Abstract Background Tibia vara (Blount’s disease) is an orthopedic condition that affects young children causing varus deformity of the knee. Blount described tibia vara as “an osteochondrosis located at the medial side of the proximal tibial epiphysis.” Currently, tibia vara is considered an acquired disease of the proximal tibial metaphysis, However, rather than an epiphyseal dysplasia or osteochondrosis. The exact cause is unknown, but enchondral ossification seems to be altered. Suggested causative factors include infection, trauma, osteonecrosis and a latent form of rickets, although none of these has been proved. Combination of hereditary and developmental factors is the most likely cause. Weight bearing must be necessary for its development because it does not occur in non-ambulatory patients and the relationship of early walking and obesity to Blount disease has been clearly documented. Purpose to establish, through the literatures, what is the effectiveness of temporary hemiepiphysiodesis by eight plate in treating Blount’s disease. Methodology A systematic review was carried out on several studies which met the inclusion criteria. In this Study we conducted an up-to-date review of the studies discussing results of use of eight plates in temporary hemiepiphysiodesis in Blount disease and published between June 2008 and June 2018. Results in our study, success rate is 67.1% with a considerable rate of correction of mechanical axis deviation ranging between 0.58 mm\month to 1.5mm\month. Success rate in infantile age group was 95%, while in adolescent age group was 48%. Conclusion it is evident that results of 8-plate in Blount disease is much better in infantile group of age than adolescent group. So, 8-plate can be used in treatment of Blount’s disease up to stage IV. The results are better in low BMI and younger age group .

<p class="abstract">Blount’s disease is more common in Afro-Caribbean, obese, female child. Early walking have been associated with the disease process. Adult patients with Blount’s disease who received no treatment previously, are rarely reported across the other parts of the world and hence less discussed. The treatment of neglected cases of Blount’s disease often requires correction of varus deformity, internal tibial torsion and medial plateau depression. We are reporting one such patient seen at our hospital. A skeletally mature, obese (BMI 43.2), male patient with bilateral tibia vara was operated for hemi-plateau elevation combined with a valgus osteotomy. Fixation was achieved with plate and staples. Surgical correction of one such patient is technically demanding and more complicated. If left untreated , the main risk is premature osteoarthritis.</p>

Background:Dual energy CT (DECT) has diagnostic potential in gout patients. DECT can automatically colour-code presumed urate deposits based on radiodensity (Hounsfield Units, HU) and DECT ratio (difference in attenuation between high and low kV series) of lesions. However, other materials may imitate properties of urate deposits, most importantly calcium-containing material, dense tendons and artefacts, which may lead to misinterpretations. The characteristics of DECT lesions in gout patients have not yet been systematically investigated.Objectives:To evaluate the properties and locations of colour-coded DECT lesions in gout patients.Methods:DECT were performed in patients with suspected gout. Patients were separated into gout and non-gout patients based on joint fluid microscopy findings. DECT of the hands, knees and feet were performed using default gout settings and colour-coded lesions were registered. Only location-relevant lesions were analysed (e.g. nail bed artefacts excluded). Mean density (mean of HU at 80 kV and Sn150 kV), mean DECT ratio, size and location of each lesion was determined.Subgroup analysis was performed post-hoc evaluating potential differences in properties and locations of lesions. Lesions were separated into groups according to properties (Figure 1, grey box): 1)Size—to separate artefacts characterised by small volume (possible artefacts). 2)DECT ratios—to separate calcium-containing material characterised by high DECT ratio (possible calcium-containing material). 3)Density—to separate dense tendons characterised by low DECT ratio and low HU values (possible dense tendons). Lesion fulfilling all urate characteristics (large volume, low DECT ratio, high density) were labelleddefinite urate deposits. Finally, for non-gout patients, properties ofnon-gout urate-imitation lesions(properties asdefinite urate deposits) were analysed.Results:In total, 3918 lesions (all lesions) were registered in gout patients (n=23), with mean DECT ratio 1.06 (SD 0.13), median density 160.6 HU and median size 6 voxels (Figure 1, blue box). Lesions were seen in all analysed joints, most frequently MTP1 joints (medial side), knee joints and midtarsal joints (Figure 2a). Tendon affections were also common, especially in the knee tendons (patella and quadriceps), malleolus-related tendons (e.g. peroneus and tibialis posterior) and the Achilles tendons (Figure 2a).Subgroup analyses showed thatdefinite urate deposits(figure 2b) were found at the same locations asall lesionin gout patients (figure 2a), with the four most common sites being MTP1 joints, midtarsal joints, and quadriceps and patella tendons (Figure 2b).Possible dense tendonlesions had a mean HU value of 156.5 HU—markedly higher than expected for dense tendons (<100HU)—and lesion-locations were similar todefinite urate deposits(data not shown), indicating that they primarily consisted of true urate deposits. In contrast,possible calcium-containing materialandnon-gout urate-imitating lesionshad distinctly different properties (ratios 1.33 and 1.20, respectively) (Figure 1, yellow and orange box). Furthermore, the locations of these lesions were different fromdefinite urate depositssince they were primarily found in a few weight-bearing joints (knee, midtarsal and talocrural including malleolus regions) and tendons (Achilles and quadriceps), whereas no lesions were found in either MTP1 joints or patella tendons (figure 2c).Conclusion:DECT color-coded lesions in gout patients are heterogeneous in properties and locations. Subgroup analyses found that locations such as MTP1 joints and patella tendons were characterised by almost only showingdefinite urate deposits. A pre-procedural ultrasound of the lumbar spine is frequently used to facilitate neuraxial procedures. The aim of this review is to examine the evidence sustaining the utilization of pre-procedural neuraxial ultrasound compared to conventional methods. We perform a systematic review of randomized controlled trials with meta-analyses. We search the electronic databases Medline, Cochrane Central, Science Direct and Scopus up to 1 June 2019. We include trials comparing a pre-procedural lumbar spine ultrasound to a non-ultrasound-assisted method. The primary endpoints are technical failure rate, first-attempt success rate, number of needle redirections and procedure time. We retrieve 32 trials (3439 patients) comparing pre-procedural lumbar ultrasounds to palpations for neuraxial procedures in various clinical settings. Pre-procedural ultrasounds decrease the overall risk of technical failure (Risk Ratio (RR) 0.69 (99% CI, 0.43 to 1.10), p = 0.04) but not in obese and difficult spinal patients (RR 0.53, p = 0.06) and increase the first-attempt success rate (RR 1.5 (99% CI, 1.22 to 1.86), p < 0.0001, NNT = 5). In difficult spines and obese patients, the RR is 1.84 (99% CI, 1.44 to 2.3; p < 0.0001, NNT = 3). The number of needle redirections is lower with pre-procedural ultrasounds (SMD = −0.55 (99% CI, −0.81 to −0.29), p < 0.0001), as is the case in difficult spines and obese patients (SMD = −0.85 (99% CI, −1.08 to −0.61), p < 0.0001). No differences are observed in procedural times. Ιn conclusion, a pre-procedural ultrasound provides significant benefit in terms of technical failure, number of needle redirections and first attempt-success rate. Τhe effect of pre-procedural ultrasound scanning of the lumbar spine is more significant in a subgroup analysis of difficult spines and obese patients.

Abstract Background Coronary stents are routinely placed in the treatment and prophylaxis of coronary artery disease (CAD). Current coronary stent designs are prone to developing blockages: in-stent thrombosis (IST) and in-stent re-stenosis (ISR). This is a systematic review of the design of current coronary stent models, their structural properties and their modes of application, with a focus on their associated risks of IST and ISR. The primary aim of this review is to identify the best stent design features for reducing the risk of IST and ISR. To review the three major types of stents used in clinical settings today, determining best and relevant clinical practice by exploring which types and features of offer improved patient outcomes regarding coronary angioplasty. This information can potentially be used to increase the success rate of coronary angioplasty and stent technology in the future taking into account costs and benefits. Methods Scientific databases were searched to find studies concerning stents. After the exclusion criteria were applied, 19 of the 3192 searched literature were included in this review. Studies investigating three major types of stent design were found: bare-metal stents (BMS), drug-eluting stents (DES) and bioresorbable stents (BRS). The number of participants varied between 14 and 1264. On average 77.4% were male, with a mean age of 64 years. Results From the findings of these studies, it is clear that DES are superior in reducing the risk of ISR when compared to BMS. Conflicting results do not clarify whether BRS are superior to DES at reducing IST occurrence, although studies into newer BRS technologies show reducing events of IST to 0, creating a promising future for BRS showing them to be non-inferior. Thinner stents were shown to reduce IST rates, due to better re-endothelialisation. Scaffold material has also been shown to play a role with cobalt alloy stents reducing the risk of IST. This study found that thinner stents that release drugs were better at preventing re-blockages. Some dissolvable stents might be better at stopping blood clots blocking the arteries when compared to metal stents. The method and procedure of implanting the stent during coronary angioplasty influences success rate of these stents, meaning stent design is not the only significant factor to consider. Conclusions Positive developments in coronary angioplasty could be made by designing new stents that encompass all the most desirable properties of existing stent technology. Further work is needed to investigate the benefits of BRS in reducing the risk of IST compared to DES, as well as to investigate the effects of different scaffold materials on IST and ISR outcomes.

Summary Background Miniscrew-Assisted Rapid Palatal Expansion (MARPE) is a non-surgical treatment for transverse maxillary deficiency. However, there is limited evidence concerning its efficacy. Objectives This systematic review aims to evaluate the efficacy of MARPE in late adolescents and adults by assessing success rate and skeletal and dental transverse maxillary expansion, as well as treatment duration, dental and periodontal side effects and soft tissue effects. Search methods Seven electronic databases were searched (MEDLINE, Embase, Cochrane Library, Web of Science, Scopus, ProQuest and ClinicalTrials.gov) without limitations in November 2020. Selection criteria Randomized and non-randomized clinical trials and observational studies on patients from the age of 16 onwards with transverse maxillary deficiency who were treated with MARPE and which included any of the predefined outcomes. Data collection and analysis Inclusion eligibility screening, data extraction and risk of bias assessment were performed independently in duplicate. When possible, exploratory meta-analyses of mean differences (MDs) with their 95% confidence intervals (CIs) were conducted, followed by the Grading of Recommendations, Assessment, Development and Evaluation (GRADE) analysis of the evidence quality. Results Eight articles were included: two prospective and six retrospective observational studies. One study had a moderate risk of bias, whereas seven studies had a serious risk of bias. GRADE quality of evidence was very low. MARPE showed a high success rate (mean: 92.5%; 95%CI: 88.7%–96.3%), resulting in a significant skeletal width increase (MD: 2.33 mm; 95%CI: 1.63 mm–3.03 mm) and dental intermolar width increase (MD: 6.55 mm; 95%CI: 5.50 mm–7.59 mm). A significant increase in dental tipping, a decrease in mean buccal bone thickness and buccal alveolar height, as well as nasal soft tissue change was present (P &lt; 0.05). The mean duration of expansion ranged from 20 to 126 days. Limitations One of the main drawbacks was the lack of high-quality prospective studies in the literature. Conclusions and implications MARPE is a treatment modality that is associated with a high success rate in skeletal and dental maxillary expansion. MARPE can induce dental and periodontal side effects and affect peri-oral soft tissues. Given the serious risk of bias of the included studies, careful data interpretation is necessary and future research of higher quality is strongly recommended. Registration PROSPERO (CRD42020176618). Abstract Aim This systematic review and meta-analysis aims to assess this relationship and determine the most appropriate age for recommendation of surgery. Method According to the “Preferred Reporting items for systematic review and meta-analysis” (PRISMA) statement, a literature search was performed across Medline, EMBASE and the Cochrane library from 1946-2018. Articles examining a relationship between age and myringoplasty or type 1 tympanoplasty success rates were screened. Results 20 articles encompassing data from 2244 procedures were included. The overall results conveyed a clear correlation between increasing age and rising success rate. A t-test was conducted which demonstrated a significant (P = 0.05) transition at aged 10, whereby success rate below age 10 was 70.6% and above 10 was 86%. Conclusions This systematic review and meta-analysis has uncovered a clear correlation between increasing age and increasing success rate for myringoplasty in the paediatric population. Furthermore, a significant transitional point has been demonstrated at the age of 10 and We hope that knowledge of increased success rates particularly after the age of 10 helps clinicians make more informed decisions about when to operate

INTRODUCTION: Sit-to-stand and stand-to-sit are two of the most mechanically demanding activities undertaken in daily life and which are usually impaired in stroke subjects. OBJECTIVES: To determine the distinguishing characteristics in musculoskeletal biomechanical outcomes of the sit-to-stand and stand-to-sit activities with stroke subjects, with an emphasis on the clinical management of stroke disabilities, in a systematic review. MATERIAL AND METHODS: An extensive literature search was performed with the MEDLINE, CINAHL, EMBASE, PEDro, LILACS, and SciELO databases, followed by a manual search, to select studies on musculoskeletal biomechanical outcomes in both activities with stroke subjects, without language restrictions, and published until December/2007. RESULTS: Out of the 432 studies, only 11 reported biomechanical outcomes of both activities and none reached the total score on the selected quality parameters. The majority of the experimental studies which compared groups did not achieve acceptable scores on their methodological quality (PEDRo). The investigated conditions and interventions were also restricted. Only one study compared biomechanical outcomes between the activities, but only evaluated the time spent to perform them. Few musculoskeletal biomechanical outcomes have been investigated, being weight bearing on the lower limbs and duration of the activities the most investigated. CONCLUSION: There is little information regarding musculoskeletal biomechanical outcomes during these activities with stroke subjects and no definite conclusions can be drawn regarding the particularities of these outcomes on their performance with stroke survivors.
