AbstractHealthcare workers (HCWs) are at the frontline of the Coronavirus Disease 2019 (COVID-19) pandemic response, yet there is a paucity of literature on their knowledge, attitudes and practices (KAP) in relation to the pandemic. Community Health Workers (CHWs) in Mozambique are known locally as agentes polivalentes elementares (APEs). While technical guidance surrounding COVID-19 is available to support APEs, communicating this information has been challenging due to restrictions on travel, face-to-face group meetings and training, imposed from May to August 2020. A digital health platform, upSCALE, that already supports 1,213 APEs and 299 supervisors across three provinces, is being used to support APEs on effective COVID-19 management by delivering COVID-19 sensitive SMS messages, training modules and a COVID-19 KAP survey.The KAP survey, conducted from June 2020 to August 2020, consisted of 10 questions. Of 1,065 active upSCALE APEs, 28 percent completed the survey. Results indicate that only a small proportion of APEs listed the correct COVID-19 symptoms, transmission routes and appropriate prevention measures (25 percent, 16 percent and 39 percent, respectively) specifically included in national health education materials. Misconceptions were mainly related to transmission routes, high risk individuals and asymptomatic patients. 84 percent said they followed all government prevention guidelines. The results from the KAP survey were used to support the rapid development and deployment of targeted COVID-19 awareness and education materials for the APEs. SMS messages were continually adjusted based on the KAP survey results and a series of seven COVID-19 training modules were developed and disseminated. A follow-up KAP survey is planned for November 2020.Adapting the existing upSCALE platform enabled a better understanding, in real time, of the KAP of APEs around COVID-19 management. Subsequently, supported improved delivery of tailored messages and education, vital for ensuring a successful COVID-19 response.

Abstract Background Noncommunicable disease burden is rising in Malaysia, accounting for 72% of all deaths. Urbanization and globalization have contributed to changing patterns of diet and physical activity, creating an obesogenic environment that increases noncommunicable disease risk, especially in low-income populations. Community-based and technological interventions can play an important role in addressing structural determinants that influence noncommunicable disease burden. The Better Health Programme Malaysia aims to co-create and develop a community-based digital intervention for low-income populations to enable community stakeholders to address obesogenic environments and improve people’s knowledge, attitudes, and practices related to noncommunicable disease risk. Methods This quasi-experimental study will assess community member and community health volunteer knowledge, attitudes, and practices on noncommunicable disease prevention, risk factors, and health-seeking behavior in three geographical areas of Kuala Lumpur, each representing a different ethnicity (Malay, Indian, and Chinese). Assessment will take place before and after a 9-month intervention period, comparing intervention areas with matched control geographies. We plan to engage 2880 community members and 45 community health volunteers across the six geographic areas. A digital health needs assessment will inform modification of digital health tools to support project aims. Intervention co-creation will use a discrete choice experiment to identify community preferences among evidence-based intervention options, building from data collected on community knowledge, attitudes, and practices. Community health volunteers will work with local businesses and other stakeholders to effect change in obesogenic environments and NCD risk. The study has been approved by the Malaysian Ministry of Health Medical Research Ethical Committee. Discussion The Better Health Programme Malaysia anticipates a bottom-up approach that relies on community health volunteers collaborating with local businesses to implement activities that address obesogenic environments and improve community knowledge, attitudes, and practices related to NCD risk. The planned co-creation process will determine which interventions will be most locally relevant, feasible, and needed. The effort aims to empower community members and community health volunteers to drive change that improves their own health and wellbeing. The learnings can be useful nationally and sub-nationally in Malaysia, as well as across similar settings that are working with community stakeholders to reduce noncommunicable disease risk. Abstract. The physical aspects and knowledge of soil erosion in six communities in rural Chiapas, Mexico were assessed. Average erosion rates estimated with the RUSLE model ranged from 200 to 1,200 ha−1 yr−1. Most erosion rates are relatively high due to steep slopes, sandy soils and bare land cover. The lowest rates occur where corn is cultivated for much of the year and slopes are relatively low. The results of a knowledge, attitudes and practices (KAP) survey showed that two-thirds of respondents believed that the major cause of soil erosion was hurricanes or rainfall and only 14 % of respondents identified human activities as causes of erosion. Forty-two percent of respondents indicated that the responsibility for solving soil erosion problems lies with government, as opposed to 26 % indicating that the community is responsible. More than half of respondents believed that reforestation is a viable option for reducing soil erosion, but only a third of respondents were currently applying reforestation practices and another one-third indicated that they were not following any conservation practices. The KAP results were used to assess the overall level of knowledge and interest in soil erosion problems and their solutions by compiling negative responses. The community of Barrio Vicente Guerrero may be most vulnerable to soil erosion, since it had the highest average negative response and the second highest soil erosion rate. However, Poblado Cambil had the highest estimated soil erosion rate and a relatively low average negative response rate, suggesting that soil conservation efforts should be prioritized for this community. We conclude that as long as the economic and productive needs of the communities are not solved simultaneously, the risk of soil erosion will increase in the future, which threatens the survival of these communities.

AbstractDigital Health-Enabled Community-Centered Care (D-CCC) represents a pioneering vision for the future of community-centered care. Utilizing an artificial intelligence-enabled closed-loop digital health platform designed for, and with, community health workers, D-CCC enables timely and individualized delivery of interventions by community health workers to the communities they serve. D-CCC has the potential to transform the current landscape of manual, episodic and restricted community health worker-delivered care and services into an expanded, digitally interconnected and collaborative community-centered health and social care ecosystem which centers around a digitally empowered community health workforce of the future.

Background. Biomedical waste (BMW) is defined as unwanted materials generated during diagnosis, treatment, operation, immunization, or in research activities including production of biologicals. Healthcare workers are responsible for the proper management of this waste for human safety and for the protection of the environment. Methods. An analytical knowledge, attitude, and practice (KAP) study was carried out at Biyem-Assi District Hospital from June 1st to July 5th, 2018, including 100 health workers from different departments. Variables of interest were knowledge, attitudes, and practices of the respondents. A structured and pretested questionnaire was used for data collection. Data analysis was carried out using software Epi Info version 7.2.2.6. Logistic regression was used to establish the relationship between knowledge, attitudes, and practices. Results. Nurses constituted 32.0% of the participants, and more than half of the participants had 1–4 years of working experience (56.0%). Overall, the level of knowledge was satisfactory at 50.0%, that of attitudes was as unfavorable at 83.0%, and that of practices was as poor at 50.0%. Favorable attitudes were associated to satisfactory level of knowledge (ORa = 5.14 [3.10–8.51] and p=0.005). Good practices were associated to good level of knowledge (ORa = 5.26 [3.17–8.7] and p<0.001) and a favorable attitude (ORa = 7.30 [2.25–23, 71] and p<0.001). Conclusion. The level of knowledge was considered unsatisfactory for half of the staff interviewed. Attitudes were unfavourable at 83.0% and poor practices at 50.0%. Staff with a good level of knowledge were more likely to have favourable attitudes towards BWM. Also, good knowledge and attitude positively influenced the practice with regard to BMW management.
