23 000 children below the age of 5 years are under surveillance.Findings to dateResearch from the BHP has given rise to the hypothesis that vaccines, in addition to their targeted effects, have important non-specific effects altering the susceptibility to other infections. Initially, it was observed that mortality among children vaccinated with the live BCG or measles vaccines was much lower than the mortality among unvaccinated children, a difference, which could not be explained by prevention of tuberculosis and measles infections. In contrast, mortality tended to be higher for children who had received the non-live Diphtheria-Tetanus-Pertussis vaccine compared with children who had not received this vaccine. Since the effect differed for the different vaccines, no bias explained the contrasting findings.Future plansNew health interventions are introduced with little assessment of real-life effects. Through the HDSS, we can describe both the implementation of interventions (eg, the vaccination programme) and their effects. Furthermore, the intensive follow-up allows the implementation of randomised trials testing potential better vaccination programmes.

Complex financial conversion projects with large budgets have many different challenges. For companies that want to survive in conditions of tough competition, legacy (old) systems must continue to provide the required service throughout the project life cycle and in some circumstances even after project completion partly. In this case, the term coexistence comes into prominence. During this period, testing phase takes more critical role while integration systems' complexity and risk amount increase. Determining testing approach to use is essential to make sure both transformed and legacy systems provide service synchronously. In this paper, testing practices applied in the long conversion processes are discussed. Primarily, the basic features of the critical financial systems are addressed and then the main adoption methods in the literature are summarized. Then a variety of testing methodologies are presented depending on those adoption methods. These samples based on real-life experiences of transformation project. The most extensive example of real-time online financial systems is core banking systems. This paper covers the testing life cycle process of the large scale project of core banking system transformation project of a bank in Turkey.
