A túlsúlyos és elhízott betegek kezelésében alapvető szempont az egészséges életmód elsajátítása és beépülése a beteg mindennapjaiba. A kecskeméti Irányított Betegellátási Modellkísérletben indított prevenciós program célja a túlsúlyos és elhízott betegek cardiovascularis kockázati tényezőinek csökkentése, illetve a szív- és érrendszeri megbetegedések megelőzése volt. A program keretében 2489 túlsúlyos és elhízott 18 éven felüli egyént szűrtek ki és vettek gondozásba. A betegek személyre szabott diétás és mozgásprogramot kaptak. A prevenciós programot szakképzett dietetikusok és gyógytornászok segítségével az obezitológiában jártas belgyógyász szakorvosok irányították. A betegek nyomon követése során a testsúlyváltozás mellett az eseti vérnyomásértékek alakulását, a metabolikus paramétereket és a fizikai terhelhetőséget is dokumentálták. A szerzők közleményükben a kétéves nyomon követés adatait dolgozták fel. A betegek 72%-a jutott el az egyéves követési vizsgálatig, 36%-a a kétéves vizsgálatig. Kismértékű, szignifikáns csökkenést észleltek a BMI-ben (átlagos csökkenés: 0,5605; p < 0,001), a koleszterinszintben (átlagos csökkenés: 0,23; p < 0,001). Az átlagos derékkörfogat sem az első, sem a második év folyamán nem mutatott érdemleges változást. A többi anyagcsere-paraméterben is statisztikailag jelentős csökkenést észleltek: éhomi vércukor 0,15 mmol/l (p < 0,001), ill. 0,19 mmol/l (p < 0,03), triglicerid: 0,18 (p < 0,001), ill. 0,08 mmol/l (nem szignifikáns), a HDL-koleszterin szintekben lényeges változást nem észleltek. Az első év végére a nyugalmi vérnyomás 5,9 Hgmm-rel (p < 0,001), majd a második év végére további 0,11 Hgmm-rel (nem szignifikáns) csökkent. Ez alapján a prevenciós programot sikeresnek, hatásosnak tekintjük, bár a részt vevők egészségi állapotában bekövetkezett javulás mértéke elmarad várakozásaiktól.

(1) Background: Obesity management has become an important issue due to the COVID-19 outbreak; therefore, periodic surveys on the approaches to obesity management of the entire population and target obese population are required. (2) Methods: The study used nationally representative data from the 2018 Korea National Health and Nutrition Examination Survey. Participants reported all approaches they had used to reduce or maintain weight in the past year. Data were analyzed with multiple response methods. (3) Results: The most commonly reported approach was exercise, which included fitness, yoga, biking, and other physical activities (74.7% of respondents), and the second most commonly reported approach was decreased food intake (69.6% of respondents). The use of approaches differed according to respondents’ demographic characteristics. Regarding sex-related differences, in particular, men preferred to exercise, while women were more likely to decrease food intake. Among men, exercise was highest in the 40–49 years age group (48.3%). Among women, decreased food intake was highest in the same age group (16.1%). (4) Conclusions: These findings indicate that it is necessary to introduce individualized weight management approaches and measures according to target groups in obese adults.

As the overweight and obese population increases, one must be mindful of the implications on the delivery of dental care to this group. Appropriate facilities must be available, which may warrant structural and equipment adaptations to clinical and non-clinical areas. The complexity of dental treatment planning and delivery may be compounded by medical comorbidities, and careful consideration must be given to the suitability and safety of conscious sedation and general anaesthesia in order to facilitate treatment. This article aims to discuss how safe provision and equitable access to dental care can be achieved for the bariatric population. CPD/Clinical Relevance: This article aims to discuss the challenges posed by the increase in the overweight and obese population and considerations to be taken for provision of safe and equitable bariatric dental care.

Obesity may attenuate the expression of brain-derived neurotrophic factor (BDNF), thereby increasing the risk of cognitive dysfunction. High-intensity interval exercise (HIIE) has been shown to be as or more effective than continuous moderate-intensity exercise (CME) in promoting the expression of BDNF in normal-weight individuals. Therefore, the primary purpose of this study was to examine whether or not acute HIIE could be utilized as a practical model to explore the BDNF response in obese versus normal-weight subjects when compared to acute CME. The potential relationship of exercise-induced BDNF with blood lactate and cortisol was also examined. Twelve male subjects (six obese and six normal-weight) participated in a counterbalanced and caloric equated experiment: HIIE (30 min, 4 intervals of 4 min at 80%–90% of VO2max with 3 min of active recovery at 50–60% VO2max) and CME (38 min at 50%–60% VO2max). Blood samples were collected prior to, immediately following exercise, and 1 h into recovery for measurements of serum BDNF, blood lactate, and plasma cortisol. Our results showed that the BDNF response to acute HIIE was greater than CME in obese subjects when compared to normal-weight subjects. Similarly, although acute HIIE induced greater blood lactate and plasma cortisol levels than CME, obese subjects produced less blood lactate, but no difference in cortisol than normal-weight subjects. These findings suggest that acute HIIE may be a more effective protocol to upregulate BDNF expression in an obese population, independent of increased lactate and cortisol levels. Impact statement High-intensity interval exercise (HIIE) has been shown to be a time-efficient exercise strategy that provides similar or superior physiological benefits as traditional continuous moderate-intensity exercise (CME). Our previous study demonstrated an equivalent elevation on the BDNF response in both obese and normal-weight individuals following 30 min of acute CME. To discover a time-efficient exercise strategy to improve brain health in an obese population, the present study found that obese individuals elicit a greater level of BDNF following acute HIIE versus CME than normal-weight individuals. These findings indicate that acute HIIE may be an effective strategy to upregulate BDNF expression in obese individuals.

Transnasal Humidified Rapid Insufflation Ventilatory Exchange (THRIVE) is a relatively new noninvasive oxygenation technique with a broad range of applications. It is used in the treatment of type one respiratory failure, as a preoxygenation tool, as a rescue and temporising measure in difficult airways, and as step-down oxygen therapy in patients after extubation. Its use has also been described in laryngeal surgeries, but they mainly involved normal-weight subjects or were used as a bridging oxygenation therapy before definitive airway is secured. The major benefits of using THRIVE in obese subjects undergoing laryngeal surgery include a tubeless and uninterrupted surgical field. This advantage is especially crucial in obese patients as they tend to have limited oropharyngeal space, rendering a shared airway technically challenging for surgeons. However, concerns of potential difficult airway and shorter safe apnoeic time in the obese population limit its use. In this case, we report its use as the sole oxygenation strategy in a morbidly obese patient undergoing airway surgery. Our experience suggests that THRIVE can provide a conducive operating field and adequate oxygenation in short apnoeic laryngeal procedures in the obese population, without causing excessive hypercarbia.
